Summary

Engine Review Pass runs on every PR, review and comment event. A run that evaluates a head before its review lands fails, and that failed job stays on the SHA after a later run passes. The PR stays BLOCKED until someone reruns the old run by hand. #3782 needed ten reruns across two heads.

When a verdict passes, the job now reruns the oldest failed, cancelled or timed-out Engine Review Pass job at that SHA, by job ID. The rerun re-reads the reviews and, if it passes, reruns the next one, until none are left.

  • One rerun at a time, because a queued run is cancelled by the next one in the concurrency group.
  • Repairs only touch this PR's own runs, because another PR at the same SHA shares the status.
  • Repairs are skipped if the PR head has moved, because a rerun would replace a queued evaluation of the newer head.
  • A rerun GitHub rejects, such as an expired run, is skipped so newer runs still get repaired.
  • A run counts as stale only if Engine Review Pass itself failed or was cancelled in it. Otherwise a run that is red for another job would rerun forever.
  • The job gets actions: write. It runs only the script on the base or PR merge ref and never checks out PR code.

If this step fails, nothing gets worse: it logs a warning, the job and status are unaffected, and the PR stays blocked as it does today.

Two fail-open gaps closed

Both are on main today and were reported by Codex.

  • Behaviour change: a native "changes requested" review now blocks the gate. A member's latest decision stands until they approve or it is dismissed, matching GitHub's rule. It used to be ignored unless it held a REVIEW: marker. Reviews from non-members still do not count.
  • Blocking comments can no longer leave an old success in place. Comment runs have no job under the required name on the PR head, so a run that died or could not post kept an earlier success. Every comment run now replaces the status with pending before evaluating, because a queued run with a marker can be replaced by a run without one, so those cases leave the PR blocked. A comment run that cannot post a status now fails, which shows up as a red run on main. The cost, chosen deliberately: a brief pending on a passing PR whenever anyone comments, and a comment run that dies leaves a previously passing PR pending until the next evaluation. Staying blocked is preferred to keeping a stale success.

The script's API calls also retry 3 times.

Editing a native review now re-evaluates the gate.

Accepted gap, by maintainer decision: if every status write in a comment run fails after retries, including the first, on a head that already has a success, the earlier success stands. The run goes red on main, but nothing on the PR head can show it. Codex reported this as blocking at 565c4b59. No status-based gate can close it, because it needs a write to succeed. Accepting only review markers would close it, but 21 of the last 40 merged PRs used comment markers.

Second accepted gap, by maintainer decision: if a comment run cannot load the PR after retries, it has no head SHA to post to, so an earlier success stands. The run goes red on main.

Third accepted gap, by maintainer decision: an issue_comment run that is cancelled by hand, or whose runner fails before the script starts, never posts pending, so an earlier success stands. A queued run replaced by a newer one is covered, because the newer run posts pending itself. No script change can close it. Accepting only review markers would close all three gaps, but that changes how reviews are posted, so it is left for a separate change.

Known and not changed here, both on main today:

  • PRs that share a head SHA share one status, so a passing PR can overwrite another PR's failure. The first attempt at fixing this introduced races.
  • A PR can edit this workflow and post its own result. The workflow's own comment already describes this job as an audit trail and not a cryptographic gate.

Accepted by maintainer decision: two availability risks this PR adds, both of which leave a PR blocked and neither of which can cause an unsafe merge.

  • A push landing between the head check and the rerun can displace the queued evaluation of the new head. It clears on the next event or a manual rerun. Keying the concurrency group by SHA would remove this, but comment runs have no SHA, so a stale success could then overwrite a blocking comment.
  • The extra pending writes use GitHub's limit of 1,000 statuses per SHA and context faster. A head at that limit stays blocked until a new commit.

Why not the earlier approach

The first commits on this branch renamed the job so only the commit status carried the required name. Codex rejected that twice with five P1s. Once the job stops carrying the verdict, an old success status stays authoritative until something overwrites it, and a race, a read-only fork token or a timeout can each prevent that write. Keeping the job name removes all of those, because the job can fail without any token and shows as pending while it runs. This also needs no fallback check run.

Live test on this PR

  • A passing review at c5eba42f reran the failed Engine Review Pass job with nobody touching it: review run 41990142402 logged the rerun, and run 41989961844 passed on attempt 2. The result: no failed or cancelled Engine Review Pass entries in the check rollup, which #3782 needed by hand.
  • Merge state reaches CLEAN once the normal pipeline finishes at this head

This covered one stale run. A chain across several stale runs has not run live.

Test plan

  • Ran the real script against a mocked GitHub API in 47 scenarios: all verdict paths, native review decisions, member and non-member marker comments, lost status writes, oldest-first rerun, timed-out runs, a rejected oldest run, runs red for another job, end of chain, never rerunning itself, other workflows, in-progress runs, a failing verdict never rerunning, comment events, and both rerun API failures.
  • YAML parses. Job name, verdict logic and failure behaviour match main.

Notes

  • Fork PR runs have a read-only token, so their reruns fail and they keep today's manual-rerun behaviour.
  • Reruns use the workflow version of the original run, so runs from before this merges have no rerun step. A chain over those stops after one rerun, and later passing runs pick up the rest. Stale runs on open PRs clear as they get new evaluations.

🤖 Generated with Claude Code

The Engine Review Pass job and the commit status it posts shared a name,
so both counted toward the required context. A commit status holds one
latest value per SHA, but check runs pile up per event. A run that
evaluated a head before its review landed left a failed check run behind,
and that kept the PR blocked after a later review passed. PR 3782 needed
ten runs rerun by hand to get unblocked.

Rename the job to Engine Review Evaluator so it no longer matches the
required context, and stop failing it on a missing or blocking review:
the status carries that verdict. The job now fails when the status cannot
be posted, which used to be only a warning, and API calls retry.

Walkthrough

The workflow updates commit-status handling for issue-comment runs and searches prior runs on the current head SHA for eligible evaluations to rerun. It also includes allowed reviewers’ latest native review decisions in the blocker calculation. A latest CHANGES_REQUESTED decision blocks alongside blocked engine markers.

Estimated code review effort: 3 (Moderate) | ~20 minutes

Merge Risk: Low · up to 565c4

The automatic repair of stale review-gate runs may not work for cancelled or timed-out runs. Those runs could stay red until someone reruns them manually. The review gate itself is unaffected, so this is a bounded problem to address or accept before merging.

Security architecture risk: Low · up to 565c4

The change strengthens review blocking and adds automated workflow repair. No newly exploitable bypass was established, but reviewer restrictions, required-check enforcement, and effective workflow permissions could not be verified.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • observed — The job newly requests actions:write. Its repair selector is constrained to the current repository, SHA, and workflow name, but the operation reruns failed jobs for the selected workflow run, not exclusively Engine Review Pass. Other failed jobs can therefore execute again with their own declared permissions, including checklist-comment writes and Head SHA Check Guard status writes. Actual co-failed jobs and effective runtime token scopes were not supplied.

Security Findings and Attack Paths

  • inferred — The deferred replace-before-trust-filter candidate does not establish a new authorization bypass. Native decisions replace entries only for the same authenticated login, so another account cannot erase a trusted reviewer’s blocker. Same-login association changes remain unresolved. Moreover, native blocking was absent at the base, while the existing marker-blocking conditions remain in place; bypassing only the added native condition would not demonstrate worsened acceptance relative to the base.

Trust Boundaries and Controls

  • observed — Marker processing requires OWNER, MEMBER, or COLLABORATOR association, an allowed engine, and the current head SHA; author-engine PASS markers are excluded. The preliminary pending write also requires an allowed association. Native blockers are computed separately from raw reviews, retain reviewer-login ownership, and exclude ordinary comments from decision replacement.

Resilience and Maintainability Implications

  • inferred — A successful pending write improves interruption safety by replacing an earlier success before comment evaluation finishes. It does not provide an atomic fail-closed transition: if both pending and final writes fail, an earlier PR-head success can remain, although the comment workflow is marked failed. This residual outage condition predates the PR’s mitigation and is not shown to be worsened. Required-check enforcement and platform status ordering remain unverified.

Hardening Proposals

  • proposed — Consider job-scoped repair to keep automated retries within Engine Review Pass. Verify repository review restrictions, required-check enforcement, and effective token permissions before treating the gate and its recovery paths as fully fail-closed.

With the job renamed, a failed status write only fails a non-required
job. If an earlier review had already posted success, a later blocking
review that could not be written left the PR mergeable.

When a failure verdict cannot be posted, create an Engine Review Pass
check run with a fixed external_id, which takes the required name for
that case only. The next posted success clears it, so normal runs add
no check runs under the required name and stale failures do not return.

REVIEW: FAIL codex d5020b5

Reviewed by Codex CLI 0.166.0 (gpt-6.1-sol) in its read-only sandbox, started from a Claude Code session on the author side. The prompt asked for correctness, fail-open behaviour, concurrency, token permissions and test gaps, and did not include the earlier reviews.

Found two merge-blocking defects.

  1. [P1, merge blocker] A blocking review can leave an earlier success intact when the token cannot write. pr-guards.yml:1315

If posting the failure status fails, the fallback uses the same token to create a check. For fork PR review events, that token normally has read-only permissions, so both writes fail. A prior success—posted by an issue_comment run with write permissions—remains valid despite the blocking review. The evaluator fails, but its renamed context, Engine Review Evaluator, is not required.

This is a concrete permission failure, beyond a hypothetical API outage. GitHub documents the fork token restrictions. Binding the required context to GitHub Actions does not help: the stale success came from that app too.

Confidence: high. Executing the actual script with mocked write-denied APIs preserved success after processing BLOCKED, while marking only the evaluator failed.

  2. [P1, merge blocker] Different PRs sharing a head SHA can overwrite each other’s verdict. pr-guards.yml:1174, pr-guards.yml:1304

Concurrency is keyed by PR number, and reviews are fetched for one PR, but the required status is keyed only by repository, SHA, and context. Create two branches at the same commit: PR A has a blocking marker; PR B has a passing marker. A posts failure, then B posts success to the same required status. A’s blocking marker still exists, but its required context now passes. The same mechanism can supply success to a PR with no passing review of its own.

Removing the required job name leaves no PR-specific failing evaluator check to counter that overwrite. Serializing by SHA alone would not fix the underlying problem: sequential evaluations also overwrite each other.

Confidence: high. Running the actual script sequentially against two mocked PRs sharing a SHA produced failure for A followed by shared success for B. GitHub evaluates required checks against the commit SHA.

Non-blocking test gap: The diff adds no tests, and repository searches found no targeted coverage for this evaluator. Regression coverage should include both scenarios above, API read failures after a prior pass, and fallback-check creation and recovery.

Verification limits: The reproductions used the workflow’s JavaScript with mocked APIs; I did not exercise live GitHub branch protection, check-suite selection, repository fork-token settings, or fallback-check updates. I used your stated required context and app binding. No files were modified.

… gate

Renaming the job left no failure signal that was specific to one PR or
that a read-only token could write, which a Codex review showed in two
ways.

The commit status belongs to the SHA, so a PR with a passing review could
overwrite the failure of another PR at the same commit. The status now
passes only if every open PR at that head passes.

A fork's token cannot post a verdict, so a blocking review on a fork PR
could not retract a success posted earlier by a comment event. Fork PRs
now always get a failure, so that success cannot exist. This is a
behaviour change: fork PRs can no longer clear this gate.

REVIEW: FAIL codex 9e4a2d7

Reviewed by Codex CLI 0.166.0 (gpt-6.1-sol) in its read-only sandbox, started from a Claude Code session on the author side. Same prompt as the review of d5020b5.

Found three merge-blocking defects.

  1. [P1] Runs for different PRs can overwrite a blocking verdict with stale success.
    pr-guards.yml:1471, concurrency at line 1174.
The status belongs to the SHA, but concurrency groups belong to individual PRs. For two PRs sharing a head, run A can read passing reviews, pause before publishing, then run B reads a newly posted BLOCKED marker and publishes failure. A subsequently publishes success, restoring mergeability despite the block. Nothing revalidates A’s snapshot before publishing. Reproduced using the actual embedded script with mocked APIs. Confidence: high. Blocks merging.

  2. [P1] Rejecting forks cannot retract an inherited passing status.
    pr-guards.yml:1330, fallback at line 1315.
After an internal PR earns success on SHA H, a fork can open a different PR at H without its own passing review. The evaluator returns failure, but the fork’s read-only token cannot write either the failure status or the fallback check. Only Engine Review Evaluator fails; the required Engine Review Pass remains successful. Binding the context to GitHub Actions does not distinguish these PRs. GitHub documents read-only tokens for fork PRs. Reproduced the denied-write path with mocked APIs. Confidence: high under the documented default fork permissions. Blocks merging.

  3. [P1] Evaluator timeout or cancellation leaves earlier success authoritative.
    pr-guards.yml:1170, timeout at line 1183.
With an existing success, submit a blocking review and let its evaluator time out during an API request, before publishing failure. The required context stays successful because the running/failed evaluator now has a different name. No pending status invalidates the old success, and the fallback only executes when JavaScript catches a failed status write. Thus a runner-level failure can leave the PR mergeable despite the blocking review. Confidence: high from control flow; runner timeout behavior was not exercised live. Blocks merging.

Non-blocking test gap: the diff adds no tests, and I found no targeted tests for this evaluator. Coverage should exercise shared-SHA interleavings, fork write denial, interrupted evaluation, and fallback recovery. This adds no separate blocker beyond the defects above.

I inspected the requested diff and surrounding workflow without modifying files. Validation used an in-memory Node harness executing the embedded script. I could not verify live branch protection, repository token settings, GitHub mergeability, or fallback-check behavior across real workflow runs.

Replaces the rename in the earlier commits on this branch. Two Codex
reviews found that once the job stops carrying the verdict, an old
success status stays authoritative until something overwrites it, and a
race, a read-only token or a timeout can each prevent that write.

Restore the job and script to main, so a job under the required name
still fails without any token and shows pending while it runs. Fix the
original problem directly: when a verdict passes, rerun the oldest failed
or cancelled evaluation at that SHA. It re-reads the reviews and, if it
passes, reruns the next. If rerunning fails, the old result stands and
the PR stays blocked, as it does today.

REVIEW: FAIL codex 03ae812

Reviewed by Codex CLI 0.166.0 (gpt-6.1-sol) in its read-only sandbox, started from a Claude Code session on the author side. Same prompt as the two earlier reviews.

I found one merge-blocking fail-open defect in the surrounding gate. The added retry logic also has recovery gaps. Only the workflow changed; I modified nothing.

  1. Merge blocker — a blocking verdict can leave an earlier success effective. High confidence.
pr-guards.yml:1265 catches commit-status publication errors and continues. For issue_comment events, line 1192 also deliberately avoids failing the job.
Reproduction: first obtain a passing status; then post a valid BLOCKED marker for that SHA while status publication returns 403 or another API error. The previous success remains, and this evaluation finishes successfully on the default branch. The required context cannot reflect the blocker. A mocked execution of the actual script confirmed failed: false with no status published. Pre-existing, but blocks treating this as a reliable merge gate.

  2. Non-blocking recovery defect — one unrecoverable candidate prevents repairing later runs. High confidence.
pr-guards.yml:1296 retries candidates oldest first, but any rejected rerun exits the entire search through the outer catch. An expired or otherwise unre-runnable oldest candidate therefore prevents trying newer eligible runs. Every successful evaluation repeats the same selection. This leaves reviewed PRs blocked rather than allowing unsafe merges. Introduced by this change.

  3. Non-blocking recovery gap — timed-out evaluations are never retried. High confidence.
pr-guards.yml:1276 recognizes only failure and cancelled, although this job has a five-minute timeout. A timed_out required check remains outside the repair mechanism. Introduced retry coverage gap.

  4. Conditional merge blocker — native blocking reviews are ignored unless they contain a marker. High confidence in behavior; policy-dependent severity.
pr-guards.yml:1357 accepts CHANGES_REQUESTED reviews but never interprets that state; only marker lines affect the verdict. A mocked execution with a valid non-author PASS plus a separate native blocking review published success. If “blocking review” includes GitHub’s native review state, this violates the requirement. If policy explicitly recognizes only REVIEW: BLOCKED/FAIL markers, this is non-blocking. Pre-existing.

The new actions: write permission matches GitHub’s rerun endpoint requirement. However, reruns retain the original run’s privileges and SHA/ref; the new permission does not repair original runs whose tokens lack it.

No regression tests accompany the change. Important missing cases include status-write failure after an earlier success, rejected oldest retries, timeouts, queued-run replacement, and multiple stale evaluations.

I could not verify live branch protection, actual token restrictions, run/check SHA attribution, or GitHub’s behavior under concurrent reruns. The script checks were mocked, not live GitHub integration tests.

A rejected rerun, such as an expired run, ended the whole search, so
every later pass retried the same run and newer ones stayed red. Catch
errors per run and move on to the next. Also treat timed_out as stale,
because this job has a timeout and only failure and cancelled were
retried. Found by Codex at 03ae812.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Both are on main today and were reported by Codex.

A native changes-requested review was ignored unless it held a REVIEW
marker. A member's latest decision now blocks until they approve or it is
dismissed, matching GitHub's rule. Decisions are read from the raw review
list because the marker filter drops reviews with an empty body.

A blocking comment could leave an earlier success in place if its run
died or could not post a status, because comment runs have no job under
the required name on the PR head. A member's marker comment now replaces
the status with pending before evaluating, so those cases leave the PR
blocked. Plain comments skip this, so bots do not make a passing PR
flicker. If a comment run cannot post a status, the run now fails.

Behaviour change: a native changes-requested review now blocks the gate.

pr-guards.yml:1268 — High confidence; blocks merging.

Start with a passing gate, then post a valid BLOCKED marker. If both the initial pending write and final failure write fail, the helper swallows both errors. core.setFailed() fails the issue_comment job on the default branch; it does not replace the successful status on the PR head. The PR can therefore remain mergeable despite the blocking marker.

An in-memory execution of the actual script with mocked API failures confirmed: attempted writes were pending and failure, the job failed, and the stored PR status remained success. A transient API outage is enough; this does not require a permanently misconfigured token.

  2. Editing away a PASS marker bypasses the pending invalidation.
pr-guards.yml:1324 — High confidence; blocks merging.

The preflight tests only the comment’s new body. Edit the sole passing comment to remove its marker, and this condition becomes false. The run fetches reviews and comments while the old successful status remains effective. If the runner is cancelled or times out before posting the final verdict, that success persists despite there being no qualifying review.

A mocked execution stopped immediately before the review fetch confirmed that this path had made no status writes. Invalidate on relevant edits even when the new body contains no marker.

These failures matter because comment-triggered workflows run against the default branch, rather than supplying a failing check on the PR head. See GitHub’s event documentation.

Non-blocking test gap: only the workflow changed, and I found no dedicated tests for this evaluator. Add coverage for failed pending/final writes, marker removal followed by cancellation, concurrent event bursts, native review transitions, and stale-run repair. Missing tests are not a separate merge blocker, but the two reproduced defects are.

Verification limits: I inspected the requested diff and surrounding workflow and ran mocked script checks without modifying files. I could not verify live branch protection, token permissions, fork behavior, GitHub’s selection among duplicate check runs, or the rerun API’s behavior under concurrency. The mocks do not establish live GitHub mergeability.

The pending step only read a comment's new text, so editing a marker
away skipped it, and a run that then died left the withdrawn success in
place. Check the previous text as well. Found by Codex at 565c4b5.

Restoring the workflow from main dropped this. Without it, one transient
5xx on a status write can lose a comment run's verdict.

Rerun the gate job directly rather than a run's failed jobs, so cancelled
and timed-out evaluations are covered without relying on how the
failed-jobs endpoint treats them, and unrelated jobs are left alone.

REVIEW: FAIL codex 9e30f0d

Reviewed by Codex CLI 0.166.0 (gpt-6.1-sol) in its read-only sandbox, started from a Claude Code session on the author side. Same prompt as the earlier reviews, plus one stated exception: the maintainer accepted the case where every commit status write in an issue_comment run fails after retries.

Two merge-blocking defects remain. Both paths predate this diff, but remain in scope under the requested gate review.

  1. PR lookup failure preserves an earlier success — blocker, high confidence.
pr-guards.yml:1207 fetches the PR before the new pending-status invalidation at line 1333. If that lookup fails after retries, the catch returns; failRun only warns for issue_comment. A newly posted blocking marker or withdrawal of the sole PASS therefore leaves the previous head success intact, potentially leaving the PR mergeable.

I executed the actual script with a mocked lookup failure: zero status writes, zero job failures, and a warning. This differs from the accepted limit: no status write is attempted, and status writes could be healthy.

  2. Editing a native review does not reevaluate the gate — blocker, high confidence.
pr-guards.yml:7 subscribes only to submitted and dismissed. After a non-author native review supplies the sole PASS, editing its body to remove that marker or replace it with BLOCKED leaves the previous success standing until another qualifying event occurs. GitHub emits pull_request_review: edited for this operation; the separate review-comment trigger covers comments on diffs. GitHub event documentation

Non-blocking test gap — high confidence: The branch changes only this workflow, with no regression tests added. I found no dedicated tests for these paths, partial status-write failures, queued-run replacement, or the stale-job rerun chain. Those cases warrant mocked tests and GitHub integration coverage.

I could not verify live branch-protection enforcement, effective token permissions, status/check interactions, or rerun/concurrency behavior against GitHub. The mock establishes local control flow, not live mergeability. No files were modified.

Editing a review to remove its PASS marker or add a blocking one left
the earlier success in place until another event arrived. Subscribe to
pull_request_review edited.

If a comment run cannot load the PR it has no head SHA to post a status
to, so an earlier success stands. Fail the run so that is visible. Both

REVIEW: FAIL codex da0bc8c

Reviewed by Codex CLI 0.166.0 (gpt-6.1-sol) in its read-only sandbox, started from a Claude Code session on the author side. Same prompt as the earlier reviews, plus two stated exceptions: the maintainer accepted the cases where every status write fails, or the PR cannot be loaded, in an issue_comment run.

  1. Merge blocker — cancellation before the first status write leaves an earlier success valid. Confidence: high.
At pr-guards.yml:1335, invalidation happens inside the running script. Consider a head with a successful status whose only PASS comment is deleted, or which receives a BLOCKED comment. If that issue_comment job is cancelled while queued, or times out during action initialization before this statement executes, no pending/failure status reaches the head. Its failed/cancelled check belongs to the default branch, so the earlier head success remains usable despite the withdrawn PASS or new blocker.

This is outside both accepted limits: PR loading and status writes need not fail; the script never reaches them. The in-job pending write cannot cover this lifecycle failure.

  2. Non-blocking finding — no regression coverage accompanies the gate changes. Confidence: high.
The branch changes only pr-guards.yml:1184; I found no tests targeting this evaluator. Missing cases include cancellation before invalidation, pending-write failure followed by a successful failure write, native review decisions with empty bodies, dismissal ordering, and overlapping runs/rerun chains. This coverage gap alone would not block merging, but the lifecycle case above needs verification beyond mocked script execution.

The added permissions match the intended API operations for a normal same-repository token. They do not guarantee those permissions for fork events. Also, retries: 3 does not retry every API failure: github-script excludes HTTP 400, 401, 403, 404, and 422 by default. github-script documentation.

I inspected the requested diff and surrounding workflow without modifying files. I could not verify live branch-protection behavior, effective token permissions, check selection across runs, or actual rerun/concurrency behavior against GitHub. No tests were run. I have not counted either accepted failure mode as a blocker.

REVIEW: FAIL codex da0bc8c

Reviewed by Codex CLI 0.166.0 (gpt-6.1-sol) in its read-only sandbox, started from a Claude Code session on the author side. Same prompt as the earlier reviews, plus three stated exceptions: the maintainer accepted the cases where every status write fails, the PR cannot be loaded, or the run dies before its script starts, in an issue_comment run.

  1. Merge-blocking: queued review withdrawals can be lost without invalidating the previous success. Confidence: high.
pr-guards.yml:1330, together with concurrency at line 1169.

The initial pending write depends on the triggering comment containing a marker. A reachable sequence is:

    • The head already has a successful gate.
    • While another evaluation is running, someone deletes the sole passing issue comment. Its evaluation queues.
    • An ordinary issue comment queues another evaluation, automatically cancelling the withdrawal evaluation.
    • The replacement starts, loads the PR, skips the initial pending write because its comment has no marker, and then reaches the five-minute timeout during review/comment retrieval.

The previous success survives even though no passing review remains. The failed replacement job belongs to the default branch. This requires neither failed status writes nor failed PR loading, and cancellation is automatic; the replacement fails after its script starts. It therefore falls outside all three accepted limits. GitHub documents that new pending jobs replace existing pending jobs even with cancel-in-progress: false.

Every issue-comment evaluation needs to invalidate the existing verdict before potentially blocking API work, regardless of its triggering comment’s contents.

  2. Non-blocking independently: the new failure-handling and repair paths lack regression tests. Confidence: high.
    pr-guards.yml:1286 and pr-guards.yml:1464.

The branch changes only this workflow, and I found no targeted tests for the evaluator. Coverage should exercise queued-event replacement, script-time timeout, partial status-write failure, native review dismissal/approval ordering, and stale-job rerun selection and chaining. The first finding is a concrete missing scenario.

I inspected the requested diff and surrounding workflow without modifying files. These findings come from static analysis and GitHub documentation; I did not reproduce them against live Actions. I could not verify repository token settings, fork-token behavior in this repository, actual status/check attribution, or successful rerun chaining. GitHub confirms that a required check and commit status sharing a name must both pass, making live verification of this dual-purpose context particularly useful.

A queued run with a marker can be replaced by a run for a plain comment.
That run skipped pending, so if it timed out during its fetches the
earlier success survived a withdrawn or blocking review. Post pending on
every comment run. This also removes the marker and author checks. The
cost is a brief pending status on a passing PR whenever anyone comments.

REVIEW: FAIL codex 3ec301f

Reviewed by Codex CLI 0.166.0 (gpt-6.1-sol) in its read-only sandbox, started from a Claude Code session on the author side. Same prompt as the earlier reviews, with the three accepted limits stated.

  1. Merge blocker — one PR can overwrite another PR’s failure. High confidence.
.github/workflows/pr-guards.yml:1261 writes a SHA-wide status, while concurrency is scoped to PR number at line 1170.

Reproduction: two PRs share head SHA S and previously passed. Remove the only PASS comment from PR B; its comment run posts failure, while its earlier successful check remains unchanged. An unrelated comment on still-passing PR A causes A to post success on S, restoring B’s required status despite B having no passing review. A blocking marker on B has the same problem.

I executed the actual script with mocked APIs and confirmed the failure→success overwrite. GitHub statuses are commit-wide and use the latest status per context. This weakness predates the diff; the new repair search at line 1286 also selects runs across PRs sharing that SHA.

  2. Merge blocker — PR-controlled workflow code can manufacture the required result. High confidence for same-repository PRs.
.github/workflows/pr-guards.yml:1173 grants write permissions to a workflow running on pull_request. A PR author can change this job to post Engine Review Pass: success on its head SHA and exit successfully, without evaluating any reviews. Both results come from GitHub Actions, satisfying the stated app binding.

The workflow therefore needs a trusted execution boundary independent of PR-authored workflow changes. GitHub documents the PR merge-ref execution context. This is also inherited, rather than introduced by this diff; the added actions: write broadens the authority available there.

  3. Non-blocking — the automatic repair chain assumes rerun jobs contain today’s repair code. Medium confidence on operational impact.
.github/workflows/pr-guards.yml:1306 reruns one old job and returns, expecting that job to repair the next. An older workflow revision without rerunStaleEvaluation() cannot continue the chain. Multiple failed evaluations can consequently require manual reruns. GitHub reruns retain the original SHA, ref, and actor privileges. This is an availability concern, not independently a fail-open finding.

No engine-gate behavioral tests were found, and this branch adds none. Missing coverage includes shared-SHA PRs, workflow trust, API-write failures, concurrency replacement, native review transitions, and repair across workflow revisions.

Verification was read-only, using source inspection and an in-memory mocked execution. I did not verify live branch protection, token restrictions, rerun/check aggregation, or actual scheduling on GitHub. The three accepted limits are excluded from these findings.

REVIEW: FAIL codex 3ec301f

Reviewed by Codex CLI 0.166.0 (gpt-6.1-sol) in its read-only sandbox, started from a Claude Code session on the author side. Scope, at the maintainer's request: regressions only. The prompt asked whether this diff introduces a defect or worsens existing behaviour compared with main. Pre-existing weaknesses were out of scope.

Yes—one merge-blocking availability regression.

  • [P1] Repair reruns can cancel the current head’s evaluation. pr-guards.yml:1306. Confidence: high. Blocks merging this change.

    A reachable sequence is:

    1. An evaluation for head A is running, with an older failed evaluation for A available to repair.
    2. The PR advances to B, and B’s evaluation waits in the same per-PR concurrency group.
    3. A passes and invokes the new rerun API. The repair job for A enters that group and replaces B’s pending evaluation.

GitHub cancels an existing pending job when another enters the same concurrency group, even with cancel-in-progress: false. The rerun retains its original event’s SHA and ref, and this script uses that event’s PR payload. It therefore evaluates A, leaving B without a completed gate evaluation. B can remain blocked until another event or manual rerun repairs it. The repair search also filters to A’s SHA, so it cannot recover the canceled B evaluation.

Difference from main: main finishes A without scheduling this extra job; the pending B evaluation can proceed. Pending-job replacement itself is pre-existing, but the automatic stale-head rerun that displaces B is new.

No additional concrete merge-blocking regressions found. The added actions: write matches the rerun endpoint’s documented permission requirement; API failures are caught.

I inspected the requested merge-base diff and surrounding workflow without modifying files. I could not verify live scheduling, repository token restrictions, or actual branch-protection enforcement; the finding follows from the workflow and documented GitHub behavior.

A repair rerun joins this job's concurrency group, where it replaces a
queued job. If the PR had advanced, that could be the evaluation of the
new head, leaving it unevaluated. Re-read the PR and skip the repair if
its head no longer matches. Found by Codex at 3ec301f.

REVIEW: FAIL codex 78cba83

Reviewed by Codex CLI 0.166.0 (gpt-6.1-sol) in its read-only sandbox, started from a Claude Code session on the author side. Scope, at the maintainer's request: regressions only, compared with main. Pre-existing weaknesses were out of scope.

Yes—two merge-blocking availability regressions.

  1. Automatic reruns can cancel the newer head’s evaluation. pr-guards.yml:1314
The head check at line 1288 is not atomic with scheduling the rerun. After it returns head A, a push can queue head B’s evaluation while this job still runs. The automatic rerun for A then enters the same PR concurrency group and replaces B’s pending job. GitHub documents that pending jobs are replaced even with cancel-in-progress: false. B can remain blocked until another event or manual rerun. Compared with main: this diff introduces the automatic stale-head job that displaces B. Confidence: high. Blocks merging. GitHub concurrency documentation

  2. The preliminary pending status can permanently exhaust the status context. pr-guards.yml:1334
GitHub permits 1,000 statuses per SHA/context. With 999 existing statuses, an ordinary comment now writes pending as status 1,000; its final success receives a validation error. Every subsequent attempt for that SHA/context also fails, leaving the required context permanently pending. Compared with main: main writes only the final verdict, so that same comment could write success as status 1,000 and permit merging. The general quota is pre-existing; consuming its last slot with an intermediate pending status is new. Confidence: high; requires a heavily updated head. Blocks merging. GitHub status API documentation

No additional non-blocking findings. The added actions: write permission matches the rerun endpoint’s documented requirement. GitHub workflow API documentation

I inspected the requested diff and surrounding workflow without modifying anything. I could not verify live token restrictions, reproduce GitHub scheduling, or exercise branch protection; I used your stated required-context configuration.

REVIEW: FAIL codex 78cba83

Reviewed by Codex CLI 0.166.0 (gpt-6.1-sol) in its read-only sandbox, started from a Claude Code session on the author side. Scope, at the maintainer's request: regressions only, compared with main, with two accepted availability risks stated.

Yes—two merge-blocking availability regressions.

  1. Repair can rerun another PR’s failed evaluation. pr-guards.yml:1301, with the rerun at line 1314. High confidence; blocks merging.

Candidates match the workflow name and head SHA, but never the PR number. Two PRs targeting different bases can share a head SHA while having different review markers. After PR A passes, this code can rerun PR B’s failed evaluation. B still lacks approval and writes failure to their shared Engine Review Pass status, blocking A again.

Difference from main: main does not automatically revive B’s evaluation after A passes. Shared commit statuses are pre-existing; this automatic cross-PR overwrite is new. GitHub documents that commit statuses affect PRs involving the commit, and reruns retain the original event’s SHA/ref. Statuses documentation, rerun documentation.

  2. An unrelated comment can leave an approved head indefinitely pending. pr-guards.yml:1333, together with the failed-write handling at line 1268. High confidence; blocks merging.

Every PR comment replaces an existing success with pending. If that write succeeds but the job subsequently times out, is cancelled, or exhausts retries on its final status write, pending persists until another evaluation succeeds. Failing the comment job does not resolve the required status on the PR head. This can disrupt an otherwise valid merge without any review changing.

Difference from main: for an unrelated comment followed by a failed final write or termination before that write, main preserves the previous success. This regression requires neither the accepted 1,000-status limit nor the accepted push/rerun race.

The added actions: write matches the rerun endpoint’s documented permission requirement; I found no separate blocker in that declaration. API documentation.

I did not execute live Actions runs or verify repository token policies, actual API responses, or branch-protection configuration beyond the supplied context. No files were modified.

Repair matched runs by SHA only, so a pass on one PR could rerun another
PR's failed evaluation at the same commit, and that run would then post
failure to the status they share. Found by Codex at 78cba83.

REVIEW: PASS codex c5eba42

Reviewed by Codex CLI 0.166.0 (gpt-6.1-sol) in its read-only sandbox, started from a Claude Code session on the author side.

This PASS has a narrow scope, set by the maintainer. The prompt asked only whether this diff introduces a defect or worsens existing behaviour compared with main. It is not a finding that the gate is secure: earlier reviews on this PR at wider scope returned FAIL, for weaknesses that exist on main, and those remain open. Three availability risks were stated as accepted and excluded: a push landing between the head check and the rerun, GitHub's limit of 1,000 statuses per SHA and context, and a comment run that dies leaving a PR pending. The live rerun behaviour was not exercised by this review.

No merge-blocking regression found. Confidence: moderate.

  • The added actions: write permission matches the rerun API’s requirement. Repair API failures are caught and logged; they preserve an existing blocked result rather than introduce a new pass.
  • Repair candidates are restricted to this workflow, PR, head SHA, and failed/cancelled/timed-out engine jobs. I found no additional concurrency defect beyond the accepted race.
  • Native review decisions now independently block passing markers. Later approvals or dismissals supersede earlier change requests; ordinary comments do not.
  • I excluded all three accepted availability risks. No other concrete regression warranted a blocking or nonblocking finding.

I could not verify live GitHub scheduling, repository token restrictions, or branch-protection enforcement. This was a read-only code and API-documentation review; no workflow runs were executed.
