Record the rules agreed in git-pkgs#514: one approval from a maintainer other than the author plus green CI, a "request changes" review blocks the merge, and bigger changes start with an issue.

Adds a "Reviewing and Merging" section to CONTRIBUTING.md, next to the Releases section from #544. It records the rules from @andrew's proposal in #514 (comment), which @simonchrz +1'd.
Two places where I had to choose wording:
