feat(eve): harden the instrumentation bus and wire providers to it

Summary

Related to #1659. Stacked on #1814 and still gated by experimental.instrumentationProviders.

This makes the provider bus durable and wires authored providers into the runtime. Every operation now has replay-stable identity; provider state and timeout-abandonment tombstones survive worker boundaries; late writes are revoked; and omitted model terminals are cleaned up with their attempt. Turn and action keys include their session/turn ownership, preventing unrelated operations from sharing rows or abandonment state.

action.* now comes from eve's durable runtime dispatch boundary and covers tools, skill loads, subagents, remote agents, approvals, and replacement-worker results. AI SDK callbacks independently retain tool.call.*, so ordinary tools emit both views. eve's agent.action span persists its ID, parent, timing, kind, and name, then reconstructs/backdates at the terminal without retaining a live span in the originating worker.

Provider setup receives evaluation?: { runId } for local eval servers. Runtime shutdown reaches the registered tracer provider, which shuts down every collected processor.

Validation

• Full eve unit suite: 6,162 passed, 1 skipped
• Full eve integration suite: 618 passed
• Focused eval, local tracing, registration, and compiled-artifact scenarios: 14 passed
• tsc -p packages/eve/tsconfig.json --noEmit
• oxlint on changed sources; pnpm guard:invariants
• E2E not run locally
