Collapse manifest-layer and asset-layer violation statuses into two umbrella statuses — MANIFEST_UNTRUSTED and FILE_UNTRUSTED — each carrying the specific cause on a `reason` sub-field. Fetch-failure reasons renamed in the same pass so the mechanism is self-describing.
