docs: Pilot MCP page for the hosted server; remove self-hosted MCP docs - #279

Add /docs/mcp (and its plain twin) documenting the hosted MCP server at https://cloud.pilotprotocol.network/mcp: client setup, OAuth 2.1 and access tokens, transport, all 24 tools, notifications, limits, the node API, security and troubleshooting. Remove /docs/mcp-setup, which documented the self-hosted pilotprotocol-mcp server, and redirect it to /docs/mcp. Point the nav, docs indexes, agents.txt, integrations.json and llms.txt at the hosted endpoint.
