Description

The Commit workflow reaches progress assessment, but rejects the returned result with assessment-text-limit at $.requirements, publishes a failing verification check and retains the preceding progress state. The original requirement/source identity is reported as none, so the diagnostics do not identify which requirement or generated text exceeded the bound.

Expected: the agent's progress assessment satisfies the unchanged bounded response contract, or diagnostics safely identify the failing requirement/field and a useful remediation. No user acceptance criterion, verification threshold or scope should need weakening.

Actual: project code passes its own full CI, while Copilot reports agent.failed, phase assessment, reason assessment-text-limit, field $.requirements, requirement none, original issue line none. This differs from the earlier scope-phase scope-clause-limit report in #828: this run progresses beyond scope and fails on the assessment result.

Reproducing the issue

  1. Inspect vypdev/domus issue Test Automated Version #6 and draft PR Test Automated Version #7 on the existing Action-managed branch feature/6-implement-home-assistant-identity-and-sqlite-platform-persistence.
  2. Inspect exact head 16f28de5bcff87aba9aa0da4422042e2bb05b0cb (application household policy checkpoint).
  3. Open the push-triggered Commit run https://github.com/vypdev/domus/actions/runs/41190397084 and failed job https://github.com/vypdev/domus/actions/runs/41190397084/job/124505840706.
  4. Observe five internal agent-admission log entries followed by the final bounded assessment contract rejection. We did not rerun this job manually.
  5. Compare the independent exact-head project CI Check: https://github.com/vypdev/domus/actions/runs/41190401094. All six required job groups pass, including backend 100% line/branch coverage, architecture/doc checks and native amd64/aarch64 container builds/tests.

This is an observed run reproduction, not a minimized fixture or a claim that the model response is visible in public logs. We did not edit the installed Copilot resources, original acceptance, schemas or limits to make it pass. We did not merge or treat the skipped draft Bugbot review as acceptance.

Additional context and comments

  • Check Copilot / Verification ID 124506275755: failure with the same reference/field.
  • Branch synchronization and PR lifecycle jobs pass; draft Review is neutral/skipped, not an accepted review.
  • Backend checkpoint: 545 passing tests; 1,884 executable statements and 504 branches at 100%. Original SDD format/consistency and catalog validators pass unchanged.
  • All 29 protected installed Copilot resources remain byte-identical. Product work continues independently, without bypassing this check.

The assessment admission failure reproduced on the manual snapshot product increment. No manual rerun or acceptance relaxation was used.

The check reports phase assessment / assessment-text-limit / $.requirements / requirement none / original issue line none. Existing progress labels and published assessments were retained; no progress was published. This belongs to the assessment failure family tracked here, distinct from scope-clause-limit in #828. Queue execution completed normally; no queue-delay defect is alleged.

The exact head passes all six required CI Check groups, including native amd64/aarch64 image cases: https://github.com/vypdev/domus/actions/runs/41202996001. Backend: 892 tests, 3,309 statements and 862 branches at 100%; independent tooling: 66 tests at 100%; architecture: 41 fixtures and an 89-file/400-dependency graph, all three contracts kept. Original SDD/catalog validators pass the unchanged twenty-section format and file ownership. The companion is 69,215 characters with requirements and case inventory retained. Frozen lock, API schema equality and documentation links pass.

Copilot and all 29 protected installed resources remain unchanged. No issue requirement, gate or verification command was weakened. Manual consistent snapshot creation/inspection is implemented; restore activation and owner policy on newer replay history remain pending. PR remains draft; skipped/neutral Bugbot is not treated as completed review. Product work continues independently.

The assessment admission failure reproduced on the final restoration-inspection deadline fix. This is a normal new commit with a real regression test, not a manual workflow retry or a contract change. The preceding preparation commit separately reproduced scope-clause-limit and is reported in #828: #828 (comment).

    Exact diagnostic:

Progress assessment result was rejected (assessment-text-limit; field $.requirements; requirement none; source none). Existing progress was retained.
Impact: The returned progress result violated its contract; progress was not published.
    Error code: agent.failed
Action: Inspect the original requirement and response schema. Details: phase assessment, reason assessment-text-limit, field $.requirements, requirement none, original issue line none. none means no verified identity is available. A fresh attempt may help when the provider can satisfy the unchanged contract; do not relax acceptance or verification.
    Reference: 6fe4cf6e-2412-4a7e-ae67-f23768b38308

Unlike the preceding commit's scope rejection, this run reached assessment. That distinction is observed from the terminal diagnostic; it is not proof that the intermittent scope failure is resolved. The run admitted nine agent executions, retained prior progress and then failed the returned assessment contract. No run was manually restarted.

Every CI Check group passes for this exact head: https://github.com/vypdev/domus/actions/runs/41210642698 (aggregate check 124564844597). Backend: 1,088 tests, 3,761 statements and 992 branches at 100%. Tooling: 66 tests independently at 100%; 11 Node policy tests and type checking pass. Architecture: 41 negative fixtures, 94 files/444 dependencies, three contracts kept; mypy 60 source files. Ruff, local links, both native CPU-family image jobs and the unchanged SDD/catalog validators pass. The primary companion retains all twenty sections/full requirement-case inventory at 69,851 characters; the restoration companion is 22,895 characters. All 29 protected Copilot resources remain byte-identical.

This follow-up adds a final inspection-budget check. Its regression test initially failed with DID NOT RAISE StorageFailure and then passed. The preceding increment implements pure complete-operation reconciliation and real blocked private SQLite preparation/inspection; runtime history import/replay, consumed-recovery guards and verified offline activation remain explicit unfinished requirements. No requirement or gate was removed to fit this error. The owner's installed Edge.2 image is unchanged; pinned-image CI validates that existing package, not packaged restoration. No owner HA restore/household/device change was made.

Bugbot is still neutral/skipped on the draft and is not accepted as a complete current-head review. The PR remains unmerged. Product work continues independently without editing Copilot, weakening admission or changing its models/configuration.

Another ordinary push reproduces the same bounded assessment rejection on a separate, smaller maintenance issue. This is not a new UI-SDD attempt or a retry of the original issue #6 input.

    Observed log sequence:

    2026-10-31T21:09:18.5177212Z Agent execution admitted (codex codex-cli 0.166.0).
    2026-10-31T21:09:29.4892199Z Agent execution admitted (codex codex-cli 0.166.0).
2026-10-31T21:09:48.0512307Z Progress assessment result was rejected (assessment-text-limit; field $.requirements; requirement none; source none). Existing progress was retained.
    Error code: agent.failed

The independent exact-head frontend CI job passed: https://github.com/vypdev/domus/actions/runs/41242149829/job/124658067612 . It performed the frozen workspace install, strict owned type checks, real Chromium component coverage (all four supported metrics 100%), Vite/Storybook builds, four built-output browser journeys and five type/coverage/server-teardown guard tests. Backend job 124658067784 also passed: 1,411 tests, 4,704 statements and 1,308 branches, 100%. The aggregate was still waiting for the two container jobs when this follow-up was prepared; it is not claimed complete here.

Exact-head Bugbot independently completed all 3/3 partitions across all 31 changed files, with complete context, zero omitted/truncated items and no findings: https://github.com/vypdev/domus/actions/runs/41242149892 . Its progress-review success does not erase the separate assessment failure.

No generated assessment payload or offending requirement identity is exposed, so this is an observed recurrence, not a claim to know which returned text exceeded the limit. We have not manually rerun unchanged input, edited Copilot, relaxed response bounds or weakened acceptance. The same requested safe identity diagnostics and contract-conformant output remain relevant.

The complete historical reproduction report from this comment is preserved verbatim in the versioned issue #829 transcript, as part of PR #842.

Every original occurrence, source head, run, diagnostic reference, validation scope and limitation remains there unchanged. This preserves the full evidence in the reviewable diff instead of the truncated per-comment conversation excerpt. The issue requirements, fail-closed validation, scope, verification and coverage gates remain unchanged. Historical consumer results do not establish the hidden rejected assessment payload or successful adoption of this fix.

Future occurrences can use separate focused comments to keep each report completely reviewable.

Documentation-closeout head 509ec85: assessment fails, complete review and CI pass

Source 509ec85dd48207fc99f50d57f7dc2ac3e4a2b36c closes the planned task operation/migration guides and clarifies schema7 source versus immutable installed M1/Edge.3. Its 11 changed paths are documentation/specification/evidence only; backend and tooling runtime source bytes remain unchanged.

Original Commit run 41296096028 / job 124817149506 resolves the same Copilot SDK 3e7ceea and codex-cli 0.166.0. After all ten evidence parts, the final logged prompt length is 102,674 and issue-description length 15,287. At 2026-11-01T10:11:27.6283827Z final assessment again rejects with assessment-text-limit, $.requirements, requirement none, source none. Reference e98327a4-efb1-475e-9845-70882ce0400c; separate Verification check 124818001188 fails. Original response text is unavailable; no requirement identity or exact over-limit content is invented.

All project execution checks and aggregate CI Check 124817942656 / run 41296100017 pass at this exact head, including both architectures. Hosted backend reruns all 2,899 tests with 7,605 statements / 2,146 branches at 100%, then passes the independent exact-source audit.

Separately, full Bugbot review job 124817161483 completes all 22 partitions, 113 files and 168 diff fragments. Every context source is complete with zero omissions/truncation. Outcome no-findings; open/reopened/verification-required/unknown all zero, one historically dismissed Python3.14 false positive retained with native evidence and report #831. Review check 124818288794 passes. This successful review is not the failed progress assessment and does not conceal it.

Per the owner's explicit instruction, the reported Copilot progress failure does not itself block merging a feature with complete tests/coverage and complete clean review. No Copilot code/resource, prompt, bound, model, marker or acceptance check was changed, and no unchanged failure was retried.

    Journal-source observation 12d0e2a

Consumer Domus PR #15 at12d0e2a375af0232a597f4d2c4ef11718bd3e4cc: original Commit run41299096428/job124826116128. Final logged prompt length171,845, issue-description length37,251. The final progress assessment is rejected at2026-11-01T10:50:30.8423522Z with assessment-text-limit, $.requirements, requirement/source none; reference c9a7eca4-0bfb-4430-9aa6-73667a3ab462. Verification check124826539211 fails while preceding progress remains preserved. Raw response is not exposed; no verified rejected requirement identity is inferred.

This is a new implementation source (bounded immutable journal plus owned browser-storage adapter), not an unchanged-job retry. The local135-case Chromium campaign and exact runtime-inventory audit pass at100% of all four supported metrics across four authored modules. The new hosted Frontend application job124826126814 also passes. Independent Bugbot review is partial because of the separately reported genuine conversation limit #843, and published one valid Unicode-control finding, which we are correcting with real failing regressions; it is not blamed on progress parsing. Branch synchronization's distinct PAT workflow-write/abort issue is reported in #844.

No Copilot source, resource, configuration, prompt, bound or model was changed. Keep the complete requirement and original source evidence when fixing the final-assessment text contract. This report does not claim full interface acceptance or a complete clean review.

Verified new recurrence of assessment-text-limit while publishing progress for vypdev/domus PR #15.

    Source and exact execution:

    Observed contract diagnostic:

Impact: The returned progress result violated its contract; progress was not published.
    Error code: agent.failed
Action: Inspect the original requirement and response schema.
Details: phase assessment, reason assessment-text-limit, field $.requirements,
requirement none, original issue line none.
    Reference: 1925868e-6b1f-4e09-950c-97abe93ea7df

The failure occurred after the job emitted “Publishing result: 3 result(s), 4 step(s), 1 error(s)” and published the verification check. No verified requirement identity or original issue line was available. The bounded diagnostic does not provide the offending raw value or measured/allowed length, so I am not inferring either.

This is distinct from the same head's project CI failure: two existing package-contract assertions still expected Edge.3/the old three-stage recipe. They are corrected in Domus; the current complete local backend campaign passes 2984 cases with 107 modules/7900 statements/2228 branches at 100%. Both actual immutable-image jobs at 119e504 already pass 19 cases each, including old-image upgrades, restoration and exact packaged UI resources. No product failure is being relabeled as a Copilot issue.

The push-side Bugbot ownership skip is separate and expected while the PR-owned analysis executes. The PR analysis completed its code partitions with no unresolved findings; its independent conversation coverage problem remains reported in #843. No source, settings, credentials, workflows or acceptance/schema limits belonging to Copilot were changed, and no rerun loop or branch workaround was started. Work continues with normal commits on the Action-managed ref.

Expected: emit a schema-valid bounded assessment (or a precise useful diagnostic tied to the actual requirement) while preserving the existing contract. Please investigate in Copilot rather than asking the consumer to relax validation or shorten approved SDD scope.

The same assessment-phase contract failure recurs on Domus PR28, exact head 3a2c4b33769f26eaa97bd9eeb0c1ce3a6d4fdb50.

Consumer change: tests/evidence only. Actual console restore plus an exact original PUT replay cannot recreate the removed alias. Real closed-tab restoration uses the public Chromium sessions API in a disposable profile without journal injection. Normal no-store navigation and a separate explicit BFCache browser policy are distinguished. Whole local compiled Python/SQLite campaign:87passed, zero skipped/flaky, both deployments. Production runtime remains byte-identical to the prior verified head with1,231frontend and4,374backend tests/100% supported coverage. New-head CI executes those suites independently; no full186-case acceptance is inferred.

    Reproduction/evidence:

    • Consumer issue ✨🧑‍💻 - Complete the household administration workspace after the verified API and controller checkpoint domus#27; existing managed ref feature/27-complete-the-household-administration-workspace-after-the-verified-api-and-controller-checkpoint.
    • Push workflow41623348165/job125874378762. The exact run URL is checked against job metadata before publishing this comment.
    • Actual rejection timestamp 2026-11-03T10:58:45.8077392Z.
    • Progress assessment result was rejected (assessment-text-limit; field $.requirements; requirement none; source none). Existing progress was retained.
    • agent.failed; phase assessment, reason assessment-text-limit, field $.requirements; no verified requirement/source identity. RetryableYes; previous progress preserved.
    • Reference f99c64e7-bf1e-454a-ab9b-35a8f41efb10.
    • The candidate assessment text, observed length and attributable requirement are not exposed. No response contents, root cause or requirement mapping are inferred. Please investigate the producer/validator contract while preserving every requirement and the existing bounds.
    • Independent native review41623357628/job125874412581 has complete context, all9/9partitions, zero candidate/published/open/reopened/verification-required/unknown findings; one historicalfixed finding. The push Bugbot ownership skip is not counted as a second accepted review.
    • Independent CI Check41623357791/job125877182730 now passes all applicable product jobs for this exact head, including 1,231 frontend tests, 4,374 backend tests, full supported coverage, 87 compiled browser journeys, 26 catalog journeys and 32 actual container checks on each CPU architecture.

Copilot source/resources/workflows/configuration/models/credentials/bounds and all owning SDDs are unchanged. No manual retry, forced finding resolution, scope reduction or acceptance/coverage relaxation. Full raw diagnostics are retained privately; no owner HA credentials, session cookies or identity payloads are published.

    Current-head recurrence:9988e44849a9c50a11718aecf88fddddf3e6082d

The unchanged assessment contract failed again at2026-11-03T12:27:37.1659471Z in Commit run41635441044/job125915337610. Diagnostic: phase assessment, reason assessment-text-limit, field $.requirements, requirement none, source none; error agent.failed; reference 84744b84-b47c-4bfc-a465-699299cff372. Synthetic check Copilot / Verification125916853070 reports the same rejection and confirms previous progress labels/assessments retained, retryableYes. Two complete bounded comparison parts were collected before final assessment. The log exposes no verified offending requirement or response length; no unsupported identity or root-cause inference is made.

Separate native PR review completed9/9 and published one finding; this progress failure does not substitute for its review result. No Copilot changes, reduced scope, acceptance/verification relaxation or blind retry. Independent product CI continues against the exact head.

Recurrence on the Edge.6 installation increment: fb92abe17594d1d2e6bf32d50ecd48b97d55fc51

The unchanged assessment contract failed again in consumer Domus PR #28, issue #27, on the existing Action-managed branch. This increment packages the reviewed administration runtime as Edge.6, freezes the original Edge.5 recipe independently and adds actual installer-image compatibility checks; it does not change Copilot.

    • Exact source head: fb92abe17594d1d2e6bf32d50ecd48b97d55fc51.
    • Actual Commit workflow 41682010334 / job 126077831354. The Actions job metadata was checked before retrieving its logs; the separate synthetic check ID is not treated as an Actions job ID.
    • Rejection at 2026-11-03T17:15:49.9391433Z: Progress assessment result was rejected (assessment-text-limit; field $.requirements; requirement none; source none). Existing progress was retained.
    • Error agent.failed, phase assessment, reason assessment-text-limit, field $.requirements; original issue line/verified requirement identity unavailable. Reference 7650f2d5-ab3d-445f-ab4f-e2f0974c37fb.
    • Synthetic check Copilot / Verification ID 126079619915 reports lifecycle blocked, succeeded 2 / failed 1 / skipped 0, previous progress labels/assessments retained and retryable Yes. This describes that workflow's result, not a product implementation blocker.
    • The response text and its observed length are not exposed. No candidate requirement, response content or underlying root cause is inferred from the safe none fallback.
    • Separate native PR review 41682018756 / job 126077862243 completed at the same source with complete context and all 11/11 partitions. Telemetry: candidate 0, published 0, open 0, reopened 0, verification-required 0, unknown 0; historical fixed 5 / obsolete 1; outcome no-findings. All six historical threads remain automatically resolved by the reviewer.
    • Complete local product verification: 4,395 backend / 1,258 Chromium frontend / 178 tooling tests; complete supported global/per-file coverage and independent source inventories pass. The actual pinned aarch64 Edge.6 installer image passes four new container checks and all 32 administration browser journeys. Both hosted CPU jobs and the remaining application CI jobs are still running; local evidence is not substituted for those gates.
    • The original seven SDD policy modules are byte-verified against SDK source 3e7ceea08e6131360e026937d1c0cc1b120472e0; all 20 owning documents and 186 required cases remain retained. Copilot source, resources, workflows, configuration, models, credentials and bounds are unchanged.

Please investigate the assessment producer/validator contract while preserving complete requirement provenance and unchanged acceptance bounds. No blind retry, shortened SDD scope, forced finding resolution or relaxed verification was used. Independent product work continues; actual owner HA adoption and human UX are not claimed.

Hosted verification has now completed for the same unchanged head: CI Check41682018322 / aggregate126084155269 passes all ten applicable jobs. Each actual amd64/aarch64 job passes40container cases and all32administration journeys with the exact-count/no-survivor gates. Hosted frontend1258/backend4395/tooling178 and independent complete-coverage audits pass. This updates the earlier in-progress observation only; the assessment rejection remains reproducible and Copilot is unchanged.

Recurrence on distinct HA directory boundary acceptance: e5f08ebe46f1ae3e2f81080746c0fd76c705b9fc

The same assessment-phase rejection occurred after an ordinary tests/evidence-only push to Domus PR #28, issue #27. No runtime, package, permission, Copilot, scope or verification change was made.

    • At 2026-11-03T19:10:29.2070938Z, all 55 required paths are represented by three complete bounded source parts, recovery usage null. Each partition completes. At 19:11:27.8374671Z, logged prompt length is 66,095 characters and issue description length 29,542.
    • At 2026-11-03T19:11:48.1821156Z: Progress assessment result was rejected (assessment-text-limit; field $.requirements; requirement none; source none). Existing progress was retained.
    • Primary error agent.failed, phase assessment, reason assessment-text-limit, field $.requirements; original issue line and verified requirement identity are unavailable. Retryable Yes. Reference 307fc7fe-b54e-4c6f-82cc-9def00414b61.
    • Existing progress labels and published assessments are retained. The returned candidate text, its actual length and attributable requirement are not exposed; none is inferred. This is distinct from the preceding scope-clause-limit / $.lines occurrences in 🐛🧑‍💻 - Progress scope assessment rejects an API requirement with scope clause limit in generated issue plan metadata #828.
    • Independent native PR review 41700555989 / job 126142397778 completes all 12/12 partitions with complete context on this exact head. Telemetry: candidate/published/overflow/open/reopened/verification-required/unknown all zero; historical fixed6 / obsolete1; outcome no-findings. The push-side ownership skip is not counted as a review.
    • Product increment: six real deterministic WebSocket-provider cases prove exactly256 distinct canonical accounts succeed and distinct257 or a malformed256th account never certify a valid prefix, in both deployments. Standalone own-profile proof remains separate from unavailable directory metadata; native technical credentials never select a human through their own current_user.
    • Local complete backend: 4,402 passing cases, all151runtime modules independently audited at100% supported statement/branch coverage (10,438 statements / 2,928 branches). The39provider cases, style, links and unchanged original seven-module SDD validation pass. All20owner documents/186required cases remain. Current full product CI41700555842 is still running at report time; already-completed tooling has179passing cases and100% coverage. Prior runtime/image evidence is not substituted for fresh hosted gates.

Please investigate the producer/validator contract and safe requirement provenance while retaining the complete scope and unchanged bounds. No blind retry, manual finding resolution or acceptance relaxation was used. Copilot source/resources/workflows/configuration/models/credentials/bounds remain untouched; owner HA installation/UI, restoration and actual human UX are separate pending work. Full raw diagnostics are retained privately; no credentials, cookies or owner account payloads are published.

Additional observation of the existing assessment-text-limit failure in ha-ui (distinct from scope-clause-limit in #828).

No Copilot source, installed resources, requirements, bounds or managed ref was changed to bypass this failure. No manual unchanged-job retry was performed. Rejected output contents and actual-versus-allowed lengths are unavailable in the inspected log; no deeper cause is asserted. Please investigate upstream while we continue admitted product work. The GitHub connector could read this issue but rejected comment publication with HTTP 403, so this report uses the existing maintainer CLI session.

Further exact-head observation of #829 after the four real navigation bindings were implemented; same published assessment contract diagnostic, not a new authorization or product test failure.

No Copilot source, installed resources, output limits, issue requirements or verification gates were changed. No manual unchanged-run retry was issued. This is still an unpublished navigation-only increment; no full feature completion or merge is claimed. The failed assessment's actual output length/content and root cause remain unavailable in inspected public diagnostics. Please retain fail-closed behavior and investigate/roll out the upstream fix; product implementation continues independently.

Two further exact-head observations of the same assessment-text-limit rejection in the ha-ui consumer. Existing progress was preserved in both cases; no consumer scope, acceptance bound, Copilot source or installed resource was changed.

Consumer scope remains vypdev/ha-ui#7 and vypdev/ha-ui#10. All 64 original specification cases plus six delivery cases remain required; the feature is still incomplete. The observed push comparisons are 213f5273348651c88e78e506c7c6fd4a823f7846 -> 8aefb26503a8882a5093c431b43afb6d5864348c and 8aefb26503a8882a5093c431b43afb6d5864348c -> ecb57a83ee9600574982035baa77993c20db4335.

Both actual setup logs resolve vypdev/copilot@v3 to 3e7ceea08e6131360e026937d1c0cc1b120472e0; admitted agent runtime is codex-cli 0.166.0. Both logs contain:

Progress assessment result was rejected (assessment-text-limit; field $.requirements; requirement none; source none). Existing progress was retained.
    Error code: agent.failed
Details: phase assessment, reason assessment-text-limit, field $.requirements, requirement none, original issue line none.

The diagnostic exposes no verified requirement ID/source anchor and no actual text count or bound. No claim is made about those missing facts or a causal relationship with either source change. No unchanged manual rerun was dispatched. The second run's CLI log retrieval also printed a missing-log notice for a separate approval/check item; the actual Copilot Commit stream supplied the source pin, range, runtime and diagnostic quoted here.

Native Bugbot separately identified a test-assertion gap on the second head. Its source correction is a normal subsequent commit and does not modify Copilot or explain this assessment contract rejection. Please retain fail-closed validation, full coherent scope, prior progress and exact-head binding while investigating upstream recovery. This remains separate from the scope-clause-limit observation reported under #828.

A further ordinary push in ha-ui reproduces the existing assessment-text-limit diagnostic. This execution still uses the original pinned v3 revision below; this observation does not establish a regression in a newer upstream revision.

    Original log observations:

    2026-11-04T08:46:07.6865767Z CheckProgress: prompt length=87628, issue description length=23344.
    2026-11-04T08:46:07.7219363Z Agent execution admitted (codex codex-cli 0.166.0).
    2026-11-04T08:46:45.5133985Z Agent execution completed.
2026-11-04T08:46:45.5194315Z Progress assessment result was rejected (assessment-text-limit; field $.requirements; requirement none; source none). Existing progress was retained.

Published impact: returned progress violated its contract and was not published; agent.failed; phase assessment, field $.requirements, no verified requirement/source anchor. Reference: dce88eb5-4260-4332-bbb3-3dadff172f91. The visible log does not expose the rejected output or its measured allocation; prompt/input lengths above are not output length evidence.

Independent product CI succeeded: https://github.com/vypdev/ha-ui/actions/runs/41801411679/job/126477803692 . It runs 389 Node/Chromium tests with all four runtime coverage metrics at 100%, strict types, five reproducible packages/four clean compiled framework consumers, and the complete static Storybook browser gate across 32 locale/theme/width combinations. The separate complete native PR review reported one registration-test reliability finding, which is being fixed rather than dismissed. That review and this progress-publication failure are separate outcomes; a green CI does not make the complete feature accepted.

No manual rerun, acceptance relaxation, Copilot source/installed-resource edit or managed-branch replacement was used. All 64 primary cases and six companion refinements remain required. The original diagnostics, actual failure inputs, source/lock identities and product verification outputs are retained locally; existing published progress is unchanged.

Another ordinary ha-ui source push reproduces the same assessment-phase text-limit diagnostic on the original immutable v3 runtime; this does not establish a regression in a newer upstream revision.

No unchanged manual rerun, acceptance relaxation, Copilot source/installed-resource change or managed-ref replacement was used. All 64 primary cases and six delivery refinements remain required. Product work continues independently with a real, locally reproduced form-method collision fix; source, raw diagnostics and actual failed inputs are retained.

The same bounded progress-text guard rejected another ordinary ha-ui increment, at 209b5d8547bbda7b5e7fd27271f5a5904e4acf2a:

This is the same old pinned v3 runtime, action SHA 3e7ceea, codex-cli 0.166.0 / gpt-6-luna. It does not demonstrate a regression in newer upstream code. No Copilot source/installed resources, primary requirements, original cases, coverage thresholds or assessment limits were changed. Please preserve fail-closed schema/identity/text bounds while investigating the original model/assembly diagnostics.

Additional occurrence on the following ordinary test/documentation push, 433f5c700e0f2db62ea966430c28c2654e49f0f6:

    • Run https://github.com/vypdev/ha-ui/actions/runs/41818792538/job/126536337595 . Four bounded parts were announced at 2026-11-04T11:10:00.6482432Z; assessment was rejected at 2026-11-04T11:13:41.1222222Z with the same assessment-text-limit / $.requirements / requirement none / original source none. Reference b5669e3d-9187-4c56-9709-a59a3228a33b, retryable Yes; retained prior progress.
    • Current product CI https://github.com/vypdev/ha-ui/actions/runs/41818799679/job/126536362226 succeeds with 527 tests and unchanged complete coverage. Native PR Bugbot https://github.com/vypdev/ha-ui/actions/runs/41818799680/job/126536362227 independently completes all 11/11 partitions, complete context, zero candidates and zero open/reopened/dismissed/verification/unknown findings. Its aggregate binds exact head 433f5c7, digest b27da677.
    • No Copilot runtime/resource, requirement, coverage or schema/length-bound change was made. The new head adds permanent native-readonly conformance tests and documentation; it changes no product runtime code. Hidden model text and exceeded limit remain unobservable, so no payload/root-cause claim is inferred.

The next selector increment reproduces the same final-assessment guard at 913bab7eda0bba7be464bf58847889e337f067e0:

    • Actual job https://github.com/vypdev/ha-ui/actions/runs/41826554235/job/126562633696 . Four bounded comparison parts were announced at 2026-11-04T12:09:38.1455939Z; final assessment was rejected at 2026-11-04T12:13:22.6809600Z. Reason assessment-text-limit; field $.requirements; requirement none; original source none; error agent.failed, phase assessment; reference 70bb89c5-de08-4442-bbf0-022485cf7f26. Prior progress/labels were retained. The hidden final payload and exceeded bound are not observable; none is not a verified requirement identity.
    • Current product CI succeeds: https://github.com/vypdev/ha-ui/actions/runs/41826563065/job/126562664227 . It includes 664 passing tests, independent 39-module 100% statement/branch/function/line coverage, strict declarations, frozen five-coordinate reproducible packaging/four actual compiled consumers, original catalog SDK and 64 static catalog locale/theme/width combinations.
    • Native PR Bugbot independently succeeds: https://github.com/vypdev/ha-ui/actions/runs/41826562736/job/126562663194 . Complete context and 13/13 partitions over 165 files/181 fragments; candidate findings zero. The authoritative aggregate binds exact head 913bab7, digest 69f31eb2, complete outcome/coverage, zero open/reopened/dismissed/verification/unknown, fixed=4, obsolete=1.
    • The same old action runtime 3e7ceea, codex-cli 0.166.0 / gpt-6-luna remains installed. No new-upstream regression, raw payload cause, or permission failure is inferred. Copilot files/resources and schema/text/coverage gates remain unchanged; all 64 baseline cases and six delivery refinements remain required, and the incomplete feature is not being merged.

Retrieving the whole push run via gh also encountered its synthetic-check log IDs; the actual Actions job above supplies the complete error log. A missing synthetic job log is not being reported as another execution failure.
