fix(composio): bound upstream MCP initialize

Summary

• bound configured upstream MCP initialization to one second
• return the local initialize result promptly when the upstream stalls
• preserve initialization-before-notifications/initialized ordering
• add a never-resolving upstream regression case

Verification

• 6 focused connector-proxy tests pass
• Node 24 / pnpm 10 typecheck passes
• production build passes
• exact diff and working tree are clean

Contributor follow-up for milind-soni#501 and issue milind-soni#481. Merging this branch into fix/composio-mcp-initialize-handshake updates the existing upstream PR without replacing its history.
