Closes #143. Phase 3 of the 2026-09-30 docs health plan. Append-only notes per ADR-033; no decision changes.

  • Both sides of every amendment now say so. ADR-007/012 (amended by ADR-017), ADR-013 (§2 by ADR-026), ADR-014 (by ADR-025/027/028/034), ADR-015 (by ADR-025), ADR-016 (session bullet by ADR-025 §3), ADR-019 (the fly.toml exception from ADR-025 §6), ADR-025 (refined by ADR-027, and an "Amends" line), ADR-000 (asset lists moved by ADR-034), ADR-017 and ADR-034 state what they amend.
  • Stale samples labelled illustrative where the code diverged: zerolog and MaxResponseTime (ADR-013), gorilla/csrf, bluemonday, the unimplemented per-user/per-email rate tiers, "Cloudflare environment variables" (ADR-014), JWT_SECRET/ENABLE_CACHE/METRICS_PORT and the nine missing env vars (ADR-015), Cloudflare secrets and runtime rotation (ADR-001 §4).
  • Corrections: ADR-005 no longer says regeneration drift is unchecked; ADR-016's Status states what shipped and what did not (docs(adr): ADR-016 caching is Accepted but unimplemented — implement ETag/edge headers or supersede it #135); ADR-000 records that its code-quality metrics belong to no tier (perf: measure Total Page Weight and Core Web Vitals (Lighthouse CI) so they can leave the Aspirational tier #136 for Lighthouse); ADR-025 and ADR-033 correct their own TC wording and ADR counts.
  • Housekeeping: the 2025 implementation checklists in ADR-013–016 are marked historical; their overdue review dates are closed with a 2027-03-31 next review.

Decision taken as recommended in #143: the five legacy metadata layouts stay as they are; the canonical **Date**: then ## Status form applies to ADR-035 onward via docs/adr/TEMPLATE.md (landed in #148).

Edits were applied by script as the operator-reviewed amendment path (the PreToolUse guard covers agent Edit/Write calls). task check:adr: 11 pass, 0 warning, 0 blocker. Link check: 0 broken.

…t fixes (#143)

Append-only notes (ADR-033); no decision changes. Status lines now say
who amended whom from both ends: ADR-007/012 (amended by ADR-017),
ADR-013 (§2 by ADR-026), ADR-014 (by ADR-025/027/028/034), ADR-015 (by
ADR-025 and the two variable-surface notes), ADR-016 (incomplete, #135),
ADR-017 (also amends 007/012), ADR-034 (amends 000 §3 and 014 §4).
Section notes: ADR-001 §1 Go 1.26 and §4 host-side secrets; ADR-014 §1
no-sliding-session / §2 no sanitiser / §3 hand-rolled CSRF / §4 shipped
rate tiers + Retry-After / §5 host-side secrets; ADR-015 complete env var
list; ADR-016 what shipped vs not; ADR-019 the fly.toml exception and the
graduation-log exception; ADR-025 §2 ADR-027 refinement, an "Amends" line,
and the TC-1 overstatement; ADR-000 the ADR-034 asset-list move and the
untiered code-quality metrics; ADR-005 check:generated exists; ADR-033
the ADR counts and the unwired TC-1 label. Implementation checklists in
ADR-013–016 marked historical; overdue review dates closed with a
2027-03-31 next review.

Edits were applied by script as the operator-reviewed amendment path
(the PreToolUse guard covers agent Edit/Write calls); the record is
append-only throughout — only Status lines gained qualifiers.
