Require qualified model identities at selection boundaries - #145

Require model@provider at every text and session-selection boundary so model and provider cannot drift across CLI, journal, serve, view, or swarm paths. Keep registration as a bare id plus keyword provider and migrate legacy journal and swarm records when both fields are available. Co-authored-by: EvoAgent <evo@ruiqilei.com>
