Summary

The FDA sortable spreadsheet now shows when a row has an open data gap. A new last column, Open Data Gaps, reads for example missing_kde: tlc_source_reference. Until now a shipment stored with an open gap exported with blank TLC source cells and no explanation. The repo owner chose a column over a note in Additional KDEs (JSON), and a 503 when the lookup fails.

Merge order:

  1. fix(db): keep runtime grants for data gap tracking across deploys #3816 (data gap grants) is a hard dependency. Its commits are included here and drop out of this diff once it lands. Without that grant, this PR makes every FDA export return 503, because the runtime role can't read data_gap_events.
  2. feat(edi): store shipments missing a TLC source reference with an open gap #3817 is a soft dependency. It creates the EDI gaps that prompted this, but the column works for any gap. This PR doesn't include it.

Change

  • Marker: the column is last in both the v1 and v2 sheets, so no earlier column moves. A gap belongs to a lot and CTE, so every row for that pair is marked, and other CTEs of the lot stay blank.
  • All export paths: /export, /export/all, /export/v2 and recall, through one attach_open_gaps helper. The PDF is unchanged.
  • Package: evidence_context gains known_gaps.events_with_open_data_gaps, and the README explains the column.
  • Failed lookup: the export stops with a 503 and writes no audit row, because a blank cell states that no gap was open.
  • Tenant isolation: the lookup has a tenant_id predicate and binds the tenant GUC for forced RLS.

Why /export/verify changed

It regenerates the CSV from current data and compares hashes, so a plain new column would have caused two false tamper reports:

  • Gaps closing: a gap that closed after the export would change the bytes. Gaps are now read as of the export moment, and that moment is stored as the audit row's generated_at. Verify reads them as of the same moment. No migration needed.
  • Older exports: every export logged before this column would mismatch on the header. On a mismatch, verify also tries the older layout. It still compares against the stored hash, so a real mismatch stays a mismatch.

Zip hashes, the Merkle root and verify_audit_pack.py hash file bytes and don't parse columns, so they needed no change.

Verification

Production image with Postgres 17 and Redis 7, using #3816, #3817 and #3813 merged locally, with EDI ingest of asn_856.edi for two tenants:

Scenario Result
Lot with an open gap, v1, /export/all, v2, recall Marked missing_kde: tlc_source_reference
Lot with no gap Cell blank
Tenant B, same lot code as tenant A's gap, no gap of its own Cell blank
Runtime role bound to tenant B, counting tenant A's gaps 0 rows
Requesting another tenant's export 403
Package Manifest hashes match, events_with_open_data_gaps=1
/export/verify on v1, /export/all, v2 VERIFIED
Gap closed after the export Still VERIFIED; a fresh export shows it blank
Audit row with an older-layout hash VERIFIED
Audit row with a wrong hash MISMATCH_DETECTED
SELECT on data_gap_events revoked 503, no audit row added

verify_audit_pack.py accepted the package and rejected a copy with the marker removed. That check ran on an earlier build; the only later change that reaches the image run is the /export/all audit wiring.

The table was run before the final commit, which rewrites _generate_csv_before_open_gaps to satisfy Semgrep with no change in output. That commit is covered by unit tests and Semgrep, not by a rerun in the image.

Tests: new test_fda_export_open_gaps.py and test_fda_export_open_gaps_pg.py (the second ran against the migrated database), plus the ingestion tests matching fda, export or audit_log. A new test fails if an audit write in these paths omits the moment. The first version of this PR had that bug on /export/all, and the test was confirmed to catch it.

Not run: the browser /verify page. It hashes file bytes against the manifest, and verify_audit_pack.py stood in for it.

Known limitations

  • False mismatch in a narrow race: opened_at is the ingest transaction's start. A gap whose transaction starts before an export and commits after it is missing from that export but visible to a later verify, which then reports MISMATCH_DETECTED. The window is one ingest transaction.
  • Marking is by lot and CTE, not by record: a complete shipping row for a lot is marked if another shipping record for that lot has an open gap.
  • Existing behavior, unchanged: verify re-queries current events, so a new event in the export's scope still reports a mismatch.

Not in this PR

  • PDF export: it has no marker. Its footer already points readers to the CSV for the full data set.
  • services/compliance/app/fsma_spreadsheet.py: a separate exporter with its own FDA_COLUMNS, not changed.

Reviewer note

This is regulator-facing output. The rule's text was not checked for this PR. The sheet already carries non-KDE columns such as the record and chain hashes, and this column adds to them without reordering.

Note

Medium Risk
Changes regulator-facing export bytes and verification semantics plus PostgreSQL runtime privileges; mis-grants or verify regressions could block exports (503) or false tamper signals.

Overview
FDA v1/v2 CSV exports gain a trailing Open Data Gaps column (lot + CTE scoped labels such as missing_kde: tlc_source_reference). All spreadsheet export paths call attach_open_gaps, which uses fetch_gaps_open_at with tenant GUC/RLS; a DB failure returns 503 and skips the audit row because blank cells mean “no gap open.”

/export/verify replays gaps at the audit row’s new generated_at timestamp and falls back to the pre-column CSV layout so older exports still hash-match. ZIP evidence_context and README document events_with_open_data_gaps.

v142 restores regengine grants on public.data_gap_events and column-scoped read on supplier_traceability_lots, and pins the same rules in the v111 runtime inventory so per-deploy cutover does not revoke them again (Alembic + Supabase).

Every data gap write has failed in production with permission denied.
v135 granted data_gap_events to regengine only in the migration, and the
per-deploy runtime-role cutover rebuilds grants from v111's closed
inventory, so the grant was revoked. The insert also reads
supplier_traceability_lots, which regengine never had. Gap logging is
fail-open, so the only trace was a data_gap_write_failed log line.

Add both to the v111 inventory and apply them in v142.
supplier_traceability_lots gets only the three columns the lookup reads,
and data_gap_events gets no DELETE. Both tables have forced tenant RLS.

A shipment stored with an open gap exported with blank TLC source cells and
nothing saying why. The sheet gains a last column, Open Data Gaps, naming the
gaps open on the row's lot and CTE when the export was generated.

Gaps are read as of the export, and the audit row keeps that moment, so
/export/verify regenerates the same bytes after a gap closes. Exports logged
before the column still verify against the older layout. A failed lookup stops
the export with a 503, because a blank cell states that no gap was open.

/export/all read gaps as of one moment and left the audit row to take its own
default, so verify could read gap state a moment later than the export did. A
new test requires every audit write in the export paths to carry the moment.

Walkthrough

FDA exports now include an Open Data Gaps column populated from gaps open at the export timestamp. Export audit records store that timestamp, and verification reconstructs the export with the corresponding gap data while supporting the prior CSV layout. The changes also add runtime grants for the required tables and columns, plus tests and runbook documentation.

Estimated code review effort: 3 (Moderate) | ~25 minutes

Merge Risk: Moderate · previous review 34ca5

Correct the downgrade to preserve the existing data-gap grants before merging; otherwise rollback can disable gap recording and FDA exports.

Security architecture risk: Moderate · up to 34ca5

Tenant isolation and exact hash comparisons remain intact. However, concurrent ingestion can make an authentic export fail later integrity verification because a timestamp alone does not preserve the gap state originally read. The database grants also need to be effective before the new export code runs.

Retained concerns

  • Medium · reliability · inferred: The new timestamp-based reconstruction does not preserve transaction visibility. Ingestion commits event records before recording gaps. A gap-close transaction can start before generated_at, remain uncommitted while the export reads the still-open gap, and then commit with closed_at at or before generated_at. Later verification excludes that gap and reports MISMATCH_DETECTED even when the exported event records are unchanged. A late-committing gap insertion can similarly add a label to the reconstructed past. Persisting the timestamp and sorting labels handle completed historical transitions, but not this overlapping-commit case.
Security review details

Security Blast Radius

  • inferred — The identified integrity-verification failure is scoped to exports containing the tenant lot and CTE whose gap transaction overlaps generation. Runtime permission changes operate at the shared database-role level and can affect exports across tenants using that deployment, while row access remains subject to tenant controls.

Security Findings and Attack Paths

  • inferred — Ordinary overlapping ingestion is sufficient for the snapshot inconsistency; direct database manipulation is not required. The demonstrated consequence is a false integrity mismatch, not acceptance of an incorrect hash, privilege gain, or cross-tenant disclosure.

Trust Boundaries and Controls

  • observed — Gap access combines an explicit tenant predicate with forced RLS and a tenant context binding. Runtime grants do not include DELETE on gap history, and supplier-lot access is column-scoped. Legacy-layout verification still requires exact equality with the stored hash.

Resilience and Maintainability Implications

  • observed — Gap recording already catches failures after ingestion commits and does not reject accepted events. The PR preserves that behavior while adding a fail-closed export lookup. A successful read therefore establishes the recorded gap state, not that every earlier gap-writing attempt succeeded.

Hardening Proposals

  • proposed — Persist the exact gap labels used by an export as immutable, tenant-bound audit snapshot data, or use an equivalent durable versioning scheme. Verification could then reproduce the original bytes without relying on wall-clock predicates to recreate transaction visibility.

Semgrep's FDA CSV guard only recognises the row builders by name, so the
indirect call failed SAST. Both branches now call a builder directly. Output
is unchanged.

Preserve the data-gap grants during downgrade.

The v135 and v111 grants remain required while data_gap_events remains present. The current downgrade revokes them. The next gap write can fail on either table, and record_outcomes() continues after logging the error. FDA gap reads can fail and return 503.

Restore the inherited grants instead of revoking them:

REVIEW: PASS codex 77140dd

Codex review: no blocking findings on this head. Reviewed the tenant-scoped historical gap lookup, all four export paths, CSV sanitization and column order, package evidence, audit timestamps, and verification of both current and older CSV layouts. The required grants from #3816 are now in the base branch.

Validation: 117 focused tests passed covering the new marker, FDA export service, v2 reproducibility, verification, export logging, and gap tracking; git diff --check passed. I did not rerun the production-image or live-Postgres checks during this review; their earlier results are documented in the PR.

Known limitation: an ingest transaction that starts before the export and commits afterwards can change a later historical gap lookup and produce a verification mismatch. This is disclosed in the PR and does not block this scoped spreadsheet-marker change.
