Summary

Follow-up to the FSMA 204 readiness gap analysis (#3701), stories S3 and S7. Lane: backend (export). Sibling PRs: #3713 (S1 + S2).

  • S3, EPCIS export limit stated. GET /api/v1/epcis/export silently cut a tenant's events at a literal LIMIT 2000, and the product_id filter ran after the cut, so a short list did not prove completeness. The cap is now a named constant (EXPORT_EVENT_LIMIT = 2000), the query reads limit + 1 rows through a bound :row_limit, and a new _list_events_from_db_detailed returns EpcisExportPage(events, truncated, limit); _list_events_from_db wraps it. Response metadata gains limit, truncated and ordering ("event_timestamp desc"); on the uncapped in-memory fallback limit is null and truncated is false. truncated is decided before the Python-side product filter, so it means "more matching rows exist", not "fewer than the cap came back". The tenant-scope allowlist entry moves to the new function with the recomputed query hash. Customer copy (customer-readiness.ts) and docs/PRODUCT_DESCRIPTION.md say the same thing.
  • S7, FDA package carries rule versions and known gaps. The evidence ZIP said nothing about which rules, at which versions, judged its records, and the allow_incomplete coverage-gate bypass left only a log line. New shared.rules.versions.snapshot_rule_versions(db, tenant_id, event_ids) returns the rule catalog, the distinct (rule_id, rule_version) pairs evaluated for the exported events within the tenant, and the enforcement mode; the request-workflow assembler now uses it for the catalog it always stored (same shape, same single query). _enforce_kde_coverage_gate returns whether it was bypassed. _build_fda_package writes a hashed evidence_context_{scope}_{ts}.json member ({rule_versions, known_gaps}) listed in manifest.files and under the Merkle root, adds a manifest.evidence_context pointer with at-a-glance counts, and describes it in README.txt. All four package call sites (/export, /export/all, /export/v2, recall) pass the snapshot and the gate result. Snapshot failures degrade to a stated "unavailable" instead of failing the export.
  • Docs. docs/runbooks/FDA_RECALL_RESPONSE.md "What's in the Package" now lists the real members (it named a PDF and a completeness_summary.json that were never in the ZIP).

Known gaps the package now admits to

KDE coverage ratio, events missing required fields (and that the per-event list is capped at 250), unchecked event types, missing record/chain hashes, chain verification status, whether the coverage gate was bypassed, rules enforcement mode, catalog/evaluated snapshot status, plus three fixed notes (integrity vs. source accuracy, advisory mode, paper records permitted).

Compatibility

  • The CSV bytes and the chain-verification member are unchanged. The ZIP gains one member, so the Merkle root and package hash of a package built today differ from one built before this change for the same data. Pre-production; no customer bundles exist.
  • Both public verifiers (/api/v1/proof/verify, frontend/public/sdk/verify_chain.py) and services/admin/scripts/verify_audit_pack.py accept the new member unchanged and reject a tampered one (tests added). The verifiers only require exactly one fda_spreadsheet_*.csv and one chain_verification_*.json; the new name starts with neither prefix.
  • _list_events_from_db keeps its list-returning signature.

Type of Change

  • Bug fix
  • New feature
  • Enhancement to existing feature
  • Refactor (no behavior change)
  • Documentation
  • CI/CD or infrastructure
  • Security fix

Test Plan

  • Unit tests added/updated
  • Integration tests pass
  • Manual testing performed
  • N/A (docs, config, etc.)

Added or extended:

  • services/ingestion/tests/test_epcis_persistence_1342.py: bound LIMIT :row_limit (no literal), default cap, limit + 1 row sets truncated and is dropped, exactly-limit not truncated, truncation decided before the product filter, wrapper returns a list, non-positive limit rejected.
  • services/ingestion/tests/test_epcis_router_1342.py / test_epcis_qa5_edge_cases.py: metadata limit/truncated/ordering on the DB path, limit: null on the fallback; stubs moved to _list_events_from_db_detailed.
  • services/shared/tests/test_rule_versions_snapshot.py (new): catalog only without event ids, tenant-scoped evaluated query and params, degrade-not-raise on either query, enforcement mode from env.
  • services/ingestion/tests/test_fda_export_service_1342.py: evidence member hashed, sized, in files[] and in merkle.leaves; manifest pointer; written even without a snapshot; gate bypass recorded; _build_known_gaps cap statement and partial-input tolerance; _rule_versions_for_events id extraction and degradation.
  • services/ingestion/tests/test_proof_verify_router.py: real package with the member passes /api/v1/proof/verify and the SDK; a flipped known_gaps.coverage_gate_bypassed fails both.
  • services/admin/tests/test_verify_audit_pack.py: member declared, pack passes, tampered member fails.
  • test_fda_export_router_1342.py (gate return values), test_fda_export_formatters_1342.py (kwargs forwarded and defaults), test_fda_export_recall_1342.py (recall package receives both).

Run locally (all green):

python3 -m pytest services/shared/tests/test_rule_versions_snapshot.py services/ingestion/tests/test_fda_export_*.py services/ingestion/tests/test_proof_verify_router.py services/ingestion/tests/test_epcis_*.py services/ingestion/tests/test_request_workflow_router*.py services/admin/tests/test_verify_audit_pack.py --no-cov
python3 scripts/security/scan_tenant_scope.py
python3 scripts/check_trust_safety_copy.py && python3 scripts/check_docs_links.py

Also: ruff check --select E9,F63,F7,F82 --line-length 120 clean on touched files; eslint clean on customer-readiness.ts; homepage vitest passes; both snapshot queries executed against a local PostgreSQL 16 at alembic head (a3f7ce361b0e).

Not run here: the copy gate's --gate mode compares against the main baseline, which this environment cannot fetch; the new EPCIS copy string keeps "Preview" in the same string, which is what the epcis-unlabeled rule checks.

Checklist

  • My code follows the existing patterns in the codebase
  • I have not introduced hardcoded secrets or credentials
  • If this PR adds a test that uses sys.modules[X] = stub, I justified it in the PR description (none added; the recall test harness already does this)
  • I have updated relevant documentation (if applicable)
  • My changes generate no new warnings or lint errors (one pre-existing import-order finding in fda_export/router.py is untouched)

Related Issues

Relates to #3701 (gap analysis rows 13 and 14, stories S3 and S7). Rows 13 and 14 of that doc will be updated in a follow-up docs PR once both merge.

Note

Medium Risk
Changes FDA recall export artifacts and EPCIS export semantics; behavior is covered by tests and rule snapshots degrade without failing exports.

Overview
EPCIS export now exposes an explicit 2,000-event cap (newest first) instead of silently truncating at a literal SQL LIMIT. The DB path uses _list_events_from_db_detailed with a bound :row_limit (fetches one extra row to detect cutoff) and returns regengine:limit, regengine:truncated, and regengine:ordering in export metadata; the in-memory fallback reports limit: null. Truncation is decided before the post-query product_id filter so a short page can still mean “more rows exist.” Product copy and the tenant-scope allowlist follow the renamed query.

FDA ZIP packages gain a hashed evidence_context_*.json member (under the Merkle root) with rule catalog + evaluated rule versions via new shared.rules.versions.snapshot_rule_versions, plus a known_gaps block (KDE coverage, bypassed coverage gate, hash gaps, enforcement mode, etc.). _enforce_kde_coverage_gate now returns whether allow_incomplete was used so that bypass is recorded in the package, not only in logs. All package export paths pass the snapshot and gate flag; request-workflow assembly reuses the same catalog snapshot helper. Runbook and product docs describe the real ZIP contents and the EPCIS cap.

Package Merkle roots change when built with the new member (pre-production); existing verifiers accept the new file and reject tampering to known_gaps.

…known gaps in the FDA package

S3 (EPCIS export limit stated). GET /api/v1/epcis/export silently cut a
tenant's events at a literal LIMIT 2000 and the product_id filter ran
after the cut, so a short list did not prove completeness. The cap is
now a named constant (EXPORT_EVENT_LIMIT), the query reads limit + 1
rows through a bound parameter, and a new _list_events_from_db_detailed
returns an EpcisExportPage(events, truncated, limit); the list-returning
helper wraps it. The response metadata states limit, truncated and
ordering, with limit null and truncated false on the uncapped in-memory
fallback. The tenant-scope allowlist entry moves to the new function
with the recomputed query hash. Customer copy and the product
description say the same thing.

S7 (FDA package carries rule versions and known gaps). The evidence ZIP
said nothing about which rules, at which versions, judged its records,
and the allow_incomplete coverage-gate bypass left only a log line.
New shared.rules.versions.snapshot_rule_versions returns the rule
catalog, the distinct (rule_id, rule_version) pairs evaluated for the
exported events within the tenant, and the enforcement mode; the
request-workflow assembler now uses it for the catalog it always
stored. _enforce_kde_coverage_gate returns whether it was bypassed.
_build_fda_package writes a hashed evidence_context_{scope}_{ts}.json
member ({rule_versions, known_gaps}) listed in manifest.files and under
the Merkle root, adds a manifest pointer with at-a-glance counts, and
describes it in README.txt. All four package call sites (/export,
/export/all, /export/v2, recall) pass the snapshot and the gate result;
snapshot failures degrade to a stated "unavailable" rather than failing
the export. Both public verifiers reject a tampered evidence member.

Docs: the recall runbook package table now matches the real members.

Tests: EPCIS cap and truncation (persistence, router, fallback),
snapshot unit tests with a fake session, evidence member hashing and
Merkle coverage, gate bypass recorded, verifier round trips and tamper
detection (proof/verify, SDK, admin verifier), formatter and recall
kwarg forwarding. Snapshot SQL executed against a local PostgreSQL at
alembic head.

Walkthrough

EPCIS database exports now return up to 2,000 events and report the limit, truncation status, and ordering. FDA export packages now include evidence context with rule-version snapshots and known-gap data; this context is included in package integrity checks. The test API key script uses a stat permission constant for newly created parent directories.

Priority: Normal

Estimated code review effort: 4 (Complex) | ~45 minutes

Merge Risk: Moderate · up to 68b5c

An EPCIS export that hit the 2,000-event cap can be reported as complete when a product filter removes every event from the page, which misleads recall reviewers. A rule-catalog read failure on PostgreSQL can also spoil the rule-version evidence and later queries in the same request. Fix both before merging; the remaining items are documentation and test follow-ups.

Security architecture risk: Low · up to 68b5c

The changes preserve inspected tenant boundaries and improve evidence visibility. However, a fallback path can incorrectly report an incomplete export as untruncated. That path is disabled by default in production, limiting exposure. Some failure-handling and deployment coverage remains incomplete.

Retained concerns

  • Medium · reliability · observed: A successful database export can lose its incompleteness signal at the fallback boundary. With more than 2,000 date-matching rows, product filtering can produce events=[] while truncated=true. When fallback is enabled, the router treats that empty result as grounds to substitute tenant-local memory and resets limit to null and truncated to false, even if memory contributes nothing. The fallback policy predates this PR, but the misleading completeness metadata is new. This undermines downstream decisions about whether traceability evidence is complete. Exposure is limited to fallback-enabled environments, including production only when explicitly enabled.
Security review details

Security Blast Radius

  • inferred — The demonstrated completeness failure affects the requesting tenant's export response in fallback-enabled environments. Caller-controlled product/date filters can select the triggering state, but the inspected flow does not grant another tenant's identity or widen database predicates. No cross-tenant disclosure path was established.

Security Findings and Attack Paths

  • observed — Package verification remains a consistency check against a supplied manifest, not issuer authentication. A party able to replace the package and regenerate its manifest can create another internally consistent package. This limitation predates the PR; adding the evidence-context member does not introduce a signature or an independent trust anchor.

Trust Boundaries and Controls

  • observed — Tenant-context resolution rejects missing principals and missing tenant identities, checks conflicting caller tenant claims, and seeds the tenant context for database transactions. The export consumes that resolved identity rather than selecting a tenant from its query parameters.
  • observed — Historical rule evaluations require both the supplied tenant and exported event IDs. Catalog-read and evaluation-read failures are represented by explicit unavailable statuses, which the FDA package carries into its known-gaps section rather than asserting successful rule evidence.

Resilience and Maintainability Implications

  • observed — The coverage-gate bypass remains an explicit caller acknowledgement and is still logged. The PR additionally embeds that decision in the hashed evidence member, improving preservation of the package's admitted limitations without expanding bypass authority.

Hardening Proposals

  • proposed — Preserve database provenance and incompleteness across fallback selection. A successful but empty filtered page should remain distinguishable from database unavailability, so substituting memory cannot erase a known truncation signal.

Resolves two conflicts. The EPCIS export response moved to the
regengine: namespace on main (#3680); the S3 fields ride along as
regengine:limit, regengine:truncated and regengine:ordering. The
tenant-scope allowlist keeps the moved _list_events_from_db_detailed
entry (now marked as an allowlisted secret for detect-secrets) and
drops the epcis_export entry main removed. The main-branch namespace
test now stubs the detailed listing helper.

CI status

Head 68b5c00 (00:38 UTC): every required check is green (Head SHA Check Guard: all required checks passed). The PR waits only on a non-author REVIEW: PASS <engine> 68b5c001ed8adbf42e2179f7bfb6bbaaafc8f856. Copy gate (not required) stays red for the main-baseline reason below.

History. Merged origin/main (f3f06b3) into this branch and pushed 3ad516e, then 68b5c00. What was red on e480ad6 and why:

  • PR Gate: Secrets: detect-secrets flagged the new qhash in .semgrep/tenant-scope-allowlist.yml as a hex high-entropy string. Mine. Fixed with the file's own # pragma: allowlist secret convention.
  • Repo-root security/RLS/isolation suite: tests/scripts/test_check_supply_chain.py::test_real_tree_passes_with_committed_baseline (image-digest on security.yml:872). Not this PR's: main broke at ci(security): E13 image provenance (image SBOM + keyless attest + map) #3705, fixed by ci(supply-chain): baseline locally built regengine:ci image (fixes main red after #3705) #3710 (b99fff3); the merge ports it.
  • PR Gate: Frontend: timed out waiting for Frontend CI Status Gate (queue delay); green on 68b5c00.
  • PR Gate: Backend / CI Status Gate: downstream of the above; green on 68b5c00.
  • Copy gate (not required): red on every main push since 11:21 UTC (stale baseline.json on main plus trust/page.tsx live-claim); identical list on 68b5c00, none of it in files this PR touches. The only copy this PR changes keeps "Preview" in the same string as "EPCIS", which is what the epcis-unlabeled rule checks; the main baseline needs --update-baseline to go green.

Carried in by the merge (red on 3ad516e):

Merge note: main moved the EPCIS export response to the regengine: namespace (#3680), so the S3 fields now ship as regengine:limit, regengine:truncated and regengine:ordering; the PR description's field names are superseded by that.

…gate

Ported from #3736. The Semgrep OWASP hard gate on main has flagged
scripts/create_test_api_key.py:140 (insecure-file-permissions) since
#3625 and the main merge carried the finding into this branch. The
directory mode is unchanged (owner read, write and execute only; the
key file itself stays 0600). This no-ops once main carries the fix.

REVIEW: PASS hermes 68b5c00

services/ingestion/app/epcis/persistence.py: EXPORT_EVENT_LIMIT=2000 with limit+1 truncation detection; tenant_id stays on the SELECT and on the per-event KDE fetch. Product filter still runs after the SQL limit (documented); truncated can therefore be true when the filtered page is short.
services/ingestion/app/epcis/router.py: response metadata.limit / metadata.truncated wired; in-memory fallback correctly leaves limit=None and truncated=False.
services/shared/rules/versions.py: catalog is global by design; evaluations query is tenant-scoped (tenant_id = CAST(:tid AS uuid) + ANY(:event_ids)); failures degrade to unavailable instead of omitting the section.
services/ingestion/app/fda_export_service.py: evidence_context_*.json (rule_versions + known_gaps including coverage_gate_bypassed) is hashed under the Merkle root. include_pii behavior unchanged.
services/shared/request_workflow/assembly.py: now uses shared snapshot_rule_versions; catalog shape preserved.
Authz/tenant/secrets/injection/SSRF: no new issues. Tests run on this head (rule_versions_snapshot, epcis persistence/router, fda_export service/router/formatters/recall, proof_verify, verify_audit_pack): all passed (3 skipped for missing jsonschema). CI: Copy gate red on untouched frontend/src/app/trust/page.tsx live-claim plus stale baseline noise; ignored per review scope alongside Engine Review Pass / PR Blocker / Head SHA Guard. Remaining required checks green.

  • services/ingestion/app/epcis/router.py L371-372 (Major): a truncated database page emptied by the product_id filter no longer has regengine:truncated / regengine:limit reset by the fallback branch; the reset happens only when the fallback supplies events. Test added.
  • services/ingestion/app/epcis/router.py L397: fallback events are sorted newest first (undated last) so regengine:ordering is truthful. Test added.
  • services/shared/rules/versions.py L75-86: each read runs on its own savepoint when the session has begin_nested. Test added.
  • services/shared/request_workflow/assembly.py L161-163: rule_versions_status stored beside rule_versions and hashed with the package. Tests added.
  • docs/PRODUCT_DESCRIPTION.md L34: cutoff field named as regengine:truncated inside regengine:metadata.
  • docs/runbooks/FDA_RECALL_RESPONSE.md L168, L169, L174-175: statuses to check, the 250-entry cap and its known_gaps flags, and the qualified integrity statement.
  • services/ingestion/tests/test_fda_export_recall_1342.py L435-437 and services/ingestion/tests/test_proof_verify_router.py L977-978 (plus the admin verifier test): the snapshot helper is stubbed and its arguments asserted; the tamper tests use a same-size edit and assert the hash-mismatch rejection.

Resolving these threads here; review continues on #3767.
