Supersedes #796 by @Ayush7614. It carries all six of that PR's commits, with authorship preserved, rebased onto current main, and adds one fix on top.
From #796 (unchanged apart from the rebase, which git range-diff shows as patch-identical):
Dispatch advisory specialistStart / specialistStop hooks from specialist accounting. specialistStart fires only after the child has actually started. specialistStop fires once per terminal stop through a durable specialist_stop_hook claim that follows the accounting run-ID equivalence. That covers completion, error, a recovered TaskStop, and a TaskOutput reap, including from a later process.
Allow specialist-name matchers on these events. The docs already advertised them, but LoadConfig rejected them.
Attach the hooks dispatcher to the specialist tools in both the interactive and the headless startup paths.
These events were documented and accepted by zero hooks add, but they never fired at runtime.
New in this PR (ae225b59): record specialistStop only after the orphan kill succeeds. This is the outside-diff item CodeRabbit raised on #796's last head, which @Vasanthdev2004 also flagged.
In runBackground, if SetPID fails after the child starts, the child is killed and the stop is recorded. If that kill (TerminateProcess) failed too, the code still did four things:
marked the task errored,
recorded the stop,
took the one-shot durable claim,
fired specialistStop,
even though the child might still be running. The hook then ran while the sub-agent was alive, and the claim blocked the hook when the child really exited. Marking the task non-running also let a TaskOutput or TaskStop reap take the claim before the launcher's Wait saw the exit.
Root cause: the branch treated "we tried to kill it" as the terminal transition. Now a failed kill returns the joined error, and the status update and stop are left alone. The task stays running, so any reaper does nothing. The deferred startSettled close then lets the launcher's exit callback mark the task exited and record the single stop when the child ends. A successful kill still records the stop right away, as before.
Only this branch changed. Durable-claim machinery, reapers, the hooks package, foreground paths and docs are unchanged.
Review follow-up (9a9b533f), which resolves the CodeRabbit/Copilot threads on ae225b59:
Prompt file on a failed kill. The failed-kill branch also deleted the --file prompt. A surviving child that had not read it yet would then fail at startup. That branch now leaves the prompt alone. The exit callback removes it when the child really exits, and Runtime.Close removes tracked prompts at shutdown.
One stop decision per bridge.dispatchSpecialistStopHook had two independent claims: the durable specialist_stop_hook event, and the in-process claim used when the session store errors. A won durable claim never recorded the in-process claim. So on one bridge, a transient store error before or after a durable claim dispatched the same stop twice. A won durable claim now also takes the in-process claim, and the hook runs only if both are free. The ledger format, the reapers, and the swarm path are unchanged.

make lint-static (advisory) reports 4 staticcheck items, all in files this PR does not touch (internal/proxydial, internal/tools/web_fetch.go, internal/installtest).
New regression test: TestRunBackgroundSetPIDFailureRecordsStopOnlyAfterTermination runs Executor.Run with a SetPID failure. It has two cases:
Kill succeeds (positive control): the stop and the claim are recorded immediately.
Kill fails: Run surfaces terminate orphaned pid, only specialistStart fires, no stop event or claim is recorded, and the task stays running. A reap at that point records nothing. After the child exits, the exit callback records exactly one stop and one claim and fires specialistStop once.
On #796's head with the early return removed, the test fails:
--- FAIL: TestRunBackgroundSetPIDFailureRecordsStopOnlyAfterTermination/kill_fails
accounting_test.go:869: lifecycle hooks after failed kill = "specialistStart, specialistStop", want only specialistStart
The earlier review requests on #796 are still covered by their tests:
Both follow-up regressions fail on ae225b59: prompt file removed while the child may still be running, and stop hooks = 2, want 1 across durable and fallback claims in both orders.
Specialist lifecycle hooks can be matched by agent type for specialistStart and specialistStop events in both interactive and headless runs.
Start hooks run only after a child process starts. Stop hooks run once for eligible terminal stops, including completed, failed, stopped, or reaped tasks.
Behavior
Lifecycle hook failures are recorded but do not interrupt a run. Tool-hook failures retain their existing behavior.
Documentation
Updated hook guidance and command help to explain matcher support and lifecycle behavior.
These events were documented and accepted by hooks add, but never fired.
Wire advisory dispatch from specialist accounting and allow specialist
name matchers so EXTENDING.md matches runtime behavior.
Release accountingMu before running lifecycle hooks, and dispatch
specialistStop only when the stop event is newly recorded so concurrent
finishers cannot re-run arbitrary hook commands.
Decouple stop-hook dispatch from parent-session event append so swarm
members (empty parent) and TaskOutput still get exactly one hook via a
shared LifecycleHooks claim, and wire that bridge into TaskOutput.
Address the review findings on specialist lifecycle dispatch:
- specialistStop is now claimed in the parent session's event log via a
record-once specialist_stop_hook event, sharing the accounting run-ID
equivalence (empty runId is a catch-all). A later reaper in another
process (TaskOutput poll, recovered TaskStop, restart) no longer re-runs
the hook. The in-process claim remains only as fallback for swarm
members with no parent session and for session-store errors.
- Recovered TaskStop records the terminal transition after Kill, so an
explicitly stopped recovered task emits specialistStop exactly once.
- specialistStart fires only after the child process actually started
(onStarted after Start()); a launch that fails before exec records
attempt accounting but no public start/stop hooks. Reapers derive the
same eligibility from the durable stop event's childStarted flag.
Adds a CLI startup wire-up test asserting both real entry points
(interactive TUI, headless exec) attach the lifecycle bridge to the same
LifecycleHooks the TaskOutput tool holds, fixes the stale import path on
the rebased branch, and documents the once-per-run semantics.
A background child that exits immediately can run its launcher exit
callback before Executor.LaunchBackground returns, so the terminal
accounting could claim and dispatch specialistStop before runBackground
dispatched specialistStart, and the recorded stop missed the pid that
SetPID had not stored yet.
The exit callback now waits on a settled channel closed when runBackground
returns (after the start hook and SetPID), from its own goroutine so a
synchronous injected launcher cannot deadlock waiting for a signal only
the launch return can close.
Regression test: TestRunBackgroundDefersExitUntilStartDecisionCompletes
drives a synchronous fast-exit launcher and asserts start-before-stop hook
order, the recorded stop pid, and no deadlock. It fails on the previous
code with hooks "specialistStop,specialistStart".
When SetPID fails after a background child has started, runBackground kills
the unrecorded child and records the stop. If TerminateProcess fails, the
child may still be running, but the branch still marked the task errored,
recorded the stop, took the durable specialist_stop_hook claim and fired
specialistStop. The hook could then run while the sub-agent was alive, and
the one-shot claim suppressed the hook on the child's real exit. Marking the
task non-running also let a TaskOutput or TaskStop reap claim the stop
before the launcher's Wait observed the exit.
On a failed kill, surface the joined error and return without the status
update or the stop. The task stays running, so reapers remain no-ops, and the
launcher's exit callback marks it exited and records the single stop when the
child ends. A successful kill keeps the immediate stop as before.

The change enables matcher support and dispatch for specialist lifecycle hooks. It records confirmed child starts and terminal stops, and shares the lifecycle bridge across CLI entry points and background task tools. Stop-hook claims prevent duplicate dispatch across task-stop and task-output accounting.
Matcher validation and selection now support specialistStart and specialistStop. The documentation and CLI help describe matcher behavior and lifecycle-hook handling.

The executor dispatches start hooks after confirmed starts. Accounting records whether a child started and claims stop hooks durably, with an in-process fallback. Tests cover launch failures, concurrent stops, run-ID matching, and foreground and background execution.

The CLI attaches its dispatcher to the shared lifecycle bridge. TaskOutput and TaskStop use the bridge when accounting for background-task stops. Tests cover CLI startup paths and task-tool accounting.
This change dispatches the documented specialistStart and specialistStop hooks and allows specialist-name matchers. The earlier concern about deleting the prompt file after a failed kill is reported fixed. No outstanding merge-blocking risk was found.

🟡 Changes recommended
Unresolved lifecycle issues can duplicate or prematurely fire stop hooks, dispatch hooks for children that never started, and delete prompt files while a child is still running.
…p claim
When SetPID fails and terminating the unrecorded child also fails, the child
may still be running and the task stays running. That branch still deleted
the --file prompt, so a child that had not yet read it failed at startup.
The launcher's exit callback already removes the prompt once the child
really exits, and Runtime.Close removes tracked prompts at shutdown, so the
failed-kill branch no longer removes it.
dispatchSpecialistStopHook made two independent dispatch decisions: the
durable specialist_stop_hook claim, and the in-process claim used when the
session store errors. A won durable claim never recorded the in-process
claim, so on one bridge a transient store error before or after a durable
claim dispatched the same stop twice, breaking the documented at-most-once
per process. A won durable claim now also takes the in-process claim
(including for an ineligible stop), and the hook runs only if both are free.
