Release note correction (v4.0.1)

• Present the 4.0.1 outline tree response as normal behavior exposed through refreshed MCP catalog discovery, not as an LLM migration risk.
• Preserve the factual optional presentation: detailed alternative and exact response fields.
• This PR changes only CHANGELOG.md. No application code, schemas, dependencies or workflow modifications.

Evidence

• Markdown link scan PASS; v4.0.1 release-version validation PASS.
• 9 focused Node tests and Go project identity + tool catalog tests PASS.
• Main baseline b43787c already passed full push-event Test Suite and Scorecard. The corrected release commit still must satisfy PR checks and full main-push release gates before annotated tag.

The correction must be part of immutable v4.0.1 release commit, not a post-release documentation patch.
