Label application-only fields separately from legal requirements.

A harvesting upload without a lot-code column can report that the column is required by 21 CFR 1.1325(a), although that section does not list it. Select the reason by CTE and field. Use the application intake schema wording for fields without a legal basis, while retaining legal citations for fields that have one.

Review comment at @services/ingestion/app/gap_reasons.py around lines 50 - 55:
Update the missing-field reason selection around _SECTIONS so it considers both
CTE and field: use the application intake schema wording for fields without a
legal basis, including the harvesting lot-code field, and retain the legal
citation wording for fields that have one.

New reasons become unclassified issues

Medium Severity

The diagnostic allowlist gained invalid_field but not the other new reasons this PR emits: invalid_gln, missing_location, and broken_shipment_link. Those entries are rewritten to unclassified_parser_issue, so GLN check-digit failures and broken shipment-link warnings lose the specific reason the gap report is meant to show.

Diagnostic KDE rows ignore remapping

Medium Severity

Missing-KDE diagnostic issues still use physical line numbers from source_rows, while other row-log issues are remapped through record_indices. Files with multiline cells or leading blanks then report the same record under two different row indexes, breaking the diagnostic csv_record_index_including_header contract.
