repair(msdmd): close post-114 maintenance contract gaps

Scope

Close the post-#114 documentation/maintenance defects without changing MSDMD reader behavior:

• add the canonical --snapshot-identity --strict regeneration path and native-reader runtime install steps
• keep gaps scoped to expected-block adoption and point required native obligations to requirements/diagnostics
• restore the lexical limitation warning for --legacy-blocks-only
• narrow the README unsupported-convention claim so unrecognized dialects are not treated as observed unknowns
• document the external native-reader runtimes and strict/non-strict exit semantics
• restore the distribution/activation rules and terminal propagation hmmm dropped by docs(msdmd): align public contract with native readers #114
• add a regression test for those public maintenance invariants
• regenerate skill-lib_msdmd.ts with the canonical collector against the intended final tree
