Rename the GitHub repository to cardinalfda and update URLs

The package is now cardinalfda (see DESCRIPTION), but the GitHub repository is still pharmaverse/cardinal.

This requires admin access and cannot be done from a PR:

• rename the repository cardinal -> cardinalfda
• update the pkgdown site URL and the pharmaverse listing
• update github.com/github.io URLs in DESCRIPTION, README, _quarto.yml, badges, and asset references
• optionally rename cardinal.png logo assets and the .Rproj

Until the repo is renamed, in-package URLs are intentionally left pointing at /cardinal to avoid broken links.
