#404b Disable FTS5 triggers during bulk indexing #411

Problem

Three FTS5 triggers fire on every INSERT and DELETE to symbols table. During bulk indexing, this causes 3x write amplification (each symbol INSERT triggers an FTS5 index update).

Solution

1. DROP TRIGGER for the 3 FTS5 triggers at the start of bulk index
2. After indexing completes, rebuild FTS5 index with: INSERT INTO symbols_fts(rowid, name, signature) SELECT id, name, signature FROM symbols WHERE project_id=?
3. Recreate triggers after rebuild

Expected Impact

2-3x faster DB writes during indexing. Combined with #404 and #405, targets the write bottleneck directly.
