Run worker-bound immediate PR actions after replan

Summary

• preserve completed-worker publish/update actions even when replanning marks them immediate
• normalize those worker-bound PR actions to after_success so existing action execution can publish them
• add regression coverage for immediate worker-bound PR actions while still dropping ordinary immediate watch actions
