fix(db): keep runtime grants for data gap tracking across deploys - #3816
Data gap tracking has never worked in production. Every write to public.data_gap_events fails with permission denied, so the table stays empty and the supplier time-to-fix metrics read nothing. Production logs show data_gap_write_failed ... ProgrammingError on every affected ingest since at least 2026-10-28.
Two causes:
v135 granted data_gap_events to regengine only in the migration. apply_runtime_role_cutover.py rebuilds runtime grants from v111's closed inventory on every deploy, so that grant was revoked.
The gap insert also reads public.supplier_traceability_lots, which regengine never had.
Gap logging is fail-open by design, so ingest kept working and the only trace was that log line.
Change
v111 inventory: add data_gap_events to the application tables; regengine gets SELECT, INSERT, UPDATE and regengine_sysadmin gets SELECT.
v111 inventory: regengine gets column-level SELECT on supplier_traceability_lots for tenant_id, tlc_code and facility_id only.
New v142 migration and matching Supabase SQL apply the same grants.
New test_v142_data_gap_runtime_grants.py pins the inventory, which is what survives a deploy.
Security review
Widens the runtime role's access. No DELETE on data_gap_events: gap history is evidence.
Both tables have forced RLS on get_tenant_context().
Every other column of supplier_traceability_lots stays denied.
v139 required C.Lee's approval via DevOps before reaching prod. The same care likely applies here, and this migration runs on the next deploy after merge.
Verification
Production image, on a database already migrated with the broken grants, so migrations and the cutover ran as they do in a real deploy. Built together with the EDI branch and #3813.
Before: permission denied as regengine on both tables, data_gap_write_failed in the logs, 0 rows.
After: grants are correct following the cutover, 0 failed writes, gaps open and close with a receipt.
Deployed a second time so the cutover ran again: writes still succeed and open gaps remain open.
The new structural tests pass. The live Postgres tests in test_v111_runtime_role_grants.py skipped locally, so CI's live suite is the real check on the closed inventory.
Gaps recorded unassigned as the supplier in this run, because the test lot had no supplier_traceability_lots row. Attribution to a real facility was not exercised.
Not in this PR
Gaps that failed to write before this fix are not backfilled.
Nothing alerts on data_gap_write_failed, which is why this went unnoticed.
Medium Risk
Expands the production regengine DB ACL for gap writes and lot lookups, but privileges are column-scoped with no DELETE and tenant RLS; deploy-time grant changes warrant the same operational care as prior runtime cutover revisions.
Overview
Restores data gap tracking by registering the required PostgreSQL privileges in the v111 runtime-role inventory so they survive the per-deploy cutover that had been revoking v135-only grants.
v111 inventory:regengine gets SELECT/INSERT on public.data_gap_events, UPDATE limited to close-receipt columns (closed_at, closing_record_id, closing_record_hash), and column-scoped SELECT on public.supplier_traceability_lots (tenant_id, tlc_code, facility_id) for gap attribution. regengine_sysadmin gets SELECT on data_gap_events. No DELETE on gap rows. Column grants now use a to_regclass-guarded helper so a fresh migration chain does not fail before v135 creates data_gap_events.
v142 Alembic migration and matching Supabase SQL apply the same grants on databases already at head. New migration tests pin the inventory, column scoping, Alembic/Supabase parity, and guarded grant SQL.

Every data gap write has failed in production with permission denied.
v135 granted data_gap_events to regengine only in the migration, and the
per-deploy runtime-role cutover rebuilds grants from v111's closed
inventory, so the grant was revoked. The insert also reads
supplier_traceability_lots, which regengine never had. Gap logging is
fail-open, so the only trace was a data_gap_write_failed log line.
Add both to the v111 inventory and apply them in v142.
supplier_traceability_lots gets only the three columns the lookup reads,
and data_gap_events gets no DELETE. Both tables have forced tenant RLS.

The v111 runtime grant inventory includes grants for data_gap_events and column-scoped SELECT on supplier_traceability_lots. New Alembic and Supabase migrations apply conditional grants when both tables and both roles exist. The Alembic downgrade revokes the column grants and restores table-level SELECT, INSERT, and UPDATE on data_gap_events. Tests check the inventory and the expected statements in both migrations.
This change appears to break fresh database builds, such as new environments, CI and disaster recovery, because a grant on a table that does not yet exist stops the migration chain at v111. Already-migrated databases are not affected. Guard the column grants with an existence check before merging. The earlier over-broad UPDATE grant on data-gap events has been narrowed.
Security architecture risk: Low · up to 015be
The change restores data-gap tracking with limited supplier-data access and existing tenant controls. However, it also makes broad permission to rewrite gap history persistent across deployments. Exploiting that authority requires runtime database access or another arbitrary-SQL capability; no such externally accessible path was established.
Retained concerns
Low · security · observed: The durable runtime inventory grants table-wide UPDATE on data_gap_events, although the normal close operation changes only three closure columns. A principal capable of issuing arbitrary SQL as regengine can rewrite gap identity and opening timestamps or alter closed history within database constraints. Omitting DELETE does not preserve that history. The earlier v135 grant is counterevidence against a newly invented privilege, but the base post-deployment cutover removed it; this PR makes the exposure persistent.
Security review details
Security Blast Radius
inferred — Normal inspected ingestion is confined to the authenticated tenant. Arbitrary SQL under the runtime credential is a stronger compromise: tenant RLS reads a session configuration value, so it should not be treated as an immutable single-tenant boundary against that credential holder. The added authority concerns gap-history rows and three supplier-lot columns in the database served by that credential, not new schema-owner or infrastructure authority.
Security Findings and Attack Paths
inferred — The retained findings share the durable UPDATE authorization surface. The supported attack requires arbitrary runtime-role SQL, followed by updates outside the application's three-column close operation, allowing alteration of recorded gap history and derived time-to-fix information. The inspected parameterized ingestion path does not itself establish that arbitrary-SQL capability.
Trust Boundaries and Controls
observed — The application supplies tenant identity from authenticated context, and the transaction-begin hook clears security configuration before setting the tenant value. The database tenant-context function raises when that value is absent. These controls support ordinary request isolation but do not restrict which same-tenant columns an authorized UPDATE may change.
Resilience and Maintainability Implications
inferred — The partial unique index and conflict handling prevent duplicate open rows for one gap identity, and repeated closes affect only still-open rows. Outcomes are ordered within a recording call, but the inspected implementation does not serialize independent open and close requests. These inherited guarantees support best-effort metrics rather than a globally ordered evidence ledger.
Hardening Proposals
proposed — Narrow runtime UPDATE to the closure columns and explicitly remove inherited table-wide UPDATE in both migration and recurring-inventory paths. If gap history must resist alteration by the runtime role, additionally enforce permitted close transitions and receipt binding in the database; column restrictions alone would not prevent reopening or rewriting closure fields.

v142 restores data_gap_events + column-scoped supplier_traceability_lots grants and pins them in the v111 cutover inventory so per-deploy revoke cannot empty the table again; alembic/supabase SQL match; migration tests cover inventory + no DELETE. Backend CI green on this head. (CodeRabbit least-privilege UPDATE column-scope is a hardening follow-up, not a blocker for the stated permission-denied fix.)
The runtime role only closes gaps: _CLOSE_GAPS_SQL sets closed_at,
closing_record_id and closing_record_hash, and the open path inserts with
ON CONFLICT DO NOTHING. A table-level UPDATE let regengine rewrite any
column of an opened gap (tenant_id, lot_code, gap_kind, opened_at), which
is more than the close path needs. Gap history is evidence.
v111's inventory now grants SELECT, INSERT at table level plus column
UPDATE on the three close fields, so the per-deploy cutover keeps the
narrower grant. v142 (alembic and supabase) revokes the table-level UPDATE
v135 left behind and applies the column grant. Downgrade drops the column
grant and restores v135's table-level grant. Sysadmin stays SELECT-only.

Least-privilege follow-up: runtime data_gap_events UPDATE is column-scoped to the close receipt fields; inventory + v142 (alembic/supabase) + tests agree. Main merged; CI watch next.

_build_object_grants() emitted bare column-level GRANTs for
REGENGINE_COLUMN_GRANTS and REGENGINE_SYSADMIN_COLUMN_GRANTS, while
table and sequence grants already went through the to_regclass guard in
_grant_sql. public.data_gap_events is created in v135, so a fresh
`alembic upgrade head` ran v111 before the table existed and aborted on
the new column UPDATE grant with UndefinedTable.
Column grants now go through _column_grant_sql, which uses the same
DO/IF to_regclass/EXECUTE guard. On a fresh upgrade, v111 skips tables
that are not created yet. The per-deploy cutover
(scripts/apply_runtime_role_cutover.py) runs this module again against
a fully migrated DB, where every table exists, so it still applies
every column grant. The inventory itself is unchanged.

Column-scoped data_gap_events UPDATE kept; v111 column grants now to_regclass-guarded so fresh alembic upgrade does not abort before v135. Inventory + v142 mirrors + tests agree.

Per the runbook's section 7, the precheck gains signature row 75 for v142
