Enforce noAnswerPassed in the post-dream gate. The suite checks isolationPassed, but only checks required coverage. A failed explicit no-answer query can therefore pass. Assert checkThresholds(allScores) or add an equivalent noAnswerPassed assertion.

Update the post-dream gate around the overall coverage assertion to also enforce
the no-answer requirement by asserting checkThresholds(allScores) or an
equivalent noAnswerPassed condition, while preserving the existing
isolationPassed and coverage checks.

The audit found ownership gaps in ensemble access, unsafe embedding publication, destructive consolidation/dedup behavior, and recall ranking and benchmark accounting defects. This change enforces owner boundaries and vector validity, preserves source memories until replacements are safely published, retains non-identical observations, and corrects contextual/temporal recall and actual-use ranking. Benchmark reporting now distinguishes required-item coverage from precision and preserves the existing 95% gate.
Validation completed locally on the integrated changes:
The benchmark uses 57 top-five-labelled queries and real local embeddings with aligned model IDs. It measures fixture-specific PgVector recall, with keyword search overridden empty and reranking disabled; it does not establish production precision or ensemble RRF quality. The application benchmark still uses forceExit. Fully judged/no-answer evaluation, query-order sensitivity, post-dream semantic preservation, and existing-data reconciliation remain follow-up work. No production data repair or production promotion is included.
PR CI additionally verified baseline 99.12% and post-dream 97.37% required-item coverage@5 (84 assertions each). The post-dream harness uses deterministic LLM output and successor-ID matching, so that result does not establish summary semantic fidelity. A CI-only follow-up aligns the workflow's report extraction and pass/fail markers with the new formatter; its shell gate accepted actual passing CI output and rejected failed, mixed, and missing-marker output. Quality gates are unchanged.
