feat(eve): add protocol v2 headless setup events

Summary

Related to #1786. Stacked on #1787.

Adds in the desired eve add <item> --headless flow, designed for agent use. Output is in NDJSON, and is designed to be invoked repeatedly with progressively more --answer flags until a comprehensive plan can be constructed. At that point, the plan is executed and actual mutations to external systems / code occur.

This is a breaking protocol change, in that registry items with guided setup flows will need to be installed using the latest eve version.

Validation

• pnpm --filter eve exec tsc -p tsconfig.json --noEmit --pretty false
• Focused unit suite: 162 tests passed
• pnpm exec oxlint packages/eve/src/cli/commands packages/eve/src/setup
• git diff --check
