…ukon39#2) targetStarted / HMR-recovery re-apply built bpInfo XML by hand without the condition field → conditional BPs (and hit-conditions) silently reverted to unconditional on re-apply. Route through the shared _build_bp_info_xml helper (already used by set_breakpoints) so the cached condition is preserved. +2 regression tests. Review yukon39#3 (_aggregate_breakpoints per-line "conditions" dict): not applicable — the cache stores a single condition per entry, which _aggregate_breakpoints already assigns to each of the entry's lines; there is no per-line dict.
