Summary
Why
The 0.1.4 release packaged the documentation engine but exposed only one umbrella skill in marketplace UIs. A genuinely fresh Codex task could also stop at an unavailable result when Doctor encountered a repository without a maintained docs map. The prior automated coverage validated the engine and generated adapters, but it did not exercise the complete installed-plugin flow through a fresh Codex Desktop task.

Root cause addressed
The generator treated the marketplace distribution primarily as one umbrella skill and lacked a complete Codex marketplace package boundary. Doctor also treated the absence of a maintained map as a terminal measurement boundary even when bounded root-orientation evidence was safely available.
