feat(evidence): enforce the traceability_events mutation policy and derive waiver approvers from the principal - #3717
Follow-up to the FSMA 204 readiness gap analysis (#3701), stories S4 and S5. Lane: backend (persist evidence). Sibling PRs: #3713 (S1 + S2), #3714 (S3 + S7).
S4, fsma.traceability_events mutation policy. The canonical FSMA 204 evidence table had no database-level guard: any role with UPDATE could rewrite the payload the chain hash covers, while fsma.cte_events has had an append-only trigger since v107. Migration v141 (08c7cf36ddbe, revises b8c1e4f2a901) adds a BEFORE UPDATE OR DELETE row trigger and a BEFORE TRUNCATE statement trigger that allow exactly what the production writers do today: the amendment path in canonical_persistence/writer.py (statusactive -> superseded, amended_at) and the rules-engine markers in rules/engine.py (evaluation_verdict, no_verdict_reason, evaluation_error, evaluated_at). Every other column change, any other status transition, DELETE and TRUNCATE raise check_violation. The comparison is to_jsonb(NEW) - allowlist = to_jsonb(OLD) - allowlist, so it holds on every catalog variant of the table (the Supabase twin has tenant_id TEXT and no ingested_at) and keeps covering columns added later. Break-glass is the existing transaction-local fsma.allow_mutation GUC (v095/v107 pattern; reset per transaction and per pooled connection; the retention purge is its only production user). No grant changes; idempotent (CREATE OR REPLACE, DROP TRIGGER IF EXISTS, to_regclass guard); downgrade removes both triggers and both functions. Rechain after feat(storage): partner file metadata + 30-day retention schema (v140) #3729 landed on main: a3f7ce361b0e → e7a1c9d2f4b0 (v139) → b8c1e4f2a901 (v140) → 08c7cf36ddbe (v141). Files: alembic/versions/20261028_08c7cf36ddbe_v141_traceability_events_append_only.py (renamed from …_v138_…); tests/migrations/test_v141_traceability_events_append_only.py (renamed from test_v138_…).
docs/engineering/SCHEMA_CHANGE_POLICY.md §5 gains the sanctioned mutation surface (5.1) and the SEL-589 read freeze on fsma.cte_events and fsma.critical_tracking_events (5.2, per the 2026-10-27 decision of record); appendix bumped to 1.2.0.
S5, waiver approver from the principal.PATCH /api/v1/exceptions/{case_id}/waive took waiver_approved_by from the request body, so a caller could sign a waiver in anyone's name. The approver is now the authenticated principal's key_id (session:<user id> for browser sessions, the API key id for scoped keys), 401 when empty, the same rule _audit_identity in the FDA export router follows. WaiveRequest drops the field; a body-supplied value is ignored. frontend/src/hooks/use-control-plane.ts still sends it and is unaffected (pydantic drops unknown fields); it can be removed in a frontend follow-up. resolve and comments keep their body-supplied actors for SEL-463.
Risk notes for the reviewer
The rules-engine UPDATEs run inside savepoints that swallow errors, so a wrong allowlist would have silently dropped verdicts. The allowlist is pinned by a static test against the exact SET ... text of both writers, and the behavioural tests run both shapes.
The amendment UPDATE has no savepoint; a trigger error there would abort the ingest transaction. It is covered by test_active_to_superseded_is_allowed, which runs the writer's exact statement.
Column grants: v111 still grants UPDATE to the runtime role (and scripts/apply_runtime_role_cutover.py rebuilds grants from v111 after every deploy). The trigger is the enforcement; narrowing grants to the allowlisted columns is a defence-in-depth follow-up for v111, not this PR.
No Supabase SQL twin: the earlier mutation-trigger migrations (v095, v107) have none either; the trigger SQL is catalog-neutral.

New tests/migrations/test_v141_traceability_events_append_only.py (4 static + 18 behavioural): revision chain and shape; trigger SQL covers UPDATE, DELETE, TRUNCATE with the break-glass GUC; allowlist matches the writers' statements; downgrade removes everything; triggers installed at head; verdict and error-marker updates allowed; active -> superseded allowed; superseded -> active, rejected, draft rejected; payload column updates rejected (quantity, event_timestamp, traceability_lot_code, kdes, sha256_hash, tenant_id); a payload change hidden behind a marker update rejected; DELETE rejected; TRUNCATE rejected; break-glass UPDATE and DELETE allowed; break-glass is transaction-local; re-apply idempotent and downgrade removes triggers and functions.
services/ingestion/tests/test_exception_router.py: approver is the principal's key_id; body-supplied approver ignored; missing reason is 422; empty principal identity is 401.
Run locally against a scratch PostgreSQL 16 upgraded to head (all green):
Live suites touched by the GUC change, against the same database at v141: tests/invariants/test_backfill_canonical.py (12 passed), tests/invariants/test_verify_canonical_parity_live.py -k drift (passed), test_disaster_recovery_data_volume_pg.py, test_auditor_pagination_ties_pg.py, test_product_catalog_derive_events_pg.py (passed with REMEDIATION_TEST_SUPERUSER_DSN). Also scripts/check_trust_safety_copy.py, scripts/check_docs_links.py, ruff check --select I,E9,F63,F7,F82, git diff --check.
Not run here: tests/shared/test_postgres_evidence_workflow_pg.py needs the dedicated unix-socket cluster the CI job provisions; its tamper test now wraps the UPDATE in w.admin.transaction() with the GUC, so CI will exercise it.

Relates to #3701 (gap analysis rows 11 and 14, stories S4 and S5), SEL-589 (read freeze recorded in the policy), SEL-463 (remaining body-supplied actors on resolve and comments).
High Risk
Touches FDA evidence integrity (new triggers on the canonical table) and authentication/signoff for waivers; misconfigured allowlist or trigger behavior could break ingest or rules evaluation at runtime.
Overview
Adds database-level append-only enforcement on fsma.traceability_events (migration v141): row triggers block unauthorized UPDATE/DELETE and a statement trigger blocks TRUNCATE, while still allowing the amendment and rules-engine column updates already used in production. Break-glass remains SET LOCAL fsma.allow_mutation = 'true' (retention purge and tests).
Exception waiver API (S5):PATCH …/waive no longer accepts waiver_approved_by in the body; the approver is taken from the authenticated principal’s key_id, with 401 when identity is empty. Client-supplied approver fields are ignored.
Policy/docs:SCHEMA_CHANGE_POLICY.md documents the sanctioned mutation surface (§5.1), legacy CTE read freeze (§5.2), and bumps schema version to 1.2.0.
Tests: New migration suite test_v141_traceability_events_append_only.py; router and live waiver-replay tests; existing Postgres fixtures that DELETE or simulate tampering on canonical events now set the break-glass GUC.

…erive waiver approvers from the principal
S4 (fsma.traceability_events mutation policy). The canonical FSMA 204
evidence table had no database-level guard: any role with UPDATE could
rewrite the payload the chain hash covers, while fsma.cte_events has
had an append-only trigger since v107. Migration v138 (08c7cf36ddbe)
adds a BEFORE UPDATE OR DELETE row trigger and a BEFORE TRUNCATE
statement trigger that allow exactly what the production writers do:
the amendment path (status active -> superseded, amended_at) and the
rules engine markers (evaluation_verdict, no_verdict_reason,
evaluation_error, evaluated_at). Every other column change, any other
status transition, DELETE and TRUNCATE raise check_violation. The
comparison is to_jsonb(NEW) - allowlist = to_jsonb(OLD) - allowlist, so
it holds on every catalog variant of the table and keeps covering
columns added later. Break-glass is the existing fsma.allow_mutation
GUC, transaction-local; no grant changes.
Six live tests that legitimately mutate the table (tamper simulations,
cleanups, an orphaning helper) now set the GUC inside their
transaction. SCHEMA_CHANGE_POLICY.md section 5 gains the sanctioned
mutation surface (5.1) and the SEL-589 read freeze on fsma.cte_events
and fsma.critical_tracking_events (5.2).
S5 (waiver approver from the principal). PATCH /exceptions/{id}/waive
took waiver_approved_by from the request body, so a caller could sign a
waiver in anyone's name. The approver is now the authenticated
principal's key_id (401 when empty), the same rule the FDA export audit
identity follows; a body-supplied value is ignored. The frontend still
sends the field and is unaffected.
Tests: static and behavioural v138 tests against a live PostgreSQL
(allowlisted updates, the one status transition, rejected payload
updates and transitions, DELETE, TRUNCATE, break-glass, transaction
locality, idempotent re-apply, downgrade), chain health, revision
checker; exception router tests for the derived approver, ignored body
field, missing reason and empty principal. Live suites touched by the
GUC change pass against the scratch database at v138.

It looks like this branch is under active development. To avoid overwhelming you with review comments due to an influx of new commits, CodeRabbit has automatically paused this review. You can configure this behavior by changing the reviews.auto_review.auto_pause_after_reviewed_commits setting.
The change adds an Alembic migration that restricts updates, deletes, and truncates on fsma.traceability_events, with specified update exceptions and a transaction-local override. It documents mutation rules and read-only retention tables. New and updated tests cover trigger enforcement, override behavior, and cleanup. The waiver endpoint now uses the authenticated principal’s key_id as the approver and returns HTTP 401 when that identity is empty. A live test checks that replaying a waiver does not duplicate its signoff or status-change comment.
The new guard restricts ordinary writes, but application-role SQL can still bypass it. No newly introduced merge-blocking risk was established.
Security architecture risk: Low · up to 6c270
The changes strengthen evidence integrity and waiver attribution without an identified new security vulnerability. Existing write paths remain compatible. The mutation override is not independent authorization against a compromised database session, and deployed privileges and live rollout behavior were not verified.
Retained concerns
No architecture-level concerns identified.
Security review details
Security Blast Radius
inferred — The affected security assets are canonical evidence rows and waiver audit records. Waiver authority remains bounded by exceptions.write and the authenticated tenant. Direct database mutation exposure remains bounded by the caller's existing grants and accessible rows; enabling the override does not itself grant DELETE, TRUNCATE, or additional tenant access.
Security Findings and Attack Paths
inferred — The mutation override is not an independent authorization boundary against a SQL-writing caller: the trigger accepts the custom setting without a role or approval check. This does not establish a new PR vulnerability because the base already granted runtime UPDATE with no canonical-table mutation guard. No newly reachable attack path was identified.
Trust Boundaries and Controls
observed — Waiver attribution now crosses the authentication boundary rather than trusting request-body identity. Browser sessions supply session-prefixed user identity and API keys supply credential identity. Shared API keys therefore identify the credential, not necessarily an individual human approver.
Resilience and Maintainability Implications
observed — The existing transaction-begin hook resets fsma.allow_mutation to false transaction-locally, and the retention path enables it transaction-locally before committing. These controls limit accidental carryover; they do not authenticate a caller that deliberately enables the setting within its own transaction.
Hardening Proposals
proposed — If evidence immutability must withstand a compromised runtime SQL session, authorize the override through a separately restricted maintenance role or trusted operation rather than treating a caller-settable flag as sufficient authority.

Claude-Session: https://claude.ai/code/session_01GyhDYG3ddQvVKyu2qtYfCj
The alembic catalog declares traceability_events.status NOT NULL, but
the Supabase twin leaves it nullable. With a NULL on either side,
NOT (OLD.status = 'active' AND NEW.status = 'superseded') evaluates to
NULL rather than TRUE, so the trigger let active -> NULL and
NULL -> active through without break-glass; a NULL status would drop
the row from the status = 'active' predicates the recall and FDA
exports use. The guard now uses IS NOT TRUE.
Behavioural test drops the NOT NULL constraint inside transactions the
rejection rolls back, covers both directions, and checks the constraint
is intact afterwards. Static test pins the null-safe form. Reproduced
(DID NOT RAISE) before the change; 23 v138 tests pass after it against
a scratch PostgreSQL 16 re-upgraded to v138.

Downgrade: drops both triggers and both functions — reversible; no data loss (trigger-only).
Allowlist matches production writers byte-for-byte:
canonical_persistence/writer.py: SET status = 'superseded', amended_at = NOW()
rules/engine.py: SET evaluation_verdict…, SET evaluation_error… (+ evaluated_at)
Status guard uses IS NOT TRUE (null-safe for Supabase twin). Payload/tenant_id/DELETE/TRUNCATE rejected; break-glass is SET LOCAL fsma.allow_mutation (same GUC as v095/v107; runtime resets per txn/pool).
No new tenant table → no new RLS/FORCE RLS required; existing table RLS unchanged. No grant narrowing (documented defence-in-depth follow-up).
S5 waiver approver
WaiveRequest drops waiver_approved_by; approver = principal key_id via _actor_from_principal (401 if empty). Body spoof ignored (pydantic extra ignore). Mirrors FDA export _audit_identity pattern. resolve/comments body actors left for SEL-463 (documented).
test_exception_router.py -k waive (4): PASS (principal approver, body spoof ignored, missing reason 422, empty principal 401).
Behavioural Postgres upgrade/downgrade: not run here (no Docker/Postgres on this box). Rely on CI Live DB suite + seeded migration rehearsals + the 18 behavioural cases in test_v141_traceability_events_append_only.py. Touched live fixtures correctly set break-glass GUC for cleanup/tamper simulation (synthetic tenants only).

Protect the break-glass override with authorization.
regengine has UPDATE privilege on fsma.traceability_events, and the canonical writer reaches this table through an ordinary UPDATE. PostgreSQL allows that role to set a custom two-part GUC with SET LOCAL. The trigger then returns NEW before checking the operation or changed columns.
A non-break-glass regengine session can therefore set fsma.allow_mutation = 'true' and update protected evidence columns. Gate the override through a dedicated trusted role or SECURITY DEFINER function. Do not use the GUC value alone as authorization.

Verified, and the path is real: v111 grants regengine UPDATE on fsma.traceability_events, PostgreSQL lets any role SET LOCAL an unreserved two-part GUC, and the trigger returns NEW as soon as fsma.allow_mutation = 'true'. So a session that already holds the application role can rewrite evidence columns without any further privilege.
What that path requires is raw SQL on the application role: leaked regengine credentials, or an injection that yields statement execution. Either already gives full read and write of every tenant's data, so the trigger is not the boundary that holds against it today. What the trigger does hold against is the realistic case this PR targets: an ORM or hand-written UPDATE in application code, a migration, or an operator session that forgets the policy. The repository's existing mutation guards (v095 on supplier remediation, v107 on fsma.cte_events) use this same GUC with the same semantics, and this migration deliberately matches them so the retention purge (the only production caller) keeps one break-glass contract. Two mitigations stay in place on the bypass path: the trigger logs RAISE WARNING with TG_OP, current_user and the event id to the PostgreSQL log, and sha256_hash plus the chain verification in /export/verify still detect an edited payload.
Gating the override behind a dedicated role or a SECURITY DEFINER function is the right hardening, but it is not a change I can make in proportion inside this PR: it needs a new role and grant in v111 (which scripts/apply_runtime_role_cutover.py rebuilds after every deploy), the scheduler's purge path moved onto that role or wrapped in the definer function, and the same treatment for v095 and v107 so the three guards do not diverge. That is the column-grant narrowing follow-up the PR description already names. I am leaving this thread open for the founder's call on whether to take it as a follow-up or fold it in here.
The reason will be displayed to describe this comment to others. Learn more.
@PetrefiedThunder Understood. The stated scope is protection against accidental mutations, not against a session that can execute arbitrary SQL as regengine. Matching v095 and v107 explains the compatibility constraint.
The authorization finding remains valid. RAISE WARNING provides logging, and hash-chain verification provides detection; neither authorizes the override or prevents the mutation. This does not negate the protection v141 adds against accidental updates.
I agree that the founder should decide whether to include the hardening here or accept the risk for a follow-up. I recommend recording that decision before merging. The thread should remain open pending that decision.
For the follow-up, the key verification is that regengine cannot modify protected columns even after setting fsma.allow_mutation, while the authorized retention purge still works after runtime-role cutover.
Would you like me to open a GitHub issue for that coordinated hardening?
The reason will be displayed to describe this comment to others. Learn more.
The hardening now exists as #3764 (stacked on this branch, v142): fsma.break_glass_authorized(oid) honours the GUC only for a superuser or a member of the table owner, so the runtime roles cannot arm the break-glass with raw SQL; the retention purge moves to a SECURITY DEFINER arm scoped to one tenant, fsma.cte_events, DELETE only, rows past the FSMA floor; all three guard families (v095, v107, v141) get the same check; v111's inventory carries the grants; a live test runs the runtime role via SET LOCAL ROLE regengine and asserts the bypass is refused while the sanctioned verdict UPDATE still works. This thread stays open for the founder to choose: merge #3717 as is and take #3764 right after, or hold #3717 until #3764 is reviewed.
The reason will be displayed to describe this comment to others. Learn more.
@PetrefiedThunder Understood. #3764 is the reported fix for this finding, but it does not change the bypass in #3717 alone. I have not independently verified #3764.
This thread should remain open for the founder’s merge-order decision. If the founder chooses to merge #3717 first, record explicit acceptance of the remaining bypass until v142 is deployed.
Before closing the finding, verify that runtime roles cannot bypass the guards after runtime-role cutover, sanctioned verdict updates still work, and the retention purge enforces its tenant, table, operation, and retention limits.
I couldn't check this conversation for new learnings, so no new learning was saved.

before the trigger DDL (same bound as v134), so a CREATE/DROP TRIGGER
queued behind a long ingest or export transaction fails fast and can be
retried instead of holding every later writer behind it.
- The migration tests assert `exc.value.orig.sqlstate == "23514"` with no
fallback; rls_db forces psycopg 3, which always exposes it.
- New live test tests/shared/test_exception_waiver_replay_live.py drives
the real router and ExceptionQueueService against a schema-at-head
PostgreSQL: PATCH /waive twice for one case leaves exactly one signoff
(signed by the principal, never the body) and one status-change comment.

…ted by v141
Cursor Bugbot on #3717 claimed to_jsonb(NEW) drops SQL NULL fields, so a
non-allowlisted column could be cleared past the trigger. Reproduced
against a scratch PostgreSQL at v141: to_jsonb keeps a NULL column as a
null-valued key, and clearing a populated sha256_hash, chain_hash,
product_reference or from_facility_reference raises check_violation,
including behind a sanctioned status or verdict update. The only
"allowed" case is NULL -> NULL, which changes nothing. This test
populates those columns through break-glass and pins the rejection.

Test committed replay across request boundaries.
The test injects one Session into both requests, so the replay reads the first request’s uncommitted changes. Seed the tenant and exception in setup, commit that setup, and use separate request-scoped sessions so each request exercises the production commit boundary. Run assertions from a separate session.
Because setup and request transactions will then commit, add teardown that deletes the dependent signoffs and comments, the exception, and the uniquely seeded tenant in foreign-key order. Run teardown in a finally block so the test leaves no rows.

Not a merge-only delta — two PR-side commits plus a main merge:
ca410da — SET LOCAL lock_timeout = '10s' on v141 upgrade/downgrade (same bound as v134); migration tests assert exc.value.orig.sqlstate == "23514" with no fallback; new live waiver-replay test proves PATCH /waive twice leaves one signoff (principal, never body) and one status-change comment.
468081c — merge origin/main at fb4e5df (second parent). No alembic conflict; PR migration changes carried through cleanly. Main tip has since moved to c581466 (4 commits, no new alembic files).
6c27050 — pins that clearing a populated protected column to NULL (alone or behind a sanctioned status/verdict update) is rejected. Correctly addresses the Bugbot to_jsonb NULL-drop claim; trigger uses jsonb_each key-join so null-valued keys still diff.
Alembic
Single head: 08c7cf36ddbe (scripts/check_alembic_revisions.py: OK — unique IDs, single head, no orphans).
tests/migrations/test_v141_traceability_events_append_only.py static: 1 passed; behavioural DB cases skipped (no scratch DB / testcontainers on box)
tests/shared/test_exception_waiver_replay_live.py: skipped (no designated live DB)
Behavioural DB coverage left to CI (Live DB suite + Test shared / Test ingestion already green on this head)
CI (gh pr checks 3717)
Required substantive checks green (PR Gate Backend/Frontend/Secrets, Live DB suite, Test shared/ingestion/admin/…, Alembic revision ID guard, seeded migration rehearsals, SAST, CodeQL, Cursor Bugbot, CodeRabbit). Ignoring gate-only fails: Engine Review Pass, Head SHA / CI Status Gate / PR Blocker Checklist (waiting on this non-author PASS). Frontend E2E not red on this head (Passwordless Auth E2E + PR Gate Frontend pass).
Concerns
None blocking. Soft note: main tip c581466 is 4 commits past the merge parent; none touch alembic, so no rechain required before merge, but a fresh main merge would still be polite if merge waits.

DeepSeek review on #3771:
- Single alembic head: vendor #3717 v141 (08c7cf36ddbe) and set
supplier_facilities.phone migration down_revision to that head as v142.
- Add public.supplier_facilities.phone to expected_schema.json parity snapshot.
- Pass phone= in bulk graph sync record_facility_ftl_scoping so Neo4j does
not clear phone on facility FTL scoping writes.
- Food Safety Lens: location complete = name+phone+address only; bare GLN
flags via has_unresolved_gln (no reject / A-2170).
Note: supabase mirror migration remains forward-only.

#3771 previously vendored #3717's fsma.traceability_events append-only
trigger as v141 and bumped the phone ADD COLUMN to v142. Live DB suite then
hit CheckViolation on intentional tamper UPDATEs — caused by the append-only
policy, not by phone backfill (phone never UPDATEs that table).
Remove the vendored append-only migration and rechain supplier_facilities.phone
as v141 revising v140 (b8c1e4f2a901). Append-only stays on #3717.

Merging main (6887ca5) brought in pins that the Security Audit,
Production Readiness Gate and per-service pip-audit jobs now reject:
pypdf 6.21.2, pyasn1 0.6.4 and follow-redirects 1.16.0-1.16.1. Main is
red on the same jobs. This ports #3773 byte for byte (pypdf 6.21.4,
pyasn1 0.6.5, npm overrides follow-redirects >=1.16.2 and minimatch
>=11.0.1), as already done on #3767. It no-ops once main carries #3773.

Behind current main a67a778 by 1 commit (#3792 pr-guards.yml only). Does not touch this PR's v141 files; required content CI already green. Optional Lead update for gate hygiene.
Merge commit only (no rebase). #3717 was squash-merged and its branch
deleted, so GitHub retargets this PR to main. One conflict, in
docs/engineering/SCHEMA_CHANGE_POLICY.md 5.1: main carries #3717's
paragraph, which is byte-identical to this branch's base 6c27050, and
this branch rewrote that paragraph. Resolved by keeping this branch's
version. The diff against main is the same six files as before.

Summary
Follow-up to the FSMA 204 readiness gap analysis (#3701), stories S4 and S5. Lane: backend (persist evidence). Sibling PRs: #3713 (S1 + S2), #3714 (S3 + S7).
fsma.traceability_eventsmutation policy. The canonical FSMA 204 evidence table had no database-level guard: any role with UPDATE could rewrite the payload the chain hash covers, whilefsma.cte_eventshas had an append-only trigger since v107. Migration v141 (08c7cf36ddbe, revisesb8c1e4f2a901) adds aBEFORE UPDATE OR DELETErow trigger and aBEFORE TRUNCATEstatement trigger that allow exactly what the production writers do today: the amendment path incanonical_persistence/writer.py(statusactive -> superseded,amended_at) and the rules-engine markers inrules/engine.py(evaluation_verdict,no_verdict_reason,evaluation_error,evaluated_at). Every other column change, any otherstatustransition, DELETE and TRUNCATE raisecheck_violation. The comparison isto_jsonb(NEW) - allowlist = to_jsonb(OLD) - allowlist, so it holds on every catalog variant of the table (the Supabase twin hastenant_id TEXTand noingested_at) and keeps covering columns added later. Break-glass is the existing transaction-localfsma.allow_mutationGUC (v095/v107 pattern; reset per transaction and per pooled connection; the retention purge is its only production user). No grant changes; idempotent (CREATE OR REPLACE,DROP TRIGGER IF EXISTS,to_regclassguard); downgrade removes both triggers and both functions. Rechain after feat(storage): partner file metadata + 30-day retention schema (v140) #3729 landed on main:a3f7ce361b0e→e7a1c9d2f4b0(v139) →b8c1e4f2a901(v140) →08c7cf36ddbe(v141).
