Adds a rebuildable explicit FHIR reference projection in canonical Postgres, projection heads, atomic projected create/update, bounded internal traversal, path citations, a storage-neutral compiler contract, and ADR 0005. Pure graph tests passed 12/12; type checks and diff checks passed. A final maxEdges review found and fixed a global storage-read budget regression. Local Docker was unavailable, so this remains draft until CI proves migrations 0013/0014 and DB atomicity suites. QT-4 evidence is explicitly scoped: vocabulary is strong; traversal is promising but not yet promoted.
