This pull request includes changes targeted against the July 2026 Connectathon, including the following improvements:

  • Fixed and improved Questionnaire URL handling
  • Implemented configuration approach for Questionnaire graphing including observation ranges and cut points
  • Implemented functionality for deleting sample data
  • Implemented patient deletion functionality
  • Updated and improved Transition of Care (TOC) workflow and bundle generation
    • Added TransitionOfCareCompositionService aligned with the latest IG
    • Updated TOC create/edit UI to match the latest IG
    • Improved TOC bundle creation to handle recursive references
    • Implemented a new TOC sharing flow with patient match and DocumentReference support
    • Excluded SMP Bundles from TOC bundles
    • Added Behavioral Health and Plan of Care sections to TOC flows
  • FHIR parsing/conversion robustness
    • Added handling for FHIR resources that include a field named reference
    • Added a workaround for converting QuestionnaireResponse resources with contained resources
  • UI improvements
    • Fixed sidebar width and added questionnaire response counts
    • Improved Sync Patient Record action clickability and icon spacing
