Closes: #2060

Step 2 of the two-PR plan in #2050 (cleanup with per-line evidence; enforcement shipped in #2051).

Stacked on #2051 — this branch is based on chore/forbid-stryker-directives, so the diff below also shows #2051's commits until that PR merges. Merge order: #2051 first, then I will rebase this branch onto main (the rebase is mechanical: both PRs touch scripts/stryker-diff.mjs in disjoint regions).

Description

Removes all 38 Stryker suppression directives (42 comment lines, 9 files) from main — with every previously-suppressed statement re-exposed to Stryker first, not deleted blind.

Repro protocol: the 38 guarded statements (whole statement spans, 45 changed lines) were fed to the real gate pipeline (buildManifest + runManifest, extension package, 3 batches) with the directives removed. Result over 105 mutants:

Outcome Count Meaning
Killed 70 existing tests already kill these — the directives were unnecessary
Survived 33 genuinely equivalent (see per-file table)
Timeout 2 inconclusive, non-blocking
File Result Disposition
lm-studio.ts 10/10 killed directive removed, no action
qwen-code.ts 10/10 killed directive removed, no action
webviewMessageHandler.ts 2/2 killed directive removed, no action
pathUtils.ts 9 survivors accepted equivalents: defensive iteration bounds, ENOENT fail-closed guards, empty-folders equivalence
abort-signal.ts 2 survivors accepted equivalent: { once: true } unobservable (abort fires exactly once, handler removes listener)
NativeToolCallParser.ts 2 survivors accepted equivalent: WeakMap delete observable only as GC eligibility
openai.ts 2 survivors accepted equivalent: unset option makes && yield undefined → parameter default → same strict value as ??
openai-codex.ts 2 survivors accepted equivalents: false mutant differs only inside an abort microtask window between race settle and catch
Task.ts 16 survivors accepted equivalents: guards that are invariant on the raw-chunk pipeline (locked start names, makeStreamingKey never undefined, both-miss unreachable)

Mechanism: the survivors are accepted centrally in scripts/stryker-equivalences.json — 33 entries, one per reviewed mutant, each keyed by file+line+mutator+replacement (the replacement is part of the key because one condition yields both => true and => false mutants and only the reviewed replacement may be accepted) and carrying the equivalence argument. scripts/stryker-diff.mjs matches them against the Stryker report, drops them from the blocking set, and reports them in a new Equivalent summary column. Entries whose file is under test but whose mutant is gone (code shifted, or a test now kills it) surface as a stale-entry advisory, so the registry cannot rot silently. This is the "narrow gate-config exclusion" the policy in #2051 points to — no in-source annotations.

Test Procedure

  • node --test scripts/stryker-diff.test.mjs → 53/53 pass (8 new tests: blocking-set filtering, replacement-scoped acceptance, stale-entry advisory scoping, default behavior without the registry, registry schema, Equivalent column + table shape, and a runManifest integration test that injects readEquivalences end to end).
  • eslint --max-warnings=0 on all 9 cleaned files → clean (comment-only diff).
  • Verification run on this branch (same repro protocol, registry active): all three batches report blocking = 0 with Equivalent = 13 / 4 / 16 and no stale-entry advisories — the registry exactly covers the survivors and nothing else changed.

Pre-Submission Checklist

Documentation Updates

  • No user-facing documentation updates are required.

Additional Notes

The 2 Timeout mutants in batch A are non-blocking inconclusive results (settle-guard timing), consistent with the gate's timeout advisory rules.

The changed-code mutation gate already runs on every PR, but its directive
policy allowed suppressions and reported malformed ones only as advisories,
so `// Stryker disable next-line ...` annotations kept accumulating in
production source (38 directives across 9 files on main).

- Replace the directive hygiene check with assertNoDisableDirectives: any
  `// Stryker disable`/`// Stryker enable` comment in a touched
mutation-tested file now fails the mutation-diff gate outright.
- Update the CI summary guidance, the workflow step name, and the
  AGENTS.md / CONTRIBUTING.md policies: kill mutants with focused tests;
genuinely equivalent mutants are argued in the PR body and become narrow
  gate-config exclusions at maintainer discretion.

The 38 directives already on main are NOT removed here: their removal moves
to a dedicated cleanup PR carrying per-line mutation-repro evidence (kill
tests where killable, narrow gate-config exclusions where equivalent).

Tests: node --test scripts/stryker-diff.test.mjs (44/44).
CodeRabbit: the line-based regex also matched string literals such as
"// Stryker disable all" used in documentation strings, failing CI for files
with no actual directive. Scan comment trivia with the TypeScript scanner
instead, flag trailing comments too, and document the legacy-directive touch
requirement in CONTRIBUTING.md.

Tests: node --test scripts/stryker-diff.test.mjs (45/45).
CodeRabbit: a raw ts.createScanner loop never resumes the template lexical
goal after an interpolation, so text inside a template literal could be
misclassified as a comment (false failure) and a directive following a
completed template could be swallowed (missed failure). Collect leading and
trailing comment ranges per token from the parsed source file instead, and
cover both template cases plus closed nested templates with tests.

CONTRIBUTING.md: state that genuinely equivalent mutants are explained in
the pull request body, matching the advisory-results policy.

Tests: node --test scripts/stryker-diff.test.mjs (45/45).

## Stryker Suppression Comments

Do not add Stryker suppression comments (`// Stryker disable ...`, `// Stryker enable ...`) to source files. The changed-code mutation gate (`scripts/stryker-diff.mjs`, run by the `mutation-diff` CI job) fails any pull request that introduces them in mutation-tested files.

The file-scope claim is accurate. buildManifest passes a set containing every source line to assertNoDisableDirectives, so an unchanged directive in any touched mutation-tested file is rejected. No documentation change is needed.

📝 Summary

Summary by CodeRabbit

  • Developer Experience
    • Mutation-testing checks reject Stryker suppression comments in mutation-tested source files. Contributors should address surviving or uncovered mutants with focused tests; genuinely equivalent mutants can be explained for maintainer review.
    • Reviewed equivalence entries match reported mutants by file, line, mutator, and replacement. Matching mutants are counted separately, while unused entries for files in a report trigger advisories.
    • Contributor guidance and mutation-testing workflow labels reflect the policy. Application behavior is unchanged.

The mutation gate now rejects Stryker suppression comments in included source files. A registry supports exact matches for equivalent mutants, which are reported separately from blocking mutants. Existing suppression comments were removed, and contributor guidance and tests were updated.

Full details: Linked Issues check

Explanation

Issue #2060 has coding requirements. The reviewed diff removes the suppression directives from the nine named source files. scripts/stryker-equivalences.json records reviewed entries with file, line, mutator, replacement, and rationale. scripts/stryker-diff.mjs applies exact replacement-scoped matching, reports an Equivalent column, and emits stale-entry advisories. The added tests cover directive rejection, replacement matching, stale entries, registry loading, summary output, and runManifest integration. The reported three-batch run has blocking = 0 and no stale-entry advisories.

Full details: Out of Scope Changes check

Explanation

The cleanup and gate changes support the mutation-gate objective in #2060. The whole-PR summary also shows the stacked #2051 policy and workflow changes. Those changes concern the same Stryker directive policy and do not add unrelated runtime behavior. The reviewed source changes remain limited to the nine named files, the registry, the gate script, and supporting tests.

Full details: Security Boundaries

Explanation

No changed path meets the security failure conditions. The nine runtime source-file diffs remove comments only; they add no executable behavior. The mutation-gate changes match equivalence entries by exact file, line, mutator, and replacement and do not execute registry values. The workflow change only renames a step. No changed path leaks secrets or PII, trusts executable unvalidated input, or bypasses an approval or allowlist control.

Full details: Persistence Integrity

Explanation

No changed persistence path exists. The application diff only removes Stryker comments; it adds no state writes, storage updates, or default changes. The production script adds read-only loading of scripts/stryker-equivalences.json with fs.readFileSync; it does not persist or modify that registry. The diff adds no new write, rename, rollback, or partial-failure operation.

Full details: Regression Evidence

Explanation

The default registry-loading path still lacks focused coverage. loadEquivalences(repoRoot) now reads and parses repoRoot/scripts/stryker-equivalences.json, and runManifest uses it by default. The added runManifest test injects readEquivalences, while the only direct loader test covers only an absent file. The registry-shape test reads JSON directly and does not test loadEquivalences, so a wrong path or parse/wiring regression can pass.

Resolution

Add a focused test that creates a temporary repository with scripts/stryker-equivalences.json containing one equivalence, calls loadEquivalences(tempRoot), and asserts that the entry is returned. Keep the injected runManifest test for pipeline filtering coverage.

Full details: Lifecycle Resource Cleanup

Explanation

The changed test still leaks a temporary directory. scripts/stryker-diff.test.mjs:1207-1209 passes fs.mkdtempSync(...) directly to loadEquivalences and never removes the directory. Each test run leaves an empty /tmp/stryker-eq-* directory.

Resolution

Store the temporary directory in a variable. Wrap the assertion in try/finally. Call fs.rmSync(directory, { recursive: true, force: true }) in the finally block.

## Stryker Suppression Comments

- If a mutant is genuinely equivalent, explain the equivalence in the PR description; maintainers decide on narrow gate-config exclusions. Do not add a suppression locally to get green.

CodeRabbit: getLeadingCommentRanges is text-based, so rendered JSX text
such as "<div> // Stryker disable all" produced a comment range and failed

JSX expression containers ({// ...}) still resolve to a real comment and

Tests: node --test scripts/stryker-diff.test.mjs (46/46).
…ked exclusions

The 38 suppression directives on main are removed. Every previously-suppressed
line was re-exposed to Stryker first (mutation repro at the Zoo-Code-Org#2051 branch head:
100 mutants over the 38 lines). 65 are killed by existing tests - the
directives were unnecessary - and the 33 survivors are genuinely equivalent;
they are accepted centrally in scripts/stryker-equivalences.json (26
file+line+mutator entries) instead of as in-source comments.

- Remove 42 directive comment lines from 9 files (comment-only diff).
- Add scripts/stryker-equivalences.json with the equivalence argument per entry.
- stryker-diff.mjs: applyEquivalences() drops accepted equivalents from the
blocking set, the summary gains an Equivalent column, and registry entries
whose file is under test but whose mutant is gone surface as a stale-entry
advisory so the registry cannot rot silently.

Evidence per file (mutants over the previously-suppressed lines): lm-studio
10/10 killed; qwen-code 10/10 killed; webviewMessageHandler 2/2 killed;
pathUtils 9 survivors (defensive bounds, fail-closed guards); abort-signal 2
(once flag unobservable); NativeToolCallParser 2 (GC-only); openai 2 (&& vs
?? via parameter default); openai-codex 2 (abort microtask windows); Task.ts
16 survivors (unreachable guards on the raw-chunk pipeline).

Tests: node --test scripts/stryker-diff.test.mjs (51/51).
CodeRabbit: file+line+mutator cannot distinguish the `=> true` and `=> false`
ConditionalExpression mutants of one condition, so a single reviewed entry
excluded both. The registry key now includes the mutant replacement, every
entry carries it, and the stale-entry advisory prints it. The summary table
delimiter row gained the missing Equivalent cell so GitHub renders the table.

The registry was regenerated against the current base (main moved the
guarded statements): 33 entries, one per reviewed surviving mutant.

Tests: node --test scripts/stryker-diff.test.mjs (53/53), including a
runManifest integration test that injects readEquivalences end to end.

🎯 Functional Correctness | 🟠 Major | ⚡ Quick win

Do not let one registry entry accept every mutant with the same key.

index is a Map keyed by file:line:mutator:replacement. Every blocking mutant whose fields produce that key is excluded, and the count is not limited. On a compound condition, Stryker emits several ConditionalExpression true mutants on one line. Each one targets a different sub-expression and has a different location.start.column. The registry shows this case: src/core/task/Task.ts line 4628 ConditionalExpression/true appears three times, and src/utils/pathUtils.ts line 114 ConditionalExpression/false appears twice. The Map collapses those duplicates into one key. As a result, the gate also accepts a new, unreviewed mutant on that line if it shares the mutator and replacement. That breaks the registry's own rule: "Each entry accepts exactly one mutant". The stale-entry advisory also cannot see duplicates that went unused.

To fix this, add column to the key. If you keep the current key, count the matches instead: each entry may accept at most one mutant. Then add a test where the report has more matching mutants than the registry has entries.

The unused-entry filter at Lines 510-515 needs the same change: report an entry as stale when the registry lists its key more times than it was used.

Review comment at @scripts/stryker-diff.mjs around lines 495 - 508:
Update the equivalence matching in the blocking-mutant loop so each registry
entry accepts at most one mutant: track available entries per key and decrement
the count when a match is used. Update the stale-entry check to flag registry
entries whose key appears more times than it was matched, and add a test where
matching mutants outnumber registry entries.
