fix: 丢掉空助手消息，并把模型输出预算提到环境变量

改了什么 / What changed

• 正文和 tool_calls 都空的助手消息不再落库，发给模型前也会丢掉；请求被取消时，空流不再当成成功回复。带 tool_calls 的空正文仍保留。
• 模型预算改为环境变量，默认输出上限从 8192 提高到 65536，避免思考把正文预算耗尽。同时提高输入上限、轮数、工具次数和墙钟，并增加 LLM_THINKING。
