[codex] improve TUI markdown scroll and mouse

Summary

• replace the hand-rolled markdown preview with pulldown-cmark parser support
• route mouse wheel scrolling across notes, story preview/list, palette, and settings
• add terminal mouse mode via right-click or Alt+M for native select/copy/paste
