Implements remediation PR 3: one shared backend snapshot per refresh, zero synchronous GitHub checks on command paths, bounded non-overlapping asynchronous PR refresh, freshness caching, and field-level PR persistence. Includes concurrency, stale-cache, and no-network-path tests and benchmarks.
