Added the ability to use the plugin as a rollup output plugin in generateBundle hook.
Hook option forwarding is in place and could further allow #11 resolution if needed.
At output time, we lose the transform time id, as such facadeModuleId and fileName will be probed and the first to exist used in order to allow filtering (somewhat different from the normal transform time behaviour, as the expectation at output time would be to filter against output chunks rather then input files - depending on codebase format the 2 could be equal but in most cases won't)
