feat(trust): add evidence-based capability probation

Summary

• add isolated @heybeaux/lattice-trust policy package for RT-06
• use cumulative evidence, deterministic exponential probes, and a permanent evidence cap
• preserve/transfer serialized probation state without changing existing circuit-breaker behavior

RT-06 semantics

• bench when failures > successes
• probe at +2/+6/+14 by default
• readmit only when cumulative failures <= successes
• conclude permanently when failures - successes > 3
