Closes #12364 — RFC #12062, proposal B (API + dashboard), panel 1 — Outcome by author kind.

What

• New evPanel card Outcome by author kind in the Throughput section's Evidence sub-panel, rendered first (RFC panel order) beside panels 2 / 3 / 8. Same prt-card / ev-panel chrome, inline SVG, no new deps, no inline style=.
• One grouped bar per author kind (human · agent · bot, plus any other kind the API emits) for merged, closed without merge and reverted within 14 d of merge, scaled against the largest kind; n= per row and the panel total. Tooltip carries merged rate of decided PRs and reverted rate of merged PRs (open PRs excluded).
• "What this does not show" note: reverted = reverted_by link within 14 d of merge only; schema v1 bundles do not record close-without-merge so closed may read 0; open PRs excluded from rates; merged ≠ correct.
• Honours the existing window / repo filter wiring (fetchEvidenceSummary is unchanged; the panel reads dto.outcomes_by_author_kind).
• Empty state when no kind has n > 0.
• Docs: src/docs/review-evidence.md gains a panel 1 bullet. Changelog fragment added.
• Tests: evidence_card_ui_test.go pins the new helper, panel key, bar classes, note text, CSS and panel order; evOutcomesSvg added to the ReferenceError guard.

Guardrail (RFC #12062 Non-goals)

Record/read-path only. Nothing in this PR reads into or alters merge eligibility or auto-merge; the API is unchanged and tests touch only index.html markup.
