To install:
 
1. Copy the userreport.zip folder to the blocks folder and unzip.
2. Login as Admin and Visit the Notifications page (Site Administration -> Notifications)
 
3. Go to site front-page (home)
4. Turn editing on
5. Now you can add block "userreport"
