Nine tickets from lash.index.md plus two filed along the way, with the thread that connects most of them: `lash add` and `lash format` both regenerate Markdown from a parsed model that does not carry everything the source did.

Devlog entry for today's sweep: nine tickets from
lash.index.mdplus two filedalong the way.
The thread that connects most of them is worth recording, which is what the
entry is for:
lash addandlash formatboth regenerate Markdown from aparsed model that does not carry everything the source did, and neither noticed
when the difference cost the user content. Three separate bugs came from trying
to recover a source line count from parsed metadata that cannot express it; two
more from a sentinel value that meant two different things; two from the same
piece of information being recorded in two places with no note of which was
authoritative.
