Description
Response sheet columns have moved around in the past year, and resyncing the spreadsheet to the form caused the columns to get updated properly. So now, the columns don't match the old format used in the workflow
Indexing has now been fixed with this PR; examples updated as well
Changes have been tested locally and works as expected.
