In postgresql, double-quotes always indicate an identifier (strings use single-quotes).

Add support for double-quoted identifiers, so that things like select "a" from "b" work.

Double-quoted identifiers are case-sensitive.


Needed this so that qusql could parse a pile of SQL migrations used to define a schema.
