Implements remediation PR 4: Darwin/Linux process identity, fail-closed PID fallback signaling, store-free recovery commands, stderr logger fallback, bounded private log rotation, orphan worktree cleanup, and removal of test-only helpers. Darwin amd64/arm64 compile checks pass.
