Add a changelog for the audit fixes and update the README - #41

CHANGELOG.md records the 2026-10-08 to 2026-10-09 audit fixes (#11-#40) by area, with a link to each pull request, and the one deliberate non-change (English stays the default language). The README's structure section now says each chapter has a problem-set file and a quiz file under lib/ (they were split in #38), and points to the changelog.
