Report an MCP OAuth callback failure before a retry - #2238
This change makes waitForAuthCode() report a callback listener failure that occurs before a caller waits. It follows the review discussion in #2129.
Before this change, the rejection handler cleared the listener. The next waitForAuthCode() call then started a new listener and waited for up to five minutes. The original error was not reported.
Keep a failure that occurs while no caller waits.
Report that failure once on the next waitForAuthCode() call. That call does not start a new listener.
Start a new listener on the call after that.
Give the original error to a caller that already waits, as before.
Close each failed listener. Ignore cleanup errors, so that they do not replace the callback error.
Start a new listener only after the old listener closes. The new listener can use the same port.
Discard a kept failure when a new listener starts or when close() runs. An old failure cannot affect a new attempt.
Detach the listener in close() before the cancel. The provider then does not keep the cancellation as a failure.
Keep the .catch handler that prevents unhandled promise rejections.
Reviewers should check the state changes in waitForAuthCode(), in the rejection handler in _doStartCallbackServer(), and in close().
The test setup now resets the callback server mocks before each test. One existing test used a mock from an earlier test. That test now sets up its own mock.
Test Procedure
Run cd src && npx vitest run services/mcp.
Run pnpm --dir src run check-types.
Run ESLint and Prettier on the changed files.
Run pnpm mcp:integration-check.
New tests cover these cases:
A listener fails before the first caller waits, and while a caller waits.
A listener receives a code before the first caller waits, and while a caller waits.
Two callers wait for a listener that fails immediately.
A replacement listener fails before a caller waits.
redirectToAuthorization() or close() discards a kept failure.
A new listener starts only after the failed listener closes.
close() waits until a failed listener closes.
After close(), a new attempt gets a new listener. This is true when cleanup succeeds and when it fails.
McpHub shows the callback error and disconnects the server when waitForAuthCode() rejects.
Results:
215 MCP tests passed.
6 new provider tests fail on main. They pass with this change.
I removed each new guard one at a time (9 checks). At least one test failed each time.
5 runs in random test order passed.
TypeScript, ESLint, and Prettier passed. ESLint suppression counts did not change.
The 16-case MCP OAuth integration check passed.

The source branch contains one commit. It is based on the current upstream main branch.
In Node, server.close() does not reject when the listener is already closed. The cleanup error handling is a safeguard. A test covers it with a mocked rejection.

Bug Fixes
OAuth sign-in now reports callback-listener failures consistently, including when a failure occurs before the app is waiting for a callback.
After a failed callback attempt, a new attempt can begin once the previous listener has finished shutting down.
Closing or cancelling an OAuth flow waits for cleanup, preventing leftover errors from disrupting later attempts.
Requests to a callback listener after it has completed now receive a clear “Not found” response.
The provider retains callback listener failures that occur before a wait, reports each retained failure once, and starts a later listener after cleanup. Closing the provider clears unreported failures and waits for listener cleanup. The callback server returns 404 for requests received after it resolves.

Explanation
The title "Report an MCP OAuth callback failure before a retry" matches the main change. The PR keeps callback-listener failures that happen before any waitForAuthCode() caller waits, reports each one once on the next call, and then allows a fresh listener. The title is specific, short, and in active voice, and a teammate scanning history would understand the primary change.

The description uses short, direct sentences in line with the Simplified Technical English request.
Full details: Linked Issues check
Explanation
[#2187] The reviewed head meets the coding requirements in McpOAuthClientProvider.ts. (1) Failure before a wait: the rejection handler stores the failed promise in _unreportedAuthCodeFailure when no caller awaited it (_awaitedAuthCodePromise differs). The next waitForAuthCode() returns that failure once and skips _ensureCallbackServer(), so no listener starts. (2) Later attempt: the consumed failure is cleared, and _server is null, so the following call starts a new listener through _ensureCallbackServer(). (3) Waiting caller: the awaited promise is not recorded as unreported, so the waiter receives the original error. (4) Cleanup: the failed listener is detached and stopped through stopCallbackServerIgnoringErrors(), so cleanup errors do not replace the callback error. _doStartCallbackServer() waits on _listenerCleanup before it starts a replacement. (5) Unhandled rejections: the .catch handler remains attached. close() clears the unreported failure, detaches the listener before cancel, and waits for cleanup. The PR summary lists provider tests for both timing orders, retry, port reuse, cancellation, and cleanup, and an McpHub test for error reporting and disconnect. This assessment relies on that summary and the prior review. The test files were not read directly in this review.
Full details: Out of Scope Changes check
Explanation
The changes stay within #2187. Production changes in McpOAuthClientProvider.ts implement the callback-listener failure lifecycle. The callbackServer.ts change returns HTTP 404 for requests that arrive after the callback resolves. This lets open connections end, so stopCallbackServer() can finish when a listener closes. That supports the requirement to always close a failed listener. Its loopback tests in callbackServer.listener.spec.ts cover this path. The McpHub test checks error propagation and disconnect behavior. The mock reset and the test-local mock setup support the provider tests. No unrelated behavior appears in the reviewed changes.
Full details: Regression Evidence
Explanation
Focused coverage exists for the changed behavior. McpOAuthClientProvider.spec.ts covers failures before and during waits, one-time reporting, retry, cleanup ordering, cleanup errors, stale-failure discard, close behavior, cancellation, success callbacks, and unhandled-rejection protection. McpHub.spec.ts covers error recording and disconnect behavior. callbackServer.listener.spec.ts uses a real loopback server to verify 404 responses for late requests and normal callback handling. No durable application UI change requires a Playwright snapshot.
Full details: Security Boundaries
Explanation
No changed path meets the security failure conditions. The provider changes only retain callback failures, detach and clean up failed listeners, and coordinate retries. The callback-server change returns a fixed 404 response for late requests. Existing callback input still requires the expected OAuth state before accepting a code, and no changed code exposes secrets or bypasses approval or allowlist controls.
Full details: Persistence Integrity
Explanation
No changed persistence path exists. The PR changes in-memory callback state and listener cleanup, plus late HTTP responses. saveTokens() and SecretStorage reads/writes are unchanged, and the new callback failure state is not persisted. Therefore the explicit persistence-integrity failure conditions are not introduced.
Full details: Lifecycle Resource Cleanup
Explanation
No concrete changed lifecycle leak or duplicate-work path is present. A failed callback promise detaches its listener and calls _stopDetachedListener() (McpOAuthClientProvider.ts:281-302). Replacement startup waits for _listenerCleanup before binding again (lines 253-266). close() detaches the active listener before cancellation and awaits cleanup (lines 690-712). The callback server clears its timer on cancellation or callback and now answers late requests with 404 so connections can close (callbackServer.ts:70-91).

- Keep a failure that occurs while no caller waits.
- Report that failure once on the next waitForAuthCode() call.
- Start a new listener on the call after that.

Closes: #2187
Description
This change makes
waitForAuthCode()report a callback listener failure that occurs before a caller waits. It follows the review discussion in #2129.Before this change, the rejection handler cleared the listener. The next
waitForAuthCode()call then started a new listener and waited for up to five minutes. The original error was not reported.waitForAuthCode()call. That call does not start a new listener.close()runs. An old failure cannot affect a new attempt.close()before the cancel. The provider then does not keep the cancellation as a failure..catchhandler that prevents unhandled promise rejections.Reviewers should check the state changes in
waitForAuthCode(), in the rejection handler in_doStartCallbackServer(), and inclose().The test setup now resets the callback server mocks before each test. One existing test used a mock from an earlier test. That test now sets up its own mock.
Test Procedure
