Integrate comment formatting from formatter into existing AST code

Description

Follow-up to #4492: Avoid the duplication of AST-to-prettier-document conversion functions in the new formatter package, and merge the changes needed for re-adding comments into the existing Doc functions in the ast package.

Also fix several formatting issues that existed in both ast and formatter.
