Alembic b8c1e4f2a901 (revises a3f7ce361b0e) mirroring the partner_files SQL, plus Railway helper: fsma.manifest_blobs.raw_bytes nullable with manifest_blobs_payload_present_check (raw_bytes OR non-empty storage_url). No byte moves; no envelope encryption.
Out of scope (follow-ups)
App cutover to write partner bytes into Storage / metadata rows
Stub-apply supabase SQL + manifest check on local PG
CI green (do not apply migrations)
Human review before any prod/staging apply
Schema parity / raw_bytes (CI follow-up)
Regenerated docs/schema/expected_schema.json for the three new public partner tables.
fsma.manifest_blobs.raw_bytes nullable is intentional for the partner-uploads / pointer-only cutover: Alembic v138 drops NOT NULL and adds manifest_blobs_payload_present_check requiring raw_bytes OR a non-empty storage_url (see migration + Summary Railway section above). Snapshot updated to match.
Note
High Risk
Touches tenant-isolated partner file metadata, retention hard-delete grants, manifest blob payload rules, and RLS—security- and data-lifecycle sensitive despite being migration-only until app cutover.
Overview
Adds migration-first partner first-file storage: metadata in Postgres, bytes in a private partner-uploads bucket (Supabase-only DDL), with a 30-day retention clock after report delivery.
Schema & migrations: New Alembic v140 (and matching Supabase SQL) create partner_files, partner_file_derived, and append-only partner_file_deletion_log; tenant RLS via get_tenant_context(); delete_after maintained by a trigger on report_delivered_at + 30 days (not user-writable). fsma.manifest_blobs allows pointer-only rows: raw_bytes nullable, check requires raw_bytes OR non-empty storage_url. Supabase adds the bucket and authenticated insert/select policies (no client delete).
Runtime ACLs: v111 cutover inventory extended so regengine can DML partner metadata tables (including DELETE for retention sweeps) and INSERT storage_url on manifest_blobs; regengine_sysadmin read-only on partner tables; deletion log stays INSERT+SELECT only.
Application retention:data_retention.py wires the three partner resources (with _sql_target for schema-qualified SQL), adds policies, and cleanup_deleted_records_job hard-deletes expired soft-deleted partner rows while skipping the append-only deletion log. expected_schema.json, grant verification runbook, and tests (migration idempotency, trigger behavior, privileges) updated accordingly.
App upload/retention worker cutover remains out of scope for this PR.

The migrations add partner-file metadata, derived-file references, and a deletion log. A trigger sets retention deadlines from report delivery timestamps. Row-level security and grants define tenant-scoped access. A separate migration configures a private upload bucket with tenant-scoped select and insert policies. The Alembic revision changes manifest-blob validation to accept raw bytes or a nonblank storage URL and grants column-level INSERT access to storage_url.
Resolve the open migration and access-control risks, and add live coverage for pointer-only manifest inserts before applying this schema change.
Security architecture risk: Moderate · up to a642a
Private storage and tenant checks limit exposure, but derived-file ownership is not bound to the parent tenant, routine deployment removes intended retention-table access, and rollback can remove payload validation. Application cutover and the retention worker are deferred, limiting demonstrated current exposure but not resolving these design risks.
Retained concerns
Medium · security · observed: The new derived-file foreign key validates only partner_file_id, while row-level security validates only the derived row's tenant_id. Where metadata DML grants are effective, a writer knowing another tenant's file UUID can attach an own-tenant derived row to that file. This breaks parent/child ownership isolation and makes later per-file cleanup ambiguous. Authenticated clients receive no metadata DML, and no current cross-tenant read or deletion consumer was demonstrated.
Medium · reliability · observed: The managed migration path runs privilege reconciliation immediately after upgrade. It revokes public-schema runtime grants and rebuilds the closed inventory, which contains storage_url INSERT but none of the three new partner tables. Consequently, the intended metadata and deletion-audit grants do not survive that deployment path. This is currently a latent, fail-closed integration defect because application and retention-worker cutover are deferred; when activated, it would prevent the intended least-privilege retention and audit operations.
Medium · reliability · observed: Downgrade unconditionally removes the payload-presence check but restores raw_bytes NOT NULL only if no null raw_bytes rows exist. After pointer-only rows have been introduced, rollback therefore permits new manifests containing neither bytes nor a storage pointer, weakening retained-evidence integrity relative to both the base and upgraded schema. This requires an operator-triggered downgrade after pointer-only data exists; ordinary upgrade does not enter this state.
Security review details
Security Blast Radius
inferred — The demonstrated ownership gap requires metadata-write authority and another file's UUID; it does not grant anonymous access or establish object-byte disclosure. Grant loss affects partner operations on databases using the managed reconciliation path. Payload-validation weakening requires downgrade after pointer-only records exist. No environment-wide credential or infrastructure privilege expansion was demonstrated.
Security Findings and Attack Paths
inferred — A metadata writer can submit an own-tenant derived row referencing a foreign-tenant parent because neither the foreign key nor the child policy checks their tenant equality. By contrast, an arbitrary nonblank manifest URL is only a stored value in the inspected path: a fetch, credential leak, or privileged deletion attack would require an additional consumer that was not established.
Trust Boundaries and Controls
observed — The bucket is configured private with a 25 MiB limit and a CSV-oriented MIME allowlist. Its new authenticated SELECT and INSERT policies compare the first object folder with get_tenant_context(); no client UPDATE or DELETE policy is added. The inspected tenant-context definitions read app.tenant_id, not JWT claims. Establishment of that context for deployed Storage requests and the complete deployed permissive-policy set were not verified.
Resilience and Maintainability Implications
observed — Privilege reconciliation is transactional and rolls back on lock timeout rather than leaving a partial ACL transition. This existing containment control does not correct the newly omitted partner grants: a successful reconciliation removes them. Retention-worker interruption, concurrency, object cleanup, and audit recovery remain deferred rather than proven safe.
Hardening Proposals
proposed — Before real-data cutover, bind derived ownership with a composite parent/tenant relationship, reconcile the intended partner grants through the closed inventory, and preserve a payload-presence invariant or refuse downgrade when pointer-only rows exist.
proposed — For the deferred application and worker cutover, define trusted Storage tenant-context establishment, tenant-authorized pointer resolution, and idempotent upload/deletion/audit recovery. Exercise interruption, repeated processing, concurrent deletion, delivery timestamp changes, and orphan-object cleanup before relying on the deadline as an end-to-end retention guarantee.

Ignore comment semicolons when splitting migration SQL
Every Alembic upgrade to this revision fails because _statements() treats semicolons inside SQL comments as statement boundaries. The existing comment -- Clock: delete_after = report_delivered_at + 30 days (trigger-maintained; UTC). causes the first emitted statement to begin with the uncommented text UTC)., which PostgreSQL rejects. Other comments similarly produce fragments starting with v1 may and generated col not allowed). Make the splitter comment-aware or execute the SQL without this custom splitting, and verify the actual upgrade() path rather than only applying the standalone SQL.
Re-verified head ec956ff6a65d8dcd18f56b3abdcd63c7351504b0 before posting. Read-only review (no migration apply, no DB touch). Checklist: tenant scoping / RLS, delete_after trigger, downgrade order, idempotency, revision chain, no secrets / no invented test data.
-- delete_after = … (trigger; generated col not allowed)
Simulating the splitter (no DB) yields 29 fragments; the first executable “statements” begin with UTC)., v1 may, and generated col not allowed). That means CREATE TABLE public.partner_files and CREATE OR REPLACE FUNCTION public.partner_files_set_delete_after never run as valid SQL on the Alembic path. The standalone Supabase .sql files are fine (applied as whole scripts); Railway/Alembic is not.
Fix: make the splitter comment-aware, strip -- … lines before splitting, remove ; from comments, or op.execute the mirrored SQL without custom splitting (match v137’s statement-tuple style). Re-verify by running upgrade() against an ephemeral PG — not only the raw .sql.
FAIL 2 (blocker) — delete_after can be bypassed
Trigger is BEFORE INSERT OR UPDATE OF report_delivered_at only. regengine has UPDATE on partner_files, so:
UPDATE partner_files SET delete_after ='2099-01-01'WHERE id = …;
does not fire the trigger and permanently desyncs the retention clock. INSERT and UPDATE … report_delivered_at behave correctly (NULL clears; +30 calendar days on timestamptz; stub 2026-10-01Z → 2026-10-31Z is right).
Fix:BEFORE INSERT OR UPDATE (no OF list), always recompute NEW.delete_after from NEW.report_delivered_at, so any write restores the clock.
What looks good
Revision chain:a3f7ce361b0e (v137) → b8c1e4f2a901 (v138); linear on main (…→v135→v136→v137); no new branch.
Tenant scoping:tenant_id uuid not null references public.tenants(id) on partner_files / partner_file_derived; CASE + (select public.get_tenant_context()) with sysadmin arm first; ENABLE + FORCE RLS; matches v137/epcis/data_gap shape. anon/authenticated revoked on metadata; deletion log append-only (INSERT/SELECT, no UPDATE/DELETE).
Downgrade order (Alembic): policies → trigger → function → indexes → deletion_log → derived → partner_files; manifest check dropped and raw_bytes NOT NULL restored only when safe.
Idempotency (Supabase SQL):IF NOT EXISTS / DROP … IF EXISTS + recreate / role-guarded DO blocks / bucket ON CONFLICT DO UPDATE.
manifest_blobs: DROP NOT NULL then payload check; existing NOT NULL rows satisfy the check. No byte moves / no secrets in the diff. No test files with invented IDs in this PR.
Non-blocking notes (fix after blockers)
partner_file_derived.tenant_id FK has no supporting index (repo has an unindexed-FK lint history).
engagement_id is NOT NULL with no FK (opaque for beta is OK if intentional; document it).
No CHECK that derived.tenant_id matches parent partner_files.tenant_id.
No CHECK tying status = 'delivered' to report_delivered_at IS NOT NULL.
Do not apply until FAIL 1 and FAIL 2 are fixed and a delta review PASSes. Holds SEL-549 / SEL-590 unchanged.

Derived rows can reference cross-tenant files
Medium Severity
partner_file_derived.partner_file_id references only partner_files.id, not (id, tenant_id). A child row can pair one tenant's tenant_id with another tenant's file, which child RLS cannot reject.

Add the CHECK constraint as NOT VALID to limit lock time on fsma.manifest_blobs.
ADD CONSTRAINT ... CHECK without NOT VALID takes ACCESS EXCLUSIVE and scans the whole table in one step. manifest_blobs holds ingestion evidence and receives writes from the ingest path. The ALTER COLUMN ... DROP NOT NULL at Line 287 also takes ACCESS EXCLUSIVE. Neither statement sets lock_timeout. A long-running reader or writer can queue behind the lock and stall ingestion.
Set lock_timeout for the migration. Add the constraint as NOT VALID. Run VALIDATE CONSTRAINT in a separate transaction, for example inside op.get_context().autocommit_block().

downgrade() drops the deletion log and file metadata without a data check.
partner_file_deletion_log is a hash-chained, append-only audit record. partner_files holds the retention clock and the content hashes. DROP TABLE destroys these records if any rows exist. The docstring says only "drops only what this revision created", which does not justify destroying audit evidence. The raw_bytes restore in the same function already checks for data first. The table drops need the same check.
Refuse the downgrade when partner_file_deletion_log has rows. Alternatively, require an explicit opt-in.

The "append-only" deletion log still allows TRUNCATE by service_role and regengine.
The comments state that the log is append-only. The grants revoke only UPDATE and DELETE.
service_role is not revoked TRUNCATE, TRIGGER, or REFERENCES. Supabase's default privileges on public tables normally grant these to it.
service_role has BYPASSRLS, so RLS does not protect the log from that role.
TRUNCATE ignores row-level security and the missing DELETE privilege.
A holder of service_role can therefore wipe the deletion evidence in one statement. Revoke all privileges, then grant only SELECT, INSERT. A BEFORE UPDATE OR DELETE trigger and a BEFORE TRUNCATE trigger that raise an exception would also block the table owner.

Storage Access Control
Supabase Storage is designed to work perfectly with Postgres Row Level Security (RLS).
You can use RLS to create Security Access Policies that are incredibly powerful and flexible, allowing you to restrict access based on your business needs.
## Access policies#
By default Storage does not allow any uploads to buckets without RLS policies. You selectively allow certain operations by creating RLS policies on the `storage.objects` table.
You can find the documentation for the storage schema here , and to simplify the process of crafting your policies, you can use these helper functions .
If you need different `SELECT` policies for different Storage actions, such as listing objects versus reading authenticated objects, use the operation-aware helpers `storage.allow_only_operation()` and `storage.allow_any_operation()` documented in Storage Helper Functions.
The RLS policies required for different operations are documented here
For example, the only RLS policy required for uploading objects is to grant the `INSERT` permission to the `storage.objects` table.
To allow overwriting files using the `upsert` functionality you will need to additionally grant `SELECT` and `UPDATE` permissions.
## Policy examples#
An easy way to get started would be to create RLS policies for `SELECT`, `INSERT`, `UPDATE`, `DELETE` operations and restrict the policies to meet your security requirements. For example, one can start with the following `INSERT` policy:
`
1
create policy "policy_name"
2
ON storage.objects
3
for insert with check (
4
true
5
);
`
and modify it to only allow authenticated users to upload assets to a specific bucket by changing it to:
`
1
create policy "policy_name"
2
on storage.objects for insert to authenticated with check (
3
-- restrict bucket
Allow a user to access a file that was previously uploaded by the same user:
`
1
create policy "Individual user Access"
2
on storage.objects for select
3
to authenticated
4
using ( (select auth.jwt()->>'sub') = owner_id );
`
Allow anyone to access objects in the `avatars` bucket via publishable key. The `allow_any_operation()` filter is critical here as without it users would be able to list the bucket contents.
This is not needed for public buckets, as they are already publicly accessible
`
1
create policy "Avatar images are publicly accessible." on storage.objects
2
for select using (bucket_id = 'avatars' and storage.allow_any_operation(array['object.get_authenticated_info', 'object.get_authenticated']));
`
## Bypassing access controls#
If you exclusively use Storage from trusted clients, such as your own servers, and need to bypass the RLS policies, you can use the `service key` in the `Authorization` header. Service keys entirely bypass RLS policies, granting you unrestricted access to all Storage APIs.
Remember you should not share the service key publicly.</excerpt>

<title>Custom Claims & Role-based Access Control (RBAC) | Supabase Docs</title>

<excerpt>citeturn0search1 [wordlim: 200] Published: today; Crawled: today; This guide uses the Slack Clone example to demonstrate how to add a `user_role` claim and use it in your Row Level Security (RLS) policies. ... Therefore, to access the custom claims in your application, e.g. your browser client, or server-side middleware, you will need to decode the `access_token` JWT on the auth session. ... const { subscription: authListener } = supabase.auth.onAuthStateChange(async (event, session) => {
Custom Claims & Role-based Access Control (RBAC)
Custom Claims are special attributes attached to a user that you can use to control access to portions of your application. For example:
`
1
{
2
"user_role": "admin",
3
"plan": "TRIAL",
4
"user_level": 100,
5
"group_name": "Super Guild!",
6
"joined_on": "2022-05-20T14:28:18.217Z",
7
"group_manager": false,
8
"items": ["toothpick", "string", "ring"]
9
}
`
To implement Role-Based Access Control (RBAC) with `custom claims`, use a Custom Access Token Auth Hook. This hook runs before a token is issued. You can use it to add additional claims to the user's JWT.
This guide uses the Slack Clone example to demonstrate how to add a `user_role` claim and use it in your Row Level Security (RLS) policies.
## Create a table to track user roles and permissions#
## Accessing custom claims in your application#
The auth hook will only modify the access token JWT but not the auth response. Therefore, to access the custom claims in your application, e.g. your browser client, or server-side middleware, you will need to decode the `access_token` JWT on the auth session.
In a JavaScript client application you can for example use the `jwt-decode` package:
`
1
import { jwtDecode } from 'jwt-decode'
2
3
const { subscription: authListener } = supabase.auth.onAuthStateChange(async (event, session) => {
4
if (session) {
5
const jwt = jwtDecode(session.access_token)
6
const userRole = jwt.user_role
7
}
8
})
`

The Storage Schema
Learn about the storage schema
Storage uses Postgres to store metadata regarding your buckets and objects. Users can use RLS (Row-Level Security) policies for access control. This data is stored in a dedicated schema within your project called `storage`.
When working with SQL, it's crucial to consider all records in Storage tables as read-only. All operations, including uploading, copying, moving, and deleting, should exclusively go through the API.
This is important because the storage schema only stores the metadata and the actual objects are stored in a provider like S3. Deleting the metadata doesn't remove the object in the underlying storage provider. This results in your object being inaccessible, but you'll still be billed for it.
Here is the schema that represents the Storage service, you have the option to query this table directly to retrieve information about your files in Storage without the need to go through our API.
The schema centers on three tables. Each `buckets` row can own many `objects` rows by a one-to-many relationship that joins `buckets.id` to `objects.bucket_id`. The `buckets` table holds bucket configuration. Its `id` and `name`, whether the bucket is `public`, and constraints such as `file_size_limit` and `allowed_mime_types`. The `objects` table holds per-file metadata: the owning `bucket_id`, the object `name` and `path_tokens`, a `metadata` JSON blob, and version information. The `migrations` table is standalone and tracks the schema migrations applied to the storage service.
## Modifying the schema#
We strongly recommend refraining from making any alterations to the `storage` schema and treating it as read-only. This approach is important because any modifications to the schema on your end could potentially clash with our future updates, leading to downtime.
However, we encourage you to add custom indexes as they can significantly improve the performance of the RLS policies you create for enforcing access control.</excerpt>

For example, you can implement a policy where only the owner of an object can delete it. To do this, check the `owner_id` field of the object and compare it with the `sub` claim of the JWT:
Ownership
When creating new buckets or objects in Supabase Storage, an owner is automatically assigned to the bucket or object. The owner is the user who created the resource and the value is derived from the `sub` claim in the JWT. We store the `owner` in the `owner_id` column.
When using the `service_key` to create a resource, the owner will not be set and the resource will be owned by anyone. This is also the case when you are creating Storage resources via the Dashboard.
The Storage schema has 2 fields to represent ownership: `owner` and `owner_id`. `owner` is deprecated and will be removed. Use `owner_id` instead.
## Access control#
By itself, the ownership of a resource does not provide any access control. However, you can enforce the ownership by implementing access control against storage resources scoped to their owner.
For example, you can implement a policy where only the owner of an object can delete it. To do this, check the `owner_id` field of the object and compare it with the `sub` claim of the JWT:
`
1
create policy "User can delete their own objects"
2
on storage.objects
3
for delete
4
to authenticated
5
using (
6
owner_id = (select auth.uid()::text)
7
);
`
The use of RLS policies is one way to enforce access control. You can also implement access control in your server code by following the same pattern.</excerpt>

Storage Buckets
Buckets allow you to keep your files organized and determines the Access Model for your assets. Upload restrictions like max file size and allowed content types are also defined at the bucket level.
## Access model#
There are 2 access models for buckets, public and private buckets.
### Private buckets#
When a bucket is set to Private all operations are subject to access control via RLS policies. This also applies when downloading assets. Buckets are private by default.
The only ways to download assets within a private bucket is to:
* Use the download method by providing an authorization header containing your user's JWT. The RLS policy you create on the `storage.objects` table will use this user to determine if they have access.
* Create a signed URL with the `createSignedUrl` method that can be accessed for a limited time.
#### Example use cases:#
* Uploading users' sensitive documents
* Securing private assets by using RLS to set up fine-grain access controls
### Public buckets#
When a bucket is designated as 'Public,' it effectively bypasses access controls for both retrieving and serving files within the bucket. This means that anyone who possesses the asset URL can readily access the file.
Access control is still enforced for other types of operations including uploading, deleting, moving, and copying.
#### Example use cases:#
* User profile pictures
* User public media
* Blog post content
Public buckets are more performant than private buckets since they are cached differently.</excerpt>

Bind the tenant context before using get_tenant_context() in Storage policies.
public.get_tenant_context() raises when app.tenant_id is unset. Supabase Storage provides JWT context to RLS, but this migration defines no path that sets this repository-specific GUC. Authenticated Storage SELECT and INSERT requests can therefore fail when the GUC is unset.
The policy applies to all rows in storage.objects. The scalar subquery is planned as an initplan, so the function can also run for statements targeting other buckets. The proposed CASE does not fix this: it still calls the function for partner-uploads, and the initplan may run before bucket filtering.
Use an authoritative tenant source that Storage requests actually provide, or set app.tenant_id in a trusted request path, before enabling these policies. Apply the same tenant binding to both policies.

Fixture poisons migration test DB URL
Medium Severity
The pg_engine fixture writes a Homebrew-only URL into REGENGINE_MIGRATION_TEST_DB when that variable is unset. Shared helper rls_postgres_or_skip treats a set-but-unreachable value as a hard failure, so the suite cannot skip or fall back to testcontainers. The write is process-wide and never restored, so later migration tests in the same worker inherit the same broken URL.

Require the derived row and its parent to have the same tenant.
partner_file_id references partner_files.id without comparing tenant IDs. The derived-row RLS policy checks only partner_file_derived.tenant_id, and PostgreSQL foreign-key checks bypass RLS. A permitted insert can therefore associate one tenant's derived evidence with another tenant's file; deleting that parent also cascades to the incorrectly linked row. Add a composite tenant-and-file foreign key in both migration paths. (postgresql.org)

Refuse downgrade when pointer-only manifest rows prevent restoration.
If any fsma.manifest_blobs.raw_bytes value is null, downgrade() drops manifest_blobs_payload_present_check and skips SET NOT NULL. The downgrade then succeeds with neither the previous raw-byte requirement nor the replacement payload check. Reject the downgrade before changing the constraint when such rows exist, or restore an equivalent validated invariant. As per path instructions: “Migrations must be reversible: downgrade() must actually undo upgrade().” (raw.githubusercontent.com)

Calculate the retention deadline independently of the session time zone.
Line 80 now recomputes delete_after on every update. Across a daylight-saving transition, timestamptz + interval '30 days' can differ from 720 elapsed hours in a DST-aware session. An unrelated update can therefore shift an existing deadline by an hour. Use a fixed elapsed interval, such as interval '720 hours', in this trigger and its Alembic mirror. (postgresql.org)

The v138 migration enables and forces row-level security on public.partner_files. It also defines the tenant_isolation_partner_files policy. This test disables RLS on that table before it inserts and updates rows, so its assertions cannot detect a regression in the tenant policies.
No other file under tests/ references partner_files. The new table has no test for unauthenticated access, cross-tenant access, or invalid input. Repository test guidance requires negative-path tests for security-relevant changes. Add a separate test module that keeps RLS enabled. It should check these cases:
Access without tenant context fails closed.
A session for one tenant cannot read or write another tenant's rows in partner_files, partner_file_derived, or partner_file_deletion_log.
Invalid input is rejected by the table constraints.
The applicable replay cases are covered.
Keep the trigger-timing tests in this file. Their RLS bypass is needed only to seed rows for those assertions.

Delta re-review of 290a5a0 against prior FAIL on ec956ff (review 5685449401).
Prior FAIL (1) fixed:_statements() semicolon splitter is gone. Upgrade is an explicit _UPGRADE_STATEMENTS tuple (28 complete statements). No runtime split. Comment ; and $$ function/DO bodies stay inside single strings. Each tuple entry is a full SQL statement (CREATE/DROP/ALTER/DO); no stray fragments. Partner-files DDL, trigger, RLS, and grants match supabase/migrations/20261028100000_partner_files_storage_retention.sql (normalized function + trigger identical). Alembic-only fsma.manifest_blobs payload-pointer helpers remain Railway-scoped as designed.
Prior FAIL (2) fixed: Trigger is now BEFORE INSERT OR UPDATE with no OF column list. Function always sets NEW.delete_after from report_delivered_at (NULL → NULL, else + interval '30 days'). A direct UPDATE of delete_after is overwritten; clearing delivery clears the clock. Supabase SQL matches.
Also verified (no regression):
Revision chain a3f7ce361b0e → b8c1e4f2a901
Downgrade drops policies → trigger → function → indexes → tables (derived before parent); manifest constraint restored safely
Upgrade idempotent shape (IF NOT EXISTS / OR REPLACE / DROP IF EXISTS then recreate; constraint drop+add)
RLS ENABLE + FORCE on all three tables; CASE + get_tenant_context() tenant scoping; append-only deletion log

Delta re-review 290a5a0..514ad7c (Hermes INSERT grant on storage_url). Read-only; migration not applied (SEL-549/SEL-590).
Checked:
Grant is GRANT INSERT (storage_url) ON TABLE fsma.manifest_blobs TO regengine only — runtime role, not PUBLIC / anon / authenticated / superuser. Role+column existence guards present.
Column-scoped as intended; created_at stays ungated (inventory + structural test assert it). UPDATE/DELETE/SELECT still denied on the table.
Alembic v138 grant statement matches Supabase 20261028100000_partner_files_storage_retention.sql verbatim.
Downgrade revokes first (REVOKE INSERT (storage_url) ... FROM regengine) before constraint/table teardown; revoke is also role+column guarded.
Idempotent/re-runnable: PG GRANT/REVOKE are no-ops when already in that state; IF EXISTS guards; v111 REGENGINE_COLUMN_GRANTS inventory lists storage_url so cutover keeps it across deploys.
Diff scope is grant-only: v111 inventory, v138 Alembic up/down, matching Supabase SQL, structural grant test, and two contract-test comment/assertion updates (created_at still rejected; storage_url forge case removed because the column is now intentionally writable on INSERT). No unrelated schema/RLS/trigger changes.

Missing created_at also fails READY via IS NOT DISTINCT FROM null semantics — conservative, not a false pass.
Numbering / other checks
10–17 block numbering unchanged. Check 15 (table-wide INSERT denied) still correct: has_table_privilege does not treat column-level INSERT as table-wide. Table/column names and privilege verbs match v138.
CI
Live DB suite (migrated scratch) at run 40735232655: queued (acceptable).

The current check rejects only created_at. A column-only INSERT grant on another column can still pass the table-level check and report READY = true. Enumerate every non-dropped column and reject any INSERT privilege outside the eight-column allowlist.

Add a live pointer-only storage_url INSERT assertion.
The changed test only rejects forged created_at. No live-DB test in the inspected test sources inserts raw_bytes IS NULL with a nonblank storage_url. Add an own-tenant pointer-only insert and assert tenant isolation. The structural grant test alone cannot detect an ineffective grant or RLS path.

Live DB retention gate failed: partner_files / partner_file_derived /
partner_file_deletion_log carry deleted_at but _RESOURCE_RELATIONS left
every policy unwired (#2900). Add RetentionPolicy entries, map them to
the public relations, and route soft/hard/cleanup SQL through
_sql_target. Skip deletion_log in the cleanup sweep (append-only audit).
WS-2c schema parity: regenerate docs/schema/expected_schema.json for the
three new tables; raw_bytes nullable on fsma.manifest_blobs is intentional
for pointer-only storage_url cutover (v138 payload check).
Delta from PASS a642a5eff94f…: one commit, three files (data_retention.py, test_data_retention_failure_modes_2900.py, docs/schema/expected_schema.json). Head unchanged at post time.
Blocker — hard-delete sweep vs grants
partner_files and partner_file_derived are now wired into _RESOURCE_RELATIONS and are not skipped by cleanup_deleted_records_job, so after deleted_at ages past 30 days the job issues SELECT/DELETE against public.partner_files / public.partner_file_derived.
v138 only grants regengineSELECT, INSERT, UPDATE on those tables (no DELETE). Runtime cleanup uses get_db() as that role, so hard-delete will fail with insufficient privilege once rows are eligible; the same 100 rows will be retried every tick and inflate stats["errors"]. Fix before merge: either GRANT DELETE (narrowly, matching the soft-delete→purge design) or skip hard-delete for these two policies the same way as the deletion log if metadata rows are meant to be retained after the S3 purge (#3733).
Deletion-log skip — OK (not the blocker)
Skip of PARTNER_FILE_DELETION_LOG is explicit, narrow, and keyed to that enum (alongside AUDIT_LOG), not an exception swallow. Justified: deleted_at is the event timestamp (NOT NULL default), hard_delete_days=0 would select nearly all rows immediately, and append-only is enforced by GRANT SELECT, INSERT + REVOKE UPDATE, DELETE (privilege denial, not a DELETE trigger — same practical outcome for regengine). Test change excludes only that policy from the expected failing set (mirrors AUDIT_LOG); it does not loosen failure detection. Nonblocking: skip is silent (no log line), same as the existing AUDIT_LOG continue.
Schema snapshot / payload check — OK
127→130 tables: exactly public.partner_files, public.partner_file_derived, public.partner_file_deletion_log. Only other snapshot edit: fsma.manifest_blobs.raw_bytesnullable: false → true. v138 manifest_blobs_payload_present_check is raw_bytes IS NOT NULL OR (storage_url IS NOT NULL AND length(btrim(storage_url)) > 0) — guarantees a payload; downgrade drops the check and restores NOT NULL only when no null raw_bytes rows remain. Snapshot format does not encode check constraints; column nullability matches the migration.
Other / CI
_sql_target schema-qualification looks correct. No unexpected paths in the delta. CI at this head still pending/queued (Backend 40754754335, PR Gate, Live DB suite, etc.).

Derived rows lack tenant FK
Medium Severity
partner_file_derived has separate FKs on partner_file_id and tenant_id, so a row can point at another tenant’s file. RLS only checks the derived tenant_id. Foreign-key checks bypass RLS, so this does not pin derived artifacts to the parent file’s tenant the way manifest_blobs uses a composite (ingestion_run_id, tenant_id) FK.
Hard-delete cleanup needs DELETE on partner_files and
partner_file_derived as regengine; keep deletion_log append-only.
Update v111 cutover inventory so re-apply keeps the grants, and prove
the runtime role can purge an expired structural row.
Structural test asserts GRANT/REVOKE strings + inventory tuples.
Live test: has_table_privilege DELETE true/false for sweep vs log; SET LOCAL ROLE regengine + tenant RLS; hard_delete_record on expired soft-deleted row; asserts deleted_ok and row gone (hard_delete commits).
Closes prior FAIL (hard-delete wired without DELETE privilege).
Merge main (v139 / e7a1c9d2f4b0 from #3741) and point the partner-file
storage retention migration at that head so alembic has a single head.

revision id b8c1e4f2a901.

Merge main (incl. v139 e7a1c9d2f4b0) then fix(v140) reparent — no rebase.
Migration renamed v138→v140; revision id still b8c1e4f2a901; down_revision a3f7ce361b0e→e7a1c9d2f4b0. Body byte-equivalent after those substitutions. Test + supabase comment same (supabase ≡ after v138→v140).
Grants unchanged: partner_files/derived SELECT+INSERT+UPDATE+DELETE; deletion_log SELECT+INSERT only (REVOKE UPDATE/DELETE); manifest_blobs INSERT(storage_url). v111 inventory keeps tenants SELECT=(id,status) from fix(db): grant regengine SELECT (status) on tenants (v139 / SEL-616) #3741 + partner DELETE / storage_url — no conflict with v139.
Single alembic head b8c1e4f2a901 (check_alembic_revisions.py OK). No leftover v138 filenames.
Structural test_v140 grant/statement tests: 5 passed on box (stub PG16 live tests not run — no local PG/docker).
Soft notes (non-blocking): comment-only "v138" leftovers in v111 inventory, data_retention.py, test_retention_relations_live.py, runbooks/verify-v126; PR title still says v138. test_v140 stub PG16 tests are not wired into any CI pytest list (pre-existing); Live DB suite does run tests/shared/test_*_live.py including partner DELETE privilege proof after upgrade head. Alembic revision guard + seeded rehearsals cover the chain.

Retention sweep fails without tenant context
High Severity
cleanup_deleted_records_job now selects from public.partner_files and public.partner_file_derived, which use FORCE RLS and get_tenant_context(). The job never enters tenant_scope and the begin hook clears app.tenant_id, so each sweep statement raises and expired rows are never hard-deleted.
The reason will be displayed to describe this comment to others. Learn more.
Storage RLS depends on unset GUC
High Severity
partner_uploads_select_own and partner_uploads_insert_own compare the first folder to get_tenant_context() for authenticated. Storage API sessions do not set app.tenant_id, so the helper raises and client upload or download on partner-uploads fails instead of authorizing the tenant path.

…erive waiver approvers from the principal (#3717)
* feat(evidence): enforce the traceability_events mutation policy and derive waiver approvers from the principal
S4 (fsma.traceability_events mutation policy). The canonical FSMA 204
evidence table had no database-level guard: any role with UPDATE could
rewrite the payload the chain hash covers, while fsma.cte_events has
had an append-only trigger since v107. Migration v138 (08c7cf36ddbe)
adds a BEFORE UPDATE OR DELETE row trigger and a BEFORE TRUNCATE
statement trigger that allow exactly what the production writers do:
the amendment path (status active -> superseded, amended_at) and the
rules engine markers (evaluation_verdict, no_verdict_reason,
evaluation_error, evaluated_at). Every other column change, any other
status transition, DELETE and TRUNCATE raise check_violation. The
comparison is to_jsonb(NEW) - allowlist = to_jsonb(OLD) - allowlist, so
it holds on every catalog variant of the table and keeps covering
columns added later. Break-glass is the existing fsma.allow_mutation
GUC, transaction-local; no grant changes.
Six live tests that legitimately mutate the table (tamper simulations,
cleanups, an orphaning helper) now set the GUC inside their
transaction. SCHEMA_CHANGE_POLICY.md section 5 gains the sanctioned
mutation surface (5.1) and the SEL-589 read freeze on fsma.cte_events
and fsma.critical_tracking_events (5.2).
S5 (waiver approver from the principal). PATCH /exceptions/{id}/waive
took waiver_approved_by from the request body, so a caller could sign a
waiver in anyone's name. The approver is now the authenticated
principal's key_id (401 when empty), the same rule the FDA export audit
identity follows; a body-supplied value is ignored. The frontend still
sends the field and is unaffected.
Tests: static and behavioural v138 tests against a live PostgreSQL
(allowlisted updates, the one status transition, rejected payload
updates and transitions, DELETE, TRUNCATE, break-glass, transaction
locality, idempotent re-apply, downgrade), chain health, revision
checker; exception router tests for the derived approver, ignored body
field, missing reason and empty principal. Live suites touched by the
GUC change pass against the scratch database at v138.

* fix(scripts): spell the key-dir mode as stat.S_IRWXU for the Semgrep gate
Ported from #3736. The Semgrep OWASP hard gate on main has flagged
scripts/create_test_api_key.py:140 (insecure-file-permissions) since
#3625 and the main merge carried the finding into this branch. The
directory mode is unchanged (owner read, write and execute only; the
key file itself stays 0600). This no-ops once main carries the fix.

* fix(evidence): make the v138 status-transition guard null-safe
The alembic catalog declares traceability_events.status NOT NULL, but
the Supabase twin leaves it nullable. With a NULL on either side,
NOT (OLD.status = 'active' AND NEW.status = 'superseded') evaluates to
NULL rather than TRUE, so the trigger let active -> NULL and
NULL -> active through without break-glass; a NULL status would drop
the row from the status = 'active' predicates the recall and FDA
exports use. The guard now uses IS NOT TRUE.
Behavioural test drops the NOT NULL constraint inside transactions the
rejection rolls back, covers both directions, and checks the constraint
is intact afterwards. Static test pins the null-safe form. Reproduced
(DID NOT RAISE) before the change; 23 v138 tests pass after it against
a scratch PostgreSQL 16 re-upgraded to v138.

* test(evidence): bound v141 trigger DDL lock wait, assert SQLSTATE directly, prove waiver replay is a no-op
CodeRabbit review on #3717:
- v141 upgrade() and downgrade() now run `SET LOCAL lock_timeout = '10s'`
before the trigger DDL (same bound as v134), so a CREATE/DROP TRIGGER
queued behind a long ingest or export transaction fails fast and can be
retried instead of holding every later writer behind it.
- The migration tests assert `exc.value.orig.sqlstate == "23514"` with no
fallback; rls_db forces psycopg 3, which always exposes it.
- New live test tests/shared/test_exception_waiver_replay_live.py drives
the real router and ExceptionQueueService against a schema-at-head
PostgreSQL: PATCH /waive twice for one case leaves exactly one signoff
(signed by the principal, never the body) and one status-change comment.

* test(evidence): pin that clearing a protected column to NULL is rejected by v141
Cursor Bugbot on #3717 claimed to_jsonb(NEW) drops SQL NULL fields, so a
non-allowlisted column could be cleared past the trigger. Reproduced
against a scratch PostgreSQL at v141: to_jsonb keeps a NULL column as a
null-valued key, and clearing a populated sha256_hash, chain_hash,
product_reference or from_facility_reference raises check_violation,
including behind a sanctioned status or verdict update. The only
"allowed" case is NULL -> NULL, which changes nothing. This test
populates those columns through break-glass and pins the rejection.
