release: pilotprotocol-mcp 0.4.0 — remove the hosted control plane - #25

The hosted control plane has been retired. Remove everything that served it: - `pilot-mcp attach` and `setup --managed-url` (hosted enrollment and the pinned managed-runtime download). Both now exit 1 with a message. - The --enterprise-control / --governed-resource arguments added to pilotctl send-message and send-file on managed nodes. - Native pre/post tool hooks: setup no longer installs them, and removes the entries earlier releases wrote (Pilot's own commands only). - The management section of `pilot-mcp doctor`. `hook --harness <id>` and `picoclaw-hook` remain as silent allows, so a harness that still has the old entries keeps working until setup is run again. setupremoves the hook entries earlier releases wrote into harness settings (and the Hermes approvals, Cline shims and PicoClaw process hook). Only Pilot's own commands are matched; user hooks are kept.hook --harness <id>andpicoclaw-hookstay as silent allows (exit 0, no stdin, nopilotctl, no network), so a harness that still has the old entries keeps working untilsetupis run again. A leftover control attachment no longer makes them block.Kept
