Description

This change adds the trusted fixture execution substrate and moves the Solheim provider smoke onto it. Current scope is substrate only.

Shared fixture substrate. scripts/agent-evals/README.md documents the versioned fixture contract, the trusted in-memory registry, the single real VS Code launcher and controller, and the structured trace. One controller runs one registered fixture inside one isolated extension host. The substrate carries no provider, task solution, or grading rule. Fixtures are trusted in-repo data with validated workspace files, one prompt, a tool policy, and enforced execution limits.

First fixture. scripts/solheim-smoke/README.md describes the smoke as a thin adapter on that contract. Real VS Code activates, accepts one deterministic no-tools task, and must show a nonpartial completion plus output-token usage. The existing command flow, 1280×720 12 fps recording, evidence gates and verdict.json are preserved. The shared controller now owns startup correlation and bounded teardown.

Execution trace. Each run uploads trace.json with traceSchemaVersion: 1:

  • Request rows upsert logically by message timestamp across started and updated request messages.
  • Usage, context and tool snapshots keep the latest event-sourced values and never sum cumulative snapshots.
  • Provider settings appear only as an explicit nonsecret projection with a SHA-256 digest, plus fixture and commit identity. The run fails closed before launch if the projection contains the credential.
  • Events before the task-start acknowledgment buffer under a fixed cap and replay only for the accepted task. After completion the controller drains late usage events within a bounded window.
  • Execution, accounting completeness and teardown outcomes stay separate. A teardown failure never changes an execution result.

Terminal code semantics. The trace keeps two completion signals apart:

  • COMPLETED requires the extension taskCompleted event. The extension emits it only after the completion result is accepted, or a delegated subtask hands back, and the turn is persisted. A stop request or a later session loss never changes this code.
  • The final completion_result say message is model output, not acceptance. execution.completionMessageSeen records it. It never produces COMPLETED.
  • An observer stop after that message reports STOPPED with adapterStopAfterCompletionMessage. An observer stop without it stays adapterStop. A manual loss stays SESSION_LOST. Output without a stop stays TASK_RUN_TIMEOUT. The bounded drain stays taskCompleted-only.

Honesty limits. Event-sourced numbers carry labels. Unknown or adapter-normalized values carry reasons such as notReported and are never filled with zero. Retry markers count messages, not HTTP retries. No exact provider billing claim is made. Prompts, model output, reasoning, tool arguments, error payloads and storage paths stay out of the trace. The video remains unredacted as documented.

Workflow posture. .github/workflows/solheim-provider-smoke.yml stays manual-only, restricted to refs/heads/main, pinned to the exact triggering SHA, with one credential-bearing step. .github/workflows/code-qa.yml now runs the offline agent-evals and smoke unit and type checks. Environment isolation is not a sandbox. Neither workflow runs arbitrary PR code with secrets.

Scope and follow-ups

This PR delivers substrate only. It does not deliver any coding-task evaluation. Planned follow-ups run in order: five frozen real coding tasks, independent credential-free graders, paired comparisons of exact trusted commits, repeated runs with uncertainty reporting, then advisory recurring release integration. None of these milestones exist yet. Nothing here blocks a release.

Security / reviewer notes

One credential-bearing step. GitHub permissions stay contents: read. The recorder and the VS Code child receive restricted environments rather than inherited provider or GitHub credentials. The provider key reaches the extension through IPC and may sit in temporary extension storage until teardown. That storage is never uploaded. Process and storage isolation is not a sandbox for malicious extension code. The smoke workflow runs trusted main only. No PR execution path carries secrets.

The recording can show the smoke UI, the fixed prompt and the model response. It is not text-redacted. No settings screen opens. Host logs, credentials, task configuration and storage files stay out of artifacts. The trace fails closed if the settings projection would include the credential. Cancellation or forced termination may prevent finalization and upload.

Existing environment and secret names are retained: final-vscode-review-smoke / FINAL_SMOKE_OPENAI_API_KEY. Repository configuration must restrict that environment to main and require approval. YAML alone does not establish those policies. The driver never approves dialogs or tool execution and closes the task during teardown.

Test Procedure

Offline checks passed in this round:

  • 107 tests passed in the latest round: 40 agent-evals unit, 39 smoke unit, 28 smoke workflow. The workflow suite now covers the driver process-owner stop budget and the atomic-final publication marker.
  • Agent-evals and smoke TypeScript checks passed. Prettier passed the touched files that have a formatter (record.sh has no Prettier parser; bash -n passes). The pre-push hook also ran the full monorepo type check (11 packages). No suppression count changed. ESLint does not cover scripts/.

One user-reported local live smoke run passed the verdict with completionSeen: true: total about 6390 ms, activation about 3010 ms, task about 3110 ms, 2719 input and 29 output tokens, two early buffered and replayed events, two cumulative usage snapshots, clean teardown. Its trace reported STOPPED with adapterStopAfterCompletionMessage, the expected terminal state for a headless smoke. This report is not independently verified. Raw artifacts are unavailable.

The synthetic reproduction in scripts/agent-evals/runner.test.ts mirrors that reported topology. It is clearly labeled synthetic in code and comments. It is not a sanitized actual user trace.

Reproduce the offline checks:

pnpm test:agent-evals:unit
pnpm agent-evals:check-types
pnpm test:solheim-smoke:unit
pnpm test:solheim-smoke-ci
pnpm solheim-smoke:check-types

First trusted-main workflow run is still pending. After merge and environment-policy verification, manually dispatch Solheim provider smoke on main, approve the environment deployment, and inspect verdict.json, trace.json and smoke.mp4. Earlier question-lane runs do not establish this refactor's live-CI behavior. Only a .github/workflows/solheim-provider-smoke.yml run on trusted main counts as verified CI evidence.

Documentation / visual impact

Setup, security constraints, artifacts and local commands are documented in scripts/solheim-smoke/README.md and scripts/agent-evals/README.md. No product UI, layout or visual snapshot changes are included. The video is a diagnostic aid, not a correctness oracle.

📝 Summary

Summary by CodeRabbit

  • New Features
    • Task starts initiated through IPC now receive a correlated response: successful starts include the task ID, while failures provide a sanitized message and, when available, the startup stage.
    • Added a manually run Solheim provider smoke check that reports its outcome and uploads results, including a screen recording. Smoke checks also run as part of code quality checks.
  • Bug Fixes
    • IPC-started tasks no longer shift focus to the sidebar by default.
    • Sensitive prompt, configuration, and validation details are no longer included in task-start or IPC validation logs.
  • Documentation
    • Added guidance for running the Solheim provider smoke check and reviewing its results.

This change adds correlated task-start IPC responses and a shared agent-evaluation runner. A Solheim smoke adapter uses the runner to classify task evidence and write a verdict. A manually dispatched workflow runs the smoke test, records the display, and uploads artifacts.

StartNewTask accepts an optional validated request ID. The extension sends a schema-validated success or sanitized failure for correlated requests. It logs request metadata instead of prompt and configuration values, and IPC starts do not focus the sidebar. IPC server logs omit received payload data and validation details.
Shared fixture execution and trace
scripts/agent-evals/*
The shared runner validates and registers fixtures, launches an isolated VS Code runtime, correlates task-start responses, and handles events, usage accounting, and bounded teardown. It writes a trace with execution and accounting data. Tests and documentation cover these behaviors.
Solheim fixture, evidence, and verdict
scripts/solheim-smoke/config.ts, scripts/solheim-smoke/config.test.ts, scripts/solheim-smoke/fixture.ts, scripts/solheim-smoke/task-start.ts, scripts/solheim-smoke/task-start.test.ts, scripts/solheim-smoke/evidence.ts, scripts/solheim-smoke/evidence.test.ts, scripts/solheim-smoke/run.ts, scripts/solheim-smoke/run.test.ts, scripts/solheim-smoke/driver.mts
The Solheim fixture configures a no-tools provider task. The adapter classifies accepted root-task evidence, maps outcomes to smoke codes, and writes a verdict. Tests cover the fixture, evidence, startup, and orchestration outcomes.
Workflow execution and recorded artifacts

Merge Risk: 🔵 Low · up to ace40

The smoke workflow itself runs only on main. The protected environment that holds the provider API key still allows deployments from any branch, so any job approved by an admin could read the key. The PR can merge with low risk once the owner adds a main-only deployment branch policy to the environment and keeps the required approval.

Security architecture risk: 🔵 Low · up to f34e5

The manual check is restricted to trusted main-branch code, uses temporary storage, and has limited permissions. Remaining uncertainty concerns live cleanup behavior and repository protection settings.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The new credential-bearing exposure is concentrated in one disposable CI session, its provider credential, and the uploaded visible UI. Main-only dispatch, an empty workspace, read-only GitHub permissions, and disabled persisted checkout credentials limit exposure to PR-controlled code and repository write authority; provider-side credential scope remains unspecified.

Trust Boundaries and Controls

  • observed — The new reply follows the existing client-ID routing map and contains only correlation metadata, the created task ID, or fixed failure fields. The transport still accepts the client ID asserted in command envelopes; that trust model predates this PR, and operating-system socket restrictions were not established by this review.
  • observed — The smoke selects empty tool groups and explicitly disables every always-available built-in tool except completion. Production dispatch applies disabled-tool requirements and mode authorization before executing complete tool calls, including MCP calls. The driver does not approve tool dialogs; this is application-level authority restriction rather than an operating-system sandbox.

Resilience and Maintainability Implications

  • observed — Teardown attempts task closure and escalates to process-group termination within bounded waits. A still-live direct child prevents storage deletion and is reported in the verdict. Existing smoke failures retain their original code, so storage-deletion errors are not independently reported. The exit gate tracks the directly spawned process, not independently verified descendant termination.
  • observed — The recorder receives a credential-free environment, captures a fresh non-network-listening display, and has duration, size, and shutdown bounds. Ordinary success and failure upload only the verdict and video for seven days; cancellation can prevent finalization or upload.

Hardening Proposals

  • proposed — Preserve the primary failure code while independently reporting storage cleanup and verifying whole-process-group termination. Validate these containment properties during the first live run, including interruption and startup-timeout recovery.

Explanation

The description covers the linked issue, implementation, scope, security constraints, testing, documentation, known limitations, and pending trusted-main verification. The pre-submission checklist and contact section are not completed, but the description is otherwise substantially complete.

Full details: Security Boundaries

Explanation

scripts/solheim-smoke/record.sh:105-108 executes arbitrary positional arguments with the wrapper's inherited environment. The workflow places SOLHEIM_API_KEY in that environment at .github/workflows/solheim-provider-smoke.yml:49-52. If a test, maintenance change, or local invocation runs record.sh with a command while the key is set, that command receives the key and can print or exfiltrate it. The comment that CI supplies no arguments does not enforce this boundary.

Resolution

Remove the arbitrary-argv execution mode from the credential-capable wrapper, or run test commands through a separate wrapper. If argv support remains, allowlist the exact test commands and launch them with a sanitized environment that explicitly excludes SOLHEIM_API_KEY and other credentials.

Full details: Persistence Integrity

Explanation

The smoke output directory can publish stale metadata. driver.mts creates or reuses SOLHEIM_SMOKE_OUT_DIR but does not remove verdict.json or trace.json before runSmoke; its catch block only logs SETUP_FAILED. record.sh removes only smoke.mp4. If a driver is killed during the documented KILL escalation, or fails before the artifact writes, a reused directory retains the previous verdict and trace. The workflow uploads those exact paths with no freshness check, so it can associate old state with the new recording/run.

Resolution

Clear verdict.json, trace.json, and pending artifact files before each run, or write each attempt into a unique directory and publish only that attempt’s files. On setup, write, or termination failure, remove stale finals or write a fresh failure trace and verdict. Add a run-identity check before upload so stale metadata cannot be accepted.

Full details: Regression Evidence

Explanation

The prior regression finding remains unresolved. runSmoke passes its apiKey to runAgentFixture (scripts/solheim-smoke/run.ts:38-48), and an empty key enters the runner's credentialMissing setup path before runtime.launch() (scripts/agent-evals/runner.ts:221-225). The smoke adapter tests call runSmoke only with "private-test-key" (scripts/solheim-smoke/run.test.ts:164,193,204,211,221,233,240). The setup scenario tests a launch exception, not an empty key. The lower-level runner test covers empty credentials, but it does not cover the smoke adapter's SETUP_FAILED verdict or verdict.json output.

Resolution

Add a dedicated runSmoke(runtime, "") test. Assert that the fake runtime does not launch, the verdict has code === "SETUP_FAILED" and passed === false, and writeArtifact("verdict.json", ...) receives that verdict. Keep the launch-exception setup test separate.

Full details: Lifecycle Resource Cleanup

Explanation

Startup cancellation remains unsafe in the changed runAgentFixture path. The interrupt handler at runner.ts:149-156 sets sessionLost but only signals host when one exists. During await runtime.launch(...) at runner.ts:230, host is undefined. If launch later returns a ready host, the code does not re-check sessionLost before sendCommand(StartNewTask) at runner.ts:240-260, so cancellation can start a task. If launch remains pending, the finally block cannot run and the unsubscribe listener at runner.ts:351 remains registered. The changed launcher performs asynchronous filesystem setup and VS Code download work at launcher.ts:57-81, so both paths are concrete.

Resolution

Track cancellation independently of host. Add an abort or cancellation mechanism to AgentRuntime.launch where supported. After launch returns, signal the host and skip connection and task startup when cancellation occurred. Re-check cancellation after connection and readiness waits and immediately before sendCommand(StartNewTask). Ensure the interrupt listener is removed on every startup-cancellation path, including a launch that remains pending, and clean up any launch resources.

Confirm that the environment restricts deployments to main. This workflow uses workflow_dispatch, so a branch version can remove the github.ref condition and pass branch-controlled code to the smoke step, which receives FINAL_SMOKE_OPENAI_API_KEY. Before merge, confirm that final-vscode-review-smoke blocks non-main branches and requires approval. An approval requirement alone does not block those branches.

There are two let taskId declarations in the same try block: one at Line 87 and one later. Only one appears in the shown code, so the code is valid. Evidence is collected only after waiter.onTaskEvent returns the accepted ID. This means events delivered before the response are dropped, and that is the intended gating. No change is needed.

The generic runner listened only for the taskCompleted event while the
smoke completionSeen gate observes the final nonempty completion_result
say message. The extension emits taskCompleted only after the result is
accepted and the turn is persisted, so a headless smoke can stop before
that event. Record the completion message as execution.completionMessageSeen,
report an observer stop after it as adapterStopAfterCompletionMessage, and
keep COMPLETED reserved for the accepted taskCompleted event. A manual
loss stays SESSION_LOST, output alone stays TASK_RUN_TIMEOUT, and the
bounded drain stays taskCompleted-only. Add a clearly labeled synthetic
reproduction of the reported two-pre-ack-event topology.

Comment thread scripts/agent-evals/runner.test.ts
The recorder wrote ffmpeg straight to the final smoke.mp4. A failed run
could leave a non-empty invalid MP4 at the uploaded path, and the
workflow could not report a missing video through one multi-path
if-no-files-found upload.

- write ffmpeg to a unique temporary file inside SOLHEIM_SMOKE_OUT_DIR,
with the mp4 muxer forced for the suffix-less name
- publish through validation only: non-empty plus ffprobe, then an
  atomic same-directory rename onto smoke.mp4
- remove a stale final before recording starts, and remove the
temporary file on every failure, interrupt, and validation failure
- treat a recorder that stopped on its own with a non-zero status as a
recording failure even when the fragments still probe; a recorder
alive at shutdown is judged by content validation, because ffmpeg
exits non-zero after its graceful INT or TERM stop
- write the outcome to recording-status.txt, keep the provider exit
code unchanged, and stop the provider during signal teardown
- upload metadata and the recording as separate artifacts, with an
explicit recording integrity check, so a missing video fails its own
  steps
- extend the stub-based record.sh behavior tests to cover publication,
invalid and stale output, recorder self-exit, and signal teardown
… stop

Add the negative case for the completion filter. A partial stream, a
whitespace-only final text, and an ask-side completion_result stay observed
by the fixture observer but never mark completionMessageSeen. The stop
keeps code STOPPED with stopReason adapterStop, and the written trace
records the same values.

(SOLHEIM_SMOKE_STOP_BUDGET_S, default 30s) reach the driver PID that
owns teardown and the atomic verdict and trace writes. KILL stays the
last resort for a hung driver and can leave host processes behind.

The redundant recording-status.txt marker is removed instead of made
transactional. The published smoke.mp4 is the commit marker: the
recorder removes a stale final at start, validates the temp recording
(nonempty plus ffprobe), then renames it into place atomically. The
workflow checks the final is nonempty and keeps metadata and video
uploads separate, so a missing video stays explicit and failure
metadata survives.

The KILL-escalation test now writes a static driver stub and passes both scratch paths as command arguments, read via process.argv. Quote and backslash characters in the paths guard the argv handoff. No runtime or workflow changes.

Requesting changes because the credential-bearing final-vscode-review-smoke environment must enforce main-only deployments and reviewer approval before receiving the provider secret.

timeout-minutes: 25
# Retain the existing protected environment and secret names for compatibility.
# Repository policy must restrict deployments to main and require approval.

Could we require main-only deployments and reviewer approval on this environment before it receives the provider secret?

retain their existing names to avoid an unnecessary repository-settings migration. The environment
requires the same reviewer approval as the `marketplace-production` deploy environment: a member of
the `Zoo-Code-Org/admins` team must approve each deployment before GitHub releases the secret. The
main-only deployment restriction stays in the workflow: the smoke job runs only on `refs/heads/main`.

Require a main-only deployment policy for final-vscode-review-smoke.

The workflow guard limits only this job to refs/heads/main. The environment has no deployment branch policy, so another approved job could reference it without that guard. Configure the environment to allow deployments only from main, and retain the required reviewer approval.
