Summary

Fix settings-lock cleanup that can delete a successor transaction's lock, preserve real skill-source watcher coverage without assuming exact filesystem callback counts, allow the plan-mode PowerShell permission test to finish native-parser retries, and add CodeQL analysis for fork pull requests.

The repository's managed analyses have not run on fork PRs, leaving the required Code Quality result pending. Add an explicit pull_request workflow for JavaScript/TypeScript and GitHub Actions. JavaScript/TypeScript runs default security queries plus quality queries, reporting through code scanning.

Impact

  • User-facing: settings transactions retire their owned lock before recursive cleanup, preventing an old release from interfering with a new writer.
  • Developer/maintainer: the skill watcher test checks source eligibility and fresh paths while tolerating valid callback batching. The PowerShell denial test has a 25-second deadline covering both permission checks and their bounded parser retries; production timeouts and the denial assertion are unchanged. Fork PRs gain a checked-in CodeQL workflow with pinned official actions, restricted permissions, and no dependency installation or privileged fork execution.
  • Maintainer activation: .github/codeql/README.md contains the staged rollout and exact ruleset fragments. This PR does not activate repository settings. An administrator must disable CodeQL default setup, verify both uploaded analyses, replace the managed Code Quality requirement with the two CodeQL job checks, and enable CodeQL findings-based protection after the new main baseline is uploaded. Unrelated protections and the quality error threshold are preserved.

Testing

  • Required local preflight completed on the final repair tree (Bun 1.3.13, Node 22.23.1).

  • bun install --frozen-lockfile — passed.

  • bun run check — passed: any-budget, build/smoke, deadcode, all eight test shards and conversation-arc; 10,428 passed, 4 skipped, 0 failed, with completed test reports.

  • bun run typecheck and bun run typecheck:type-tests — passed.

  • node bin/openclaude --version and NODE_DISABLE_COMPILE_CACHE=1 node bin/openclaude --version — passed.

  • bun run test:provider — 1,713 passed, 41 skipped, 0 failed.

  • npm run test:provider-recommendation — 167 passed, 0 failed.

  • Canonical upstream fetch and bun run security:pr-scan -- --base FETCH_HEAD --head HEAD — passed on the final commit.

  • The original deterministic lock regression fails before the release correction and passes after it; existing transaction and watcher coverage is retained.

  • The PowerShell timeout reproduction uses a controlled stalled executable through the real detection/parser/permission path: the original test fails at five seconds; the corrected test reaches its existing denial assertion after about 20 seconds without a CLI timeout override. Local native PowerShell is unavailable; CI provides the hosted native-runtime check.

  • Focused permission and neighboring PowerShell suites: 122 passed, 0 failed. Temporary test configuration and fixtures were isolated from the enclosing workspace; no assertions or production policies were weakened.

  • actionlint 1.7.12 passed for the new workflow. Independent focused review found no actionable fork-permission, query-configuration, or rollout issues.

  • Verified the prepared rules preserve the existing unrelated rules and require the exact new job names from GitHub Actions.

  • Live CodeQL upload/merge-gate validation is blocked by upstream settings. Current jatmn access is Write; setup API access returns 403. Until an administrator completes activation, custom CodeQL uploads can fail because default setup is still enabled, and the old managed Code Quality gate remains pending.

Notes

  • Read CONTRIBUTING.md and AGENTS.md; the maintainer explicitly requested including the CI workflow and ruleset preparation in this existing PR.
  • Provider/model behavior is unchanged; required provider suites validate integration. No UI changes; screenshots are inapplicable.
  • Linux local validation does not reproduce the supported Node CI matrix or native Windows/macOS. Website checks are inapplicable to these source/test changes and the isolated CodeQL workflow and maintainer guide.
  • Final repair commit: 0b390c3 (tree bad31df651e4d2666c5281eb49e23f67d6db719d). The latest commit changes only the existing PowerShell test deadline and its explanatory comment; all required local gates above passed on that exact source tree. The follow-up GitHub CI run is pending.

Review in Change Stack →

📝 Walkthrough

The change updates transaction lock cleanup and skill watcher test assertions. It adds a CodeQL workflow and rollout documentation. It also extends the timeout for a mutating-PowerShell permissions test.

Changes: Transaction Lock Cleanup

Layer / File(s) Summary
Retire owned lock before removal
src/utils/settings/settingsFileTransaction.ts, src/utils/settings/settings.transaction.test.ts
releaseOwnedLock renames the active lock directory to a token-specific pending path before removing it. The regression test exercises successor lock publication during cleanup.

Skill Watcher Test Assertions

Layer / File(s) Summary
Assert hooks by changed path
src/utils/skills/skillChangeDetector.test.ts
The test records hook paths and retries edits until the target path is observed. Hook assertions use recorded paths instead of mock call counts.

CodeQL Workflow

Layer / File(s) Summary
Run CodeQL and document rollout
.github/workflows/codeql.yml, .github/codeql/README.md
The workflow runs CodeQL analysis for JavaScript/TypeScript and GitHub Actions, then uploads results. The README describes workflow triggers, query suites, fork-PR limitations, and staged ruleset changes.

Permissions Test Timing

Layer / File(s) Summary
Allow parser retries in test
src/utils/permissions/permissions.test.ts
The mutating-PowerShell test now has a 25-second timeout. Comments document the parser retry allowance for the permission check and plan-mode guard.

Full details: Description check

Explanation

The description follows the required template and covers the changes, impact, testing commands and results, limitations, and follow-up activation steps. It also documents unavailable native PowerShell validation and blocked live CodeQL validation.

Full details: No Hidden Policy Change

Explanation

No hidden policy change found. The settings, watcher, and permission diffs change cleanup, test observation, and test timing only. The new CodeQL trust and permission model is an explicit PR objective, uses pull_request with read-only contents and scoped security-events: write, and is documented with administrator-only activation steps. The PR does not change repository settings or application routing, telemetry, network defaults, or product permissions.

Full details: Risk Surface Disclosed

Explanation

The PR touches CI permissions and CodeQL fork-PR execution. The workflow uses pull_request and grants only contents: read plus security-events: write; it does not run privileged fork code. The PR description states that repository settings are not activated and that uploads and the old Code Quality gate remain pending, but it does not explicitly classify CodeQL setup and ruleset migration as pre-merge blockers or post-merge prerequisites. The checked-in .github/codeql/README.md contains that rollout detail, but the available posted review contains no equivalent statement. The standing disclosure gap remains.

Resolution

Add an explicit statement to the PR description or review: CodeQL default-setup migration and ruleset changes are post-merge prerequisites, and this introducing PR may merge only after both CodeQL jobs complete; while settings remain unchanged, the old Code Quality gate remains pending and CodeQL uploads may fail.

The cited test overrides both reloadDebounce and reloadCooldown to 10 ms. Therefore, the comment's claim that a cooldown longer than 150 ms can affect this test is not supported.

src/utils/settings/settings.transaction.test.ts: 1

294-305: 🎯 Functional Correctness

The path assertion is not required.

The test starts with a new temporary root. The outer transaction publishes its pending lock successfully, so no pending-lock cleanup runs before releaseOwnedLock removes the retired path. The nested transaction also publishes successfully. Therefore, the first intercepted rmSync call is the outer release cleanup, not pending-lock cleanup.

The proposed assertion does not address a reachable failure in this test.

Run fork scans from a trusted workflow

For a forked pull_request, GitHub downgrades security-events: write to read-only unless the repository enables write tokens for fork workflows. github/codeql-action/analyze requires that permission for its upload. Therefore, these runs can finish analysis without publishing CodeQL results, so the fork-PR scan objective is not met.

Do not enable write tokens for this pull_request workflow. Fork-controlled workflow code would receive a write-capable token. Use pull_request_target from the base branch, check out the PR merge ref, and retain build-mode: none without executing PR commands.
