Description

Part of Epic #359 (parallel tool execution). This PR adds per-call state for each tool call. It does not change behavior when tools run one at a time.

Before this PR, all tool calls in a turn shared one terminalProcess slot and one rejection flag on Task. Two tool calls that ran at the same time would overwrite each other.

Changes:

Add ToolExecutionContext (toolCallId, didReject) and Task.activeToolContexts, keyed by toolCallId.
Replace Task.terminalProcess with Task.terminalProcesses, keyed by toolCallId.
Make toolCallId required in ToolCallbacks. presentAssistantMessage now passes it to every tool.
Make ExecuteCommandTool store, abort, and remove its own process by toolCallId.
Add markToolRejected in src/core/task/toolExecutionContext.ts. It sets didRejectTool and the context didReject. ExecuteCommandTool, ReadFileTool, and presentAssistantMessage use it.
Apply the continue and abort buttons to all running terminal processes.

Design choices for reviewers:

A context is created when a tool call first appears. It stays in the map until the turn reset clears it. This keeps didReject readable after the tool pushes its result.
The context does not depend on the parallel flag. The serial path still reads didRejectTool and didAlreadyUseTool, so its behavior does not change.
terminalProcesses is a map on Task. It is not a field on the context.
didAlreadyUse is not on the context. Nothing would set it. Add it when [Story 4.2a] DispatchState machine refactor #374 or [Story 4.2b] Core parallel dispatch in presentAssistantMessage #375 needs it.
The mcp_tool_use branch has no context. It passes toolCallId ?? "" to useMcpToolTool.
The issue says existing tool tests pass unchanged. About 23 specs changed, because toolCallId is now required. Most specs only add toolCallId to a callback object. newTaskTool.spec.ts and attemptCompletionTool.spec.ts use toolCallId: "" to keep their old no-id path.

Known gaps:

The continue and abort buttons send no call key. With parallel commands, one abort click stops all commands.
No test sets a rejection through ExecuteCommandTool. The spec cannot reach that path. Add one in [Story 4.2b] Core parallel dispatch in presentAssistantMessage #375, when code reads the flag.

Test Procedure

From src/, run:

pnpm exec eslint --prune-suppressions --max-warnings=0 <changed files>

Results: type check passes. All tests in those folders pass. ESLint passes, and no suppression count changes.

New tests:

src/core/task/__tests__/ToolExecutionContext.spec.ts:
- A context exists for the tool call and stays readable after the result.
- A partial block and its complete block share one context.
- A rejection sets didReject only on its own context, not on a sibling.
- A rejection also sets didRejectTool.
executeCommand.spec.ts: two calls with different ids keep separate processes. A timeout aborts only the timed-out process.
readFileTool.spec.ts: a denied read sets didReject on the context.

ExecuteCommandTool’s changed rejection behavior still lacks focused coverage. The production path now calls markToolRejected(task, toolCallId) when either the primary terminal execution or the shell-integration fallback returns rejected (ExecuteCommandTool.ts:308-312 and 324-331). The command-tool fixture has didRejectTool and terminalProcesses, but no activeToolContexts.
