Summary

EDI 856 shipments sent without tlc_source_reference are now stored with an open data gap, instead of being rejected. This is a product decision by the repo owner: X12 has no element for this KDE (21 CFR 1.1340(a)(7)), so the strict gate lost otherwise complete shipments.

Merge order:

  1. fix(ingestion): give EDI events the shipping KDEs validation requires #3781: its commits are included here and drop out of this diff once it lands. Review only the last commit until then.
  2. fix(db): keep runtime grants for data gap tracking across deploys #3816 (data gap grants). It is a hard dependency: the gap commits with the event, so without those grants a shipment missing the field is refused, not stored.

Change

  • ingest_events takes a keyword deferred_kdes, by CTE. An event missing only deferred KDEs is stored, and its result lists them in the new open_gaps field.
  • The event opens a missing_kde gap and sends no closing receipt, so it can't close the lot's open gaps. A later complete record for that lot and CTE closes them.
  • The EDI routes pass {shipping: {tlc_source_reference}}.
  • The gap is written in the event's savepoint. If it can't be written, the event is rolled back and rejected, so open_gaps never names a gap that wasn't recorded. It is requeued only to follow a complete record earlier in the same batch, so it can't reopen a gap another request closed.
  • A single tlc_source_reference on a shipping document with several lots gets a 422 and a recorded rejection before the ISA13 is reserved, because it could link a lot to the wrong source. Without the field, each lot is stored with its own gap.

Boundaries

  • Not reachable by the sender. The exemption is a server-side argument, never read from payload.source or KDEs, and the webhook route doesn't use it. A test covers this.
  • Any other missing KDE still rejects. The event is rejected if even one missing KDE isn't deferred, and an error with no gap key fails closed.
  • Shipping only. Receiving keeps the strict gate, because its rule for this KDE is critical.
  • Rules enforcement. Under RULES_ENGINE_ENFORCE=all, a repo-owner decision exempts a warning whose missing fields all belong to the deferred KDE. Otherwise all would reject every one of these shipments. The verdict stays fail, so an exception case is still recorded. Critical rules, evaluator errors and any other failure still reject, and it fails closed if the exempted results don't explain the failure count.

Verification

Production image with Postgres 17 and Redis 7, built with #3813 and the grants PR, on both EDI routes, using asn_856.edi:

Input Result
No tlc_source_reference 201, accepted=1, open_gaps=["tlc_source_reference"], open gap row
Same lot, reference sent 201, accepted=1, gap closed with a receipt
New lot, no reference, after a second deploy stored, gap stays open
Future-dated 400
Two lots, one reference 422 edi_tlc_source_reference_with_multiple_lots, nothing stored
Two lots, no reference 201, accepted=2, one open gap per lot
No reference, grants PR not applied rejected, nothing stored

All test_edi_*.py, test_webhook_compat*.py, test_r3_restored_fixtures_e2e.py and test_data_gap_tracking.py pass in the production image, along with the new test_edi_deferred_kdes.py.

Under RULES_ENGINE_ENFORCE=all with the 36 seeded rules loaded, same files on both sides, on both EDI routes:

Build No reference With reference
Without the exemption rule_violation, not stored stored, verdict pass
With it stored, verdict fail, exception case recorded stored, verdict pass

Known limitation

The repo owner accepted two narrow races between concurrent requests for the same tenant, lot and CTE. Gap outcomes for complete records are applied just after commit, so inside that window:

  • A complete record's close can also close a gap that an incomplete record has just opened. The event stays stored with the KDE blank, and scoring still counts it as missing.
  • After a batch holding a complete then an incomplete record, a replayed gap can reopen after another request's close, until the next complete record closes it.

Removing both means applying all gap outcomes inside the ingest transaction, which reworks the shared gap flow for every ingest_events caller.

Not in this PR

  • Multi-lot receipts can still share one reference. Receiving requires the KDE and has no open gap, so refusing it would leave a multi-lot 861 or 944 no way in. The false-link risk stays there until the API takes a reference per lot.
  • The FDA export shows no sign of the gap. The row appears with blank TLC source columns and nothing marks it as outstanding.
  • An EDI response still says status: "accepted" with HTTP 201 when every event was rejected.

Note

Medium Risk
Changes core ingest acceptance, transactional data-gap writes, and rules enforcement under all; boundaries are server-only and tested, but wrong deferral logic could admit incomplete shipments or bypass warnings.

Overview
EDI 856/document routes can now accept shipping events that lack tlc_source_reference (no X12 field) by passing a server-only deferred_kdes map into ingest_events. Missing only those KDEs stores the CTE, writes missing_kde gaps in the same transaction, and returns open_gaps on EventResult; any other missing KDE or failed gap write still rejects.

EDI event building now sets reference_document and optional tlc_source_reference from the form, applies ship_date/receive_date for all sets, and 422s when one tlc_source_reference would span multiple shipping lots (before ISA13 reserve). Receiving keeps the strict KDE gate.

Under RULES_ENGINE_ENFORCE=all, should_reject skips non-critical rule failures that touch only deferred KDEs (verdict stays fail for the exception queue). open_gaps_for_stored_record and batch gap-outcome logic avoid incomplete records closing lots incorrectly.

Every EDI 856 was rejected in ingest_events. REQUIRED_KDES_BY_CTE[SHIPPING]
requires reference_document, tlc_source_reference and ship_date, but:

- both EDI routes stored the BOL only as reference_document_number;
- tlc_source_reference was only copied for 944/945, and the legacy
  /api/v1/ingest/edi route had no way to accept it;
- /document set ship_date/receive_date only inside the 944/945 branch, so
856/850/810/861 events had no CTE date KDE.

Now both routes set reference_document from the extracted BOL (keeping
reference_document_number), the legacy route accepts tlc_source_reference,
/document copies it to every set, and the date KDE is chosen by CTE type
for every set. An 856 without tlc_source_reference is still rejected, but
for that one field only: X12 has no place for it.

test_r3_restored_fixtures_e2e asserted the old behavior (ship_date as an
open gap on the 856 fixture); it now expects only tlc_source_reference.

…n gap

X12 has no element for tlc_source_reference (21 CFR 1.1340(a)(7)), so an
856 sent without the form field was rejected and lost. Product decision:
store it, and record that the KDE is outstanding.

ingest_events takes a server-side deferred_kdes argument, set only by the
EDI routes, for shipping and this one KDE. An event missing only deferred
KDEs is stored, reports them as open_gaps, and opens a missing_kde gap
without closing the lot's existing gaps. Any other missing KDE still
rejects, and receiving keeps the strict gate. The exemption is never read
from the payload, which the sender controls.

Walkthrough

The EDI routes add trimmed TLC source references and reference documents to generated events. They pass server-side deferred-KDE settings for shipping events and reject a supplied reference when a document resolves to multiple lots. Ingestion accepts events when all validation gaps are configured for deferral, records those gaps, and reports them in the result. Receiving validation remains strict. Event-date KDE assignment now depends on CTE type.

Estimated code review effort: 3 (Moderate) | ~25 minutes

Merge Risk: Moderate · up to 3c050

Shipments missing a TLC source reference are now stored with an open gap instead of rejected. In rare cases, such as a mixed batch for one lot or concurrent requests, the gap can be closed or lost, so the shipment looks complete when its source reference is still missing. The event itself is not lost, and the next complete record or a later gap write corrects the state, but the tracking gap should be fixed or explicitly accepted before merge.

Security architecture risk: Moderate · up to 785db

The change remains constrained to authenticated, tenant-scoped ingestion, and initial gap creation fails closed. Material risks remain in gap replay and ordering, visibility of incompleteness in recall exports, and recovery after partially accepted uploads.

Retained concerns

  • Medium · security · inferred: Gap mutations are not bound to event freshness or insertion identity. Once an old incomplete shipment reaches ingestion again, storage can return an idempotent result, but ingestion still opens its gap; the uniqueness rule protects only currently open rows, so a previously resolved gap can reopen. Conversely, a complete record's delayed post-commit closure can close a gap opened by a subsequently committed incomplete shipment. Initial atomic writes and same-batch ordering do not preserve cross-request lifecycle ordering. These mechanisms predate the PR in part, but their application to accepted incomplete shipments is new.
  • Medium · architecture · inferred: The new accepted-incomplete state is not propagated explicitly into FDA recall records. A shipment missing only the deferred reference can retain validation_status valid when it has no other alerts, while the legacy reader excludes only rejected records. Both inspected recall readers return KDEs without joining gap state, and their shared output conversion has no open-gap field. The missing KDE remains discernible, but consumers do not receive the explicit incompleteness signal returned at ingestion. The export code is unchanged; admitting these previously rejected records creates the new contract exposure.
  • Medium · reliability · inferred: A multi-lot shipment can retain an accepted lot while a later lot rolls back because its required gap write fails. Any positive accepted count retains the interchange reservation, so an unchanged resend is rejected before reaching event-level idempotency for up to seven days when Redis deduplication is available. The reservation rule predates this PR, but deferred acceptance introduces this recovery state for reference-less 856 shipments. Failure isolation therefore leaves the rejected lot without the ordinary same-interchange recovery path.
Security review details

Security Blast Radius

  • inferred — The identified integrity effects concern shipment records and gap state within the authenticated tenant. A closing outcome affects every open gap matching that tenant, lot, and CTE, rather than only one event or gap key.

Security Findings and Attack Paths

  • inferred — An authorized sender replaying an old shipment can reach non-idempotent gap mutations after the Redis reservation expires or when deduplication fails open. Event-level deduplication prevents another identical event insertion, but does not prevent the separate gap transition. This is a source-supported integrity path, not a demonstrated cross-tenant or unauthenticated exploit.

Trust Boundaries and Controls

  • observed — The inspected EDI route requires edi.ingest permission and resolves tenant identity from the principal. Shared ingestion rechecks tenant equality. The deferred-KDE mapping is a server constant, not a payload-selected exemption; unrecognized or additional validation errors remain non-deferrable.

Resilience and Maintainability Implications

  • observed — The initial event-plus-gap write is fail-closed, but subsequent gap outcomes intentionally catch failures and commit independently after event acceptance. The stronger initial guarantee therefore does not extend to the entire lifecycle.

Hardening Proposals

  • proposed — Bind gap transitions and durable recovery work to persisted event identity and per-scope ordering, so duplicate or older records cannot reverse newer gap state. Preserve retry identity across partially accepted interchanges rather than relying solely on reservation expiry.
  • proposed — Carry explicit deferred-gap metadata into recall outputs, distinguishing record integrity and successful storage from required-KDE completeness.

Unverified finding
Verification ran but could not confirm this finding. It is shown for review, not as a verified issue.

Update this fixture pipeline to model deferred acceptance.

This assertion now expects a deferred gap, but _run_pipeline still excludes every event with a validation error. The test therefore expects an empty spreadsheet for a shipment that production now stores. Include events whose only gaps are deferred, then assert the resulting export row and its missing-reference handling.

Review findings on the deferred KDE path:

- The gap was written after commit by the fail-open record_outcomes, so
open_gaps could name a gap that was never recorded. Write it in the
event's savepoint. If it can't be written, the event is not accepted.
- The tlc_source_reference form field is a single value copied to every
event, so a document with several lots could link a lot to a source that
never assigned its code. Refuse that with a 422 before the ISA13 is
reserved. Without the field those lots are stored with open gaps.

Review findings on the previous commit:

- The refusal covered receiving, which requires the field and has no open
gap to fall back on, so a multi-lot 944 could not be ingested at all.
  Limit it to shipping sets.
- The refusal left no audit trail. Record it like other EDI rejections.
- Replaying every deferred gap after commit could reopen a gap that
another request had just closed. Requeue only to follow a close queued
  earlier in the same batch.

A 945 lot that isn't production-shaped is stored with the line's GTIN or
UPC as a prefix, so one short lot on two items is two TLCs. The guard
compared bare lots, counted one, and let one reference cover both.

Move the rebuild into a helper shared by the validator and the guard so
the two can't disagree.

Deferred tlc_source_reference still rejects shipments under RULES_ENGINE_ENFORCE=all.

webhook_compat.py:342-344 exempts a deferred KDE only from the validation rejection. The event still reaches the rules engine at lines 395-418.

For an FTL shipping event without tlc_source_reference, the seeded rule "Shipping: TLC Source Reference Required" fails (seeds.py:438-451). No inspected EDI or canonical path sets tlc_source_gln, so the rule's second field is also absent. The rule has severity warning, so the failure is not added to critical_failures. It does set compliant to False. Under all, should_reject then returns true and the outer savepoint rolls back the stored shipment.

Under cte_only, this rule does not reject. Only critical failures reject in that mode.

The deferred KDE needs an explicit rule-verdict policy. Decide whether a deferred gap should count as a rules failure under all. Then apply that policy in the rules path, or exclude this rule for EDI shipments. Do not change only the validation check at lines 342-344.

Allow recovery after partial gap-write failure.

When a later lot fails its deferred gap write after an earlier lot is accepted, the retained ISA13 reservation blocks a resend for up to seven days. Recovery must keep accepted lots idempotent, allow rejected lots to retry, and release the reservation when zero events were accepted. Keep the retry scope keyed by sender, receiver, and ISA13; do not use a narrower identity.

Update the ingestion flow that passes deferred_kdes=_EDI_DEFERRED_KDES so a
partial deferred gap-write failure preserves idempotency for accepted lots while
allowing rejected lots to retry; release the ISA13 reservation when no events
were accepted, and retain the existing reservation identity.

The deferral only skipped the KDE gate. The seeded shipping rule for
tlc_source_reference then failed as a warning, and all rejects on
warnings, so these shipments could never be stored in that mode.

should_reject takes deferred_kdes. A non-critical failure whose missing
fields all belong to a deferred KDE no longer rejects. Every other failure
still does, and it fails closed if those results don't explain the failure
count. The verdict stays fail, so the exception queue still records it.

Keep the batch close and gap reopening atomic.

For a batch ordered [complete, incomplete], the queued close can succeed and the queued gap reopening can fail. record_outcomes catches that failure and commits the close, leaving the accepted incomplete record without an open gap. Apply these transitions atomically with the records, or prevent the earlier close from closing the later gap. (raw.githubusercontent.com)

As per path instructions, “Validation failures must be recorded as evidence, never swallowed.”

Review comment at @services/ingestion/app/webhook_compat.py at line 550:
Update the queued close and gap-reopening flow around `gap_scope` and
`closes_queued` so a later accepted incomplete record cannot be left without an
open gap if reopening fails. Apply both transitions atomically with the records,
or prevent the earlier close from closing a gap that a later record in the batch
needs reopened; preserve the existing failure handling without committing a
partial transition.

REVIEW: PASS codex 3c0507e

Reviewed the current diff, including deferred-KDE authorization, event/gap savepoint atomicity, shipping-only boundaries, multi-lot shipping rejection, and RULES_ENGINE_ENFORCE=all. Ran 162 focused EDI, webhook compatibility, gap-tracking, and rules-enforcement tests on this commit: all passed. The owner-accepted concurrency limitations and receiving reference limitation are explicit in the PR; FDA gap visibility is tracked separately in #3823. No additional blocking findings in this change. Merge remains dependent on #3781; #3816 is now merged.
