This cherry-picks #723 (main 914d4fa, branch head 22c34aa) onto release/v0.1 at 4ccfb24. It's for v0.1 because of the case it fixes: an injected State line got the model to call set_policy for "describe this". The shell held that call behind a person-only Deny-first menu; it is now refused outright.

On a question, or on a line led by describe, explain, tell, show, list, summarize or report (and that states no want), a change that consent adds is refused without an ask. This covers the act call, act script and script-request paths. It brings the ADR 0002 D3 addendum, approved by the founder on 2026-11-05.

The patch is #723's merge diff, and it applied cleanly. It covers 13 files: shellcore, agentabi consent, the kernel act-inject case, and the xtask desk-term and act-inject expectations and fluency test.

Gates on main (#723):

  • Fluency: 48/50 at 169a792.
  • Shipped actbench: neutral, 108/142, the same as plain main.
  • full-ci: green at 22c34aa.
  • C: OK.

Test plan

  • The local door on this branch, at 8530362
  • full-ci on release/v0.1 (labelled)

The manager merges this by hand.

Cherry-picked from #723 (main 914d4fa, branch head 22c34aa) for
release/v0.1: an obeyed injection that would change the policy. On a
question or a describe-style line, a change consent adds is refused
without an ask; ADR 0002 D3's addendum, approved by the founder
2026-11-05.
