Adds P-040 to tested_rules (pinned by the new bundler-collision regression branch) and bumps total_tested 95->96, per the "adding a new regression branch" checklist missed during that work. Also strips two private tracking-ID references (DEC-027/029/030) left in P-021's comment and audit_id field -- unrelated to that checklist gap, found while in the file.
