Add Claude Code MCP Server documentation

🎯 Goal

Add comprehensive documentation for the new MCP Server integration in the Clawra README.

📝 Details

The MCP Server was recently added (v4.2.0). We need:

1. A screenshot/video of the Claude Code integration in action
2. Step-by-step guide for connecting Clawra to Claude Code
3. Troubleshooting section for common connection issues
4. A dedicated docs/mcp-server.md file
